Practising Medicine in Australia: The AMC Route for International Medical Graduates in 2026
02.09.2026
Which pathway to medical registration in Australia applies to an international medical graduate in 2026?
The Australian Medical Council operates four assessment pathways to medical registration in Australia in 2026: the standard pathway, the competent authority pathway, the specialist pathway and the short-term training pathway. A European medical graduate who has not completed specialty training, and did not train in the United Kingdom or Ireland, takes the standard pathway. That is the majority case.
Only one of the four involves sitting AMC examinations. On the standard pathway the AMC assesses you directly, and it states plainly that it manages assessments for that pathway only. On the other three it verifies your credentials while the Medical Board of Australia or a specialist college does the assessing. The short-term training pathway runs usually up to 24 months and does not lead to general registration.
Is my medical school listed in the World Directory of Medical Schools, and why does Australia care?
For a graduate to be eligible to apply to the Australian Medical Council in 2026, the medical school must be listed in the World Directory of Medical Schools and must carry an ECFMG sponsor note. This single check decides whether the Australian route is open to you. It takes about three minutes at www.wdoms.org, and it belongs before you choose a university, not after you graduate.
The directory is published jointly by FAIMER, a division of Intealth, and the World Federation for Medical Education. Search the school by name, then read two tabs. Programme details gives the exact title of the degree awarded. Sponsor notes states whether students and graduates are eligible to apply for ECFMG certification, for which graduation years, and under what conditions.
Three things must line up: the school appears, your degree title matches the title in the sponsor note, and your graduation year falls inside the years the note covers. The AMC says its eligibility requirements align with those ECFMG sets for its own examination. Where a school has no sponsor note its graduates cannot apply for ECFMG certification. Our university overview is a good place to start.
What is primary source verification for the Australian Medical Council, and who carries it out?
Primary source verification confirms your medical degree directly with the institution that issued it. For the Australian Medical Council in 2026 it is carried out by ECFMG through its MyIntealth platform, not by the AMC, which receives the report once ECFMG finishes. Three document types are accepted: the final medical diploma, the final medical transcript, and an alternate graduation document.
The order is fixed. Create a MyIntealth account at ECFMG, upload your credentials, and ECFMG confirms your MyIntealth ID within three business days. Nominate the AMC to receive the reports. Only then create your AMC account and initial portfolio, which needs that ID, and the AMC reviews it within five AMC business days. Verification duration is the one number nobody publishes. Start it first.
What does the AMC CAT MCQ examination test, and where can it be sat in 2026?
The AMC CAT MCQ examination is the first of two assessments on the standard pathway in Australia. It is a computer-adaptive multiple choice examination of 150 questions, each with one correct response from five options, in a single session of three and a half hours. It tests the disease process, clinical examination and diagnosis, investigation, therapy and management.
Pearson VUE administers it at selected test centres, and the AMC holds examination events in Australia and in other countries. You do not have to fly to Melbourne for the written paper. Performance is reported on a 0 to 500 scale with the pass mark described as 250, and a higher cut score arrives in 2026. Authorisation to schedule a sitting lasts twelve months, so paying starts a clock — but the pass itself does not expire.
What happens in the AMC clinical examination, and what counts as a pass?
The AMC clinical examination in 2026 consists of 16 assessed stations and four rest stations. Each station runs ten minutes: two minutes reading, eight minutes assessment. A pass requires at least nine of the fourteen scored stations, because two of the sixteen are pilot stations. Stations cover medical, surgical, women's health, paediatric and mental health presentations, each testing one competency — history taking, examination, diagnostic formulation, or management and counselling. You are asked to do one thing well.
Two formats exist and they cost differently. The in-person examination runs at the AMC National Test Centre, which reopened in Melbourne in March 2022 and seats 36 candidates a day; the online version seats 18 and requires a Windows 11 device with a 14-inch screen, webcam and plug-in headphones. That capacity gap is why the online sitting carries a levy. Results come at 4.00pm on the Friday three weeks afterwards, and passing both assessments produces the AMC certificate, generated within five business days, which is what lets you apply to the Medical Board of Australia. A workplace based assessment programme can replace the clinical examination for candidates in an accredited post.
