What is the American University of the Caribbean School of Medicine?

AUC is a private medical school in Sint Maarten awarding a Doctor of Medicine built on the United States model: five semesters of medical sciences, then 80 weeks of clinical rotations in affiliated teaching hospitals in the United States and the United Kingdom. It was founded in 1978 by the American educator Paul Tien.

The school did not start where it is now. Its first campus stood on Montserrat, and the eruption of the Soufriere Hills volcano forced an evacuation in 1995; the buildings were destroyed by pyroclastic flow in 1997. AUC had already bought land in Cupecoy on the Dutch side of Sint Maarten and begun construction in July 1996, and the replacement campus opened on 1 May 1998. In 2011 the group now trading as Adtalem Global Education bought the school for 235 million US dollars.

One more date matters. Hurricane Irma crossed Sint Maarten on 5 and 6 September 2017. Campus damage was limited, the island's infrastructure was not, and AUC moved the semester to England, resuming classes on 29 September 2017 in Preston. That emergency is the origin of today's United Kingdom Track.

Where is the AUC campus in Sint Maarten, and what is on it?

The campus sits in Cupecoy, at the western end of the Dutch side of Sint Maarten, about 150 miles from Puerto Rico. Students spend five semesters there. It is purpose-built, not converted.

AUC lists lecture halls with lecture-capture recording, clinical examination rooms for standardised-patient work, a simulation laboratory with a Harvey cardiopulmonary simulator, an anatomical dissection laboratory open around the clock, an applied research laboratory, a medical library with online journal access and 24-hour security. The round-the-clock anatomy access is the detail that matters most day to day: with three intakes a year, laboratory availability decides whether a cohort can revise on its own schedule.

Who accredits AUC, and what does that accreditation cover?

AUC is accredited by the Accreditation Commission on Colleges of Medicine, the accrediting body used by the country of Sint Maarten. The school announced continued accreditation for a six-year term and states that the accreditation runs through 2027, following an on-site inspection visit.

Two recognitions sit above that. The National Committee on Foreign Medical Education and Accreditation, part of the United States Department of Education, has determined that the Commission's standards are comparable to those of the Liaison Committee on Medical Education, which accredits medical schools inside the United States. The World Federation for Medical Education separately recognises the Commission, a recognition AUC records as running through 2026.

Keep the two storeys apart. The Commission accredits the school; the National Committee and the World Federation recognise the Commission. The upper storey is what regulators outside Sint Maarten actually read.

What do the World Directory listing and Intealth's Recognized Accreditation Policy mean in practice?

AUC is listed in the World Directory of Medical Schools, published jointly by the Foundation for Advancement of International Medical Education and Research and the World Federation for Medical Education. That listing, together with an ECFMG Sponsor Note, is the precondition for applying for ECFMG Certification at all. No listing, no certificate.

Intealth, the parent organisation of the Educational Commission for Foreign Medical Graduates, implemented its Recognized Accreditation Policy on 20 November 2024, having first announced it in 2010. A school has recognized accreditation when its accrediting agency has itself been reviewed by a quality-assurance organisation approved by Intealth. Two are approved, covering more than 50 accrediting agencies worldwide: the World Federation for Medical Education and the National Committee on Foreign Medical Education and Accreditation.

The widely misread part comes next. Intealth stated on implementation that graduating from a school without recognized accreditation does not remove eligibility for ECFMG Certification. Schools meeting the policy receive a notation in the World Directory, and from the 2026 application season a general statement appears in ECFMG Status Reports so residency programme directors can see it. It is a data point for directors, not a gate. For AUC the reading is simple: the Commission is recognised by both approved organisations, so the school sits inside the policy. Recognition dates run for fixed terms, so re-check the directory each year.

How is the AUC MD programme structured?

The degree runs in two blocks. Five semesters of medical sciences come first, taught as an organ-systems curriculum running from a foundational module into the cardiovascular, respiratory, gastrointestinal, endocrine and neurological systems, with clinical reasoning alongside from the start. Then 80 weeks of clinical training in affiliated hospitals.

Between the blocks sits a hard requirement: a pass in USMLE Step 1 before clinical rotations begin. AUC reports a 79 per cent first-time Step 1 pass rate for 2021 to 2025, and a 93 per cent first-time pass rate on Step 2 Clinical Knowledge for 2020 to 2024.

The clinical block divides into 42 weeks of core rotations and up to 38 weeks of electives. Cores are internal medicine and surgery at 12 weeks each, then paediatrics, obstetrics and gynaecology, and psychiatry at six weeks each. Electives cover subspecialty exposure or, through the Northwell Health affiliation, six-week global health placements in eight countries including Vietnam, Botswana, India and Uganda. The 2026-2027 academic calendar gives 31 August 2026 as the first day of the September semester and 4 January 2027 for the January one.

Where do AUC students do their clinical rotations?

Not on the island. AUC lists 16 affiliated teaching hospitals in the United States and eight in the United Kingdom as of 2026. The American sites cluster in New York, Connecticut and Michigan, with further sites in Louisiana, Maryland, Illinois and Florida; named examples include BronxCare Health System and The Brooklyn Hospital Center in New York and Norwalk and Danbury Hospitals in Connecticut.

The British network covers Aintree University Hospital in Liverpool, Chelsea and Westminster and West Middlesex in London, Epsom General in Surrey, Wexham Park in Slough, Stepping Hill in Stockport and St. Richard's in Chichester. Where a student rotates shapes more than convenience: a hospital that also runs residency programmes puts the student in front of the physicians who later write letters.