AMC fees have been falling. The MCQ authorisation fee dropped from AUD 3,124 to AUD 2,920 on 8 January 2024. On 1 July 2025 the in-person clinical examination fell from AUD 3,991 to AUD 3,000 and the online sitting from AUD 4,391 to AUD 3,400 — AUD 991 off each, with automatic refunds to candidates who had already paid for sittings between July and September 2025.
What does the Australian Medical Council charge in Australian dollars in 2026?
The Australian Medical Council publishes one fee schedule for the standard pathway. In 2026 the initial portfolio costs AUD 642 including one qualification, the CAT MCQ authorisation AUD 2,920, and the clinical examination AUD 3,000 in person or AUD 3,400 online, with a 0.96 per cent card surcharge on top.
| Australian Medical Council stage, 2026 | Fee in AUD |
|---|---|
| Initial portfolio, including one qualification | 642 |
| Each additional qualification | 107 |
| CAT MCQ examination authorisation | 2,920 |
| Clinical examination, in person | 3,000 |
| Clinical examination, online | 3,400 |
| Online clinical examination levy, included above | 400 |
| Workplace based assessment programme | 1,070 |
| Appeal, first station | 910 |

ECFMG charges separately for verification, in United States dollars, with further costs where more than one document is verified.
How does the competent authority pathway work for doctors trained in the United Kingdom or Ireland?
The competent authority pathway lets an international medical graduate reach general registration in Australia without sitting Australian Medical Council examinations. The Medical Board of Australia recognises six competent authorities across seven categories, two of them European: the General Medical Council in the United Kingdom and the Medical Council of Ireland. Each category pairs an assessment component with an experience component, and you need both.
| Category | Authority | Assessment | Experience |
|---|---|---|---|
| A | GMC, United Kingdom | PLAB test since 1975 | UK Foundation Year 1, or 12 months supervised |
| B | GMC, United Kingdom | UK programme quality assured by the GMC | UK Foundation Year 1, or 12 months supervised |
| C | MCC, Canada | LMCC Parts I and II, or Part I plus NAC exam | 12 months postgraduate training in Canada |
| D | ECFMG, United States | USMLE Steps 1, 2 and 3 since 1992 | Two years of accredited residency |
| E | MCNZ, New Zealand | New Zealand Registration Examination | Rotating internship, four MCNZ-accredited runs |
| F | MCI, Ireland | Programme accredited by the Medical Council of Ireland | Irish internship with certificate of experience |
| G | NBOME, United States | COMLEX-USA Levels 1, 2 and 3 from 2005 | Two years of accredited residency |

The order is specific. Secure an employment offer first, then apply to the AMC for verification. Self-assess against the criteria and apply through Ahpra for provisional registration in a specified position, with a Pre-Employment Structured Clinical Interview required if the post is in general practice. Twelve months of supervised practice follow, minimum 47 weeks full time.
Category B is the one European graduates overlook. It does not require PLAB. Graduating from a United Kingdom medical programme quality assured by the General Medical Council and completing Foundation Year 1 is itself the qualifying combination.
What does the specialist pathway involve for a European doctor who has already completed specialty training?
If you finished specialty training in Europe, the specialist pathway is your route and the Australian Medical Council will not examine you. The relevant Australian specialist medical college carries out the assessment; the AMC accredits those colleges and verifies your credentials. Everything substantive happens at the college.
Specialist recognition
Your qualification is assessed against the Australian specialist standard for your field. The route to specialist registration and to practising your specialty anywhere in Australia.
Area of need
You apply for a position in a region Australia has designated as underserved. The assessment is tied to that post, not to open-ended national practice.
Expedited specialist
For qualifications the Medical Board judges substantially equivalent to an approved Australian specialist qualification. The Board keeps the accepted qualifications list.
The expedited specialist pathway is the one to check first. It works from a list: the Medical Board of Australia names the qualifications it accepts as substantially equivalent to an approved Australian one, and a holder of a listed qualification moves through a shortened process rather than a full comparability assessment. Look for your specialty and your awarding body on that list before committing to any other route.
Specialist and general registration are separate applications with separate fees. In 2026 the Medical Board charges AUD 1,661 to apply for either, and the annual fee for specialist registration held without general registration is AUD 1,102, or AUD 925 in New South Wales.
One note from the visa side, since that is where I spend my working days. A specialist already on the Core Skills Occupation List has an employer-sponsorship route running in parallel with the college assessment. The two need not run one after the other. Sequencing them together is where months get saved.
What English language test does the Medical Board of Australia accept in 2026?
The Medical Board of Australia applies the English language skills registration standard Ahpra administers across thirteen health professions. It took effect on 18 March 2025, and the minimum test scores changed on 23 April 2026. Five tests are accepted: IELTS Academic, the Occupational English Test, PTE Academic, TOEFL iBT, and Cambridge C1 Advanced or C2 Proficiency.
| Test, scores from 23 April 2026 | Overall | Listening | Reading | Writing | Speaking |
|---|---|---|---|---|---|
| IELTS Academic | 7.0 | 7.0 | 7.0 | 6.5 | 7.0 |
| Occupational English Test | — | 350 | 360 | 350 | 360 |
| PTE Academic | 63 | 58 | 59 | 60 | 76 |
| TOEFL iBT | 91 | 22 | 22 | 23 | 24 |
| Cambridge C1 Advanced | 178 | 175 | 179 | 180 | 194 |
| Cambridge C2 Proficiency | 185 | 185 | 185 | 176 | 185 |

Three rules decide whether an existing certificate counts. Results stay valid for two years before the date you lodge. You may combine a maximum of two sittings within twelve months. Different providers cannot be combined, so a strong IELTS reading score will not rescue a weak PTE writing score.
Three routes avoid a test entirely: a combined education pathway, a school education pathway of at least ten years of schooling in English in a recognised country, and an advanced education pathway of at least six years of full-time equivalent education taught in English there. That recognised country list matters to European graduates, because it includes Malta, the Republic of Ireland and the United Kingdom. It shrinks on 18 March 2026, when South Africa comes off. Writing was the component that moved: Ahpra lowered it from IELTS 7 to 6.5 or equivalent when the Ministerial Council approved the standard on 14 November 2024.
{{cta}}What is the difference between provisional and general registration in Australia?
Provisional registration in Australia lets an international medical graduate work only in the specified position for which it was granted, under an approved supervision plan. General registration is unrestricted practice. The Medical Board of Australia requires a period of approved supervised practice under provisional registration before general registration becomes available.
The conditions are administrative rather than academic. You work in the position the registration names, under a supervision plan built to the Board's guidelines that names your supervisors and carries written confirmation from the principal supervisor, who reports on your performance to the Board. On the competent authority pathway the length is fixed: twelve months, minimum 47 weeks of full-time service. That 47-week floor is the number to plan leave around.
Fees follow the same split. In 2026 provisional registration for an international medical graduate costs AUD 773 to apply against AUD 332 for an Australian or New Zealand graduate, with an annual fee of AUD 543, or AUD 477 in New South Wales. General registration costs AUD 1,661 plus AUD 1,102 annually, or AUD 925 in New South Wales.
What does the whole sequence cost in Australian dollars in 2026?
A graduate of a European medical faculty going through the standard pathway in Australia in 2026 pays roughly AUD 9,400 in Australian Medical Council and Medical Board of Australia fees, from first portfolio to first year of general registration. That excludes the ECFMG verification fee, the English test and the visa charge.