What is the AUC United Kingdom Track, and how does it differ?

The United Kingdom Track is the same MD with the first two years relocated. The five medical sciences semesters are taught in Preston, England, on the University of Lancashire campus under a partnership with AUC. The track is open to non-United States citizens.

After Step 1, United Kingdom Track students choose where to spend the 80 clinical weeks: the eight British affiliated hospitals, or the 16 American ones. AUC reports that the first cohort completed in 2023-2024. What began as a hurricane contingency is now a standing option, and it changes the arithmetic for a European applicant: two years in northern England instead of two in the Caribbean, with no Sint Maarten residence permit.

How do transfer students and doctors with an existing degree fit in?

AUC accepts students already enrolled at another accredited medical school and reviews each application individually to decide where in the curriculum that student joins. Placement rests on completed coursework and academic record, assessed file by file. No fixed conversion table, no published formula. A second route runs for people who already hold a primary medical qualification: AUC operates an MBBS to MD pathway admitting qualified doctors with advanced standing into the medical sciences curriculum.

Two things are worth understanding here. Recognition is decided per faculty on the individual file, which is why the same transcript produces different placements at different schools. And the Educational Commission for Foreign Medical Graduates applies its own rule on top: a candidate must have completed at least four credit years at the World Directory-listed school issuing the diploma, with transferred credit documented and verifiable. Prior study is assessed twice.

Which United States medical boards recognise AUC, and what do those approvals govern?

Four jurisdictions come up repeatedly. They govern two different things: some approvals concern the graduate's licence, others concern where the school may teach.

The Medical Board of California has approved AUC, which is what allows its graduates to apply for a California licence; its review of international medical schools is among the most demanding in the United States. The Texas Medical Board has determined that AUC graduates demonstrate substantial equivalence of their education, also a licensure-side finding.

New York and Florida are different in kind. The New York State Education Department has approved AUC to place students in clinical clerkships at affiliated New York hospitals, and Florida approval permits clerkship instruction there under Florida statute. Those are teaching approvals: they govern whether the school may run rotations in the state, not whether a graduate may hold a licence. A student rotating at Flushing Hospital Medical Center is there because of the New York approval, not a licensing decision.

What does AUC cost in the 2026-2027 academic year?

Semester by semester. AUC publishes the 2026-2027 medical sciences semester at 23,909 US dollars in tuition plus 6,019 in administrative fees plus 1,657 for health insurance, giving 31,585. The clinical semester is 31,275 plus 4,268 plus 1,657, giving 37,200.

Movement is visible over three years. The fee schedule effective September 2023 put the medical sciences semester at 21,568 US dollars in tuition and the clinical semester at 28,146. Against the 2026-2027 figures that is a rise of roughly 11 per cent on each.

A worked example shows the shape of it. Take a student called Ana who starts the Sint Maarten route in September 2026, shares a dormitory room for all five medical sciences semesters, and pays every published item at the 2026-2027 rate.

That is the published schedule and nothing else. It excludes flights, food, books, USMLE and ECFMG fees, Occupational English Test fees and living costs during the clinical years. AUC also operates scholarships, and financing differs sharply by citizenship, so the number a given student pays is rarely the number on the schedule.

What licensing route does an AUC graduate follow in the United States?

Four things happen, in order. Pass USMLE Step 1 and Step 2 Clinical Knowledge. Satisfy the clinical and communication skills requirement through one of the Pathways. Have the diploma verified by the Educational Commission for Foreign Medical Graduates directly with the issuing school. Then enter the National Resident Matching Program.

The 2026 ECFMG Information Booklet sets out the certification requirements: the school must be listed in the World Directory with an ECFMG Sponsor Note covering the graduation year, the applicant must have completed at least four credit years at that school, and the final diploma and transcript go through primary-source verification with the school itself.

Pathways is where international graduates most often lose time. For the 2027 Match cycle, Intealth lists six Pathways, ranging from holding a licence to practise in another country, through graduating from a school accredited by a World Federation-recognised agency, to a clinical evaluation by licensed physicians using the Mini-CEX instrument. Every Pathways applicant, with no exception for native speakers or English-language instruction, must reach satisfactory scores on all four sub-tests of the Occupational English Test Medicine. The dates are firm: that test by December 2026, the Pathways application and supporting documents by 31 January 2027, Eastern Time. A seven-year limit runs from the first passed examination.

Only then does the National Resident Matching Program apply. It is an algorithmic match, not an application queue: applicants and residency programmes each submit ranked preference lists, and the algorithm produces binding placements announced on Match Day each March.

What has to happen between an offer and the first day of class?

Nothing here is a university deadline. These are government and logistics timings, and they are the ones that slip. Work backwards.

Can an AUC degree be used outside the United States?

Yes. The mechanism differs from the American route, though. AUC states that its graduates are eligible to apply to practise in the United States, the United Kingdom, Canada and elsewhere, with more than 9,000 alumni across those countries on the school's 2026 figures.

The British route runs through the General Medical Council rather than ECFMG. A doctor with an international primary medical qualification registers by showing it comes from a school listed in the World Directory of Medical Schools, evidencing English language competence, and satisfying the Council's knowledge and skills requirement, usually through the Professional and Linguistic Assessments Board examination or an accepted postgraduate qualification. The same directory line underpins both routes.

United Kingdom Track students should be clear on one detail. The degree awarded in Preston is a Doctor of Medicine from a Sint Maarten medical school, not a British MBBS from the University of Lancashire. Teaching location does not change the awarding institution, and General Medical Council registration follows the international-qualification route wherever the pre-clinical semesters were taught.