Take a worked example. Call the candidate Ana: a 2025 graduate of a six-year English-language programme at a European faculty, one degree to verify, no specialty training, sitting the clinical examination in person in Melbourne.
| Worked example, standard pathway, 2026 | AUD |
|---|---|
| AMC initial portfolio, one qualification | 642 |
| AMC CAT MCQ examination authorisation | 2,920 |
| AMC clinical examination, in person | 3,000 |
| Card surcharge at 0.96 per cent on AMC fees | 63 |
| General registration, application fee | 1,661 |
| General registration, first annual fee | 1,102 |
| Total, AMC and Medical Board fees | 9,388 |
| Online clinical examination instead, add | 404 |
| ECFMG verification, charged in USD | separate |
| English language test | separate |
| Visa charge, Department of Home Affairs | separate |

On the competent authority pathway the total differs, because the examination fees disappear and provisional registration is added: portfolio AUD 642, provisional registration AUD 773 plus AUD 543 annually, then general registration AUD 1,661 plus AUD 1,102 after the supervised year. AUD 4,721. The examinations are what the standard pathway costs, and also what makes it open to graduates of any listed school anywhere.
How long does it take from first application to general registration in Australia?
Plan two years from the first Australian Medical Council application to general registration in Australia, and expect most of it to be scheduling rather than waiting on decisions. The published windows are short. The long segments are the verification queue, the gap between examination events, and the supervised practice year. These are the timings the AMC and the Medical Board publish in 2026:
- MyIntealth ID confirmed by ECFMG: within 3 business days
- AMC initial portfolio reviewed: within 5 AMC business days
- Primary source verification by ECFMG: no timeframe published; track it in MyIntealth
- CAT MCQ authorisation valid to schedule a sitting: 12 months
- CAT MCQ results verified: within 7 to 10 business days of the 5-week closing date
- Clinical examination results: 4.00pm on the Friday three weeks after the examination
- AMC certificate generated: within 5 business days
- Supervised practice, competent authority pathway: 12 months, minimum 47 weeks full time
Two habits shorten that calendar. Start verification on day one, before the English test and any examination booking, because it is the only step with no published turnaround. And book the CAT MCQ as soon as your authorisation is issued, since it runs for twelve months whether you use it or not.
Which visa lets a doctor work in Australia, and where do medical practitioners sit on the skilled occupation lists in 2026?
Australia runs four skilled occupation lists in 2026: the Core Skills Occupation List, built on the ANZSCO 2022 classification, plus the Medium and Long-term Strategic Skills List, the Short-term Skilled Occupation List and the Regional Occupation List. Medical practitioners sit on the Core Skills Occupation List in strength — 34 medical occupations, from General Practitioner to Radiation Oncologist. Those lists feed the skilled visa subclasses 186, 187, 189, 190, 407, 482, 485, 489, 491 and 494.
Subclass 482, Skills in Demand
Temporary. An employer sponsors a skilled worker for a position they cannot fill locally. Streams: Core Skills, Specialist Skills, Labour Agreement. Usually a doctor's first visa.
Subclass 186, Employer Nomination
Permanent residence for a skilled worker nominated by an employer. The common destination after a period on a 482, via the Temporary Residence Transition stream.
Subclass 189, Skilled Independent
Points-tested and permanent, letting invited workers live and work anywhere in Australia. No employer required, no sponsor to be tied to.
| Medical occupations on the Core Skills Occupation List, 2026 | ANZSCO code |
|---|---|
| General Practitioner | 253111 |
| Resident Medical Officer | 253112 |
| Anaesthetist | 253211 |
| Specialist Physician (General Medicine) | 253311 |
| Psychiatrist | 253411 |
| Surgeon (General) | 253511 |
| Emergency Medicine Specialist | 253912 |
| Obstetrician and Gynaecologist | 253913 |
| Medical Practitioners nec | 253999 |
Selection of 9 from the 34 medical practitioner occupations at items 191 to 224 of the Migration (Specification of Occupations — Subclass 482 Visa) Instrument 2024, in force in 2026.
One entry deserves saying out loud. Resident Medical Officer at 253112 is on the list, so a doctor who has not yet finished specialty training is a listed occupation in their own right.
Visa application charges are set by the Department of Home Affairs and move independently of the AMC and Ahpra schedules, so check the current amount through the department's visa pricing estimator before you lodge. Registration and the visa are separate processes with separate timelines, and they can run in parallel — the sequencing that saves the most months. Our services overview sets out where we take that work on.
The author's view
I work on visa and immigration law, and the mistake I see most often has nothing to do with law. People treat registration and the visa as two queues to stand in one after the other, finishing the Australian Medical Council process before they think about sponsorship. That adds months for no reason. On the competent authority pathway the Medical Board of Australia's own published order says otherwise: secure an employment offer first, then apply for verification. The job is the beginning, not the reward at the end.
I have lived in Dubai and in India, and I am based in Belgrade now. In all three places the paperwork people feared was rarely the paperwork that delayed them. What stops people is a degree title that reads slightly differently on the diploma than in the World Directory sponsor note, or an English certificate quietly ageing past its two-year validity while they wait for a clinical examination place. Small things, fixable in month one rather than month fourteen.
My colleague Marcel Kloos, who studied dentistry in Sofia between 2015 and 2021 and now practises as a Swiss-licensed dentist in Bern, makes the same point about his own licensing route: read the destination country's rules before you pick the university, not after. Australia has made that easy. The fee schedule, the examination structure and the occupation list are all published. Do the World Directory search this week.
Summary
- The Australian Medical Council runs four assessment pathways in 2026. Only the standard pathway involves sitting AMC examinations.
- Your medical school must be listed in the World Directory of Medical Schools at wdoms.org with an ECFMG sponsor note covering your degree title and graduation year.
- Primary source verification is performed by ECFMG through MyIntealth. It is the step with no published turnaround, so start it first.
- The AMC CAT MCQ examination is 150 questions in one 3.5-hour session at Pearson VUE centres in Australia and abroad, pass mark described as 250 of 500.
- The AMC clinical examination has 16 assessed stations of 10 minutes each; a pass needs at least 9 of the 14 scored stations.
- AMC fees in 2026: portfolio AUD 642, CAT MCQ AUD 2,920, clinical examination AUD 3,000 in person or AUD 3,400 online, plus a 0.96 per cent card surcharge.
- Medical Board fees from 1 August 2026: general registration AUD 1,661 plus AUD 1,102 annually; provisional registration for an international medical graduate AUD 773 plus AUD 543 annually.
- Medical practitioners hold 34 occupations on the Core Skills Occupation List in 2026, feeding the subclass 482, 186 and 189 visas.
Conclusion
Australia publishes almost every step of this route with a price against it. A European medical graduate can therefore cost and schedule the whole thing before spending a cent — starting with a three-minute search of the World Directory of Medical Schools that settles whether the route is open at all.
The world is short of doctors and Australia is recruiting accordingly, with 34 medical occupations on its Core Skills Occupation List. If you want the registration route and the visa route planned together rather than one after the other, book a free consultation.
Frequently Asked Questions about medical registration in Australia
Does my European medical degree need to be listed anywhere before I apply to the Australian Medical Council?
Yes. The medical school must be listed in the World Directory of Medical Schools and carry an ECFMG sponsor note. Your degree title must match the title in that note, and your graduation year must fall within the years it covers. Search the school at wdoms.org and read the programme details and sponsor notes tabs.
Can I sit the AMC CAT MCQ examination without travelling to Australia?
Yes. The Australian Medical Council holds CAT MCQ examination events at venues in Australia and in other countries, administered by Pearson VUE at selected test centres. The examination is 150 multiple choice questions in one session of three and a half hours, and your authorisation to schedule a sitting lasts twelve months from issue.
How much does the whole process cost in Australian dollars in 2026?
A standard pathway candidate sitting the clinical examination in person pays about AUD 9,388: AUD 642 portfolio, AUD 2,920 for the CAT MCQ, AUD 3,000 for the clinical examination, AUD 63 card surcharge, then AUD 1,661 to apply for general registration plus the AUD 1,102 first annual fee. Verification, the English test and the visa are billed separately.
Do I still need to sit AMC examinations if I trained in the United Kingdom or Ireland?
Generally no. The Medical Board of Australia recognises the General Medical Council and the Medical Council of Ireland as competent authorities, and graduates on that pathway do not sit the AMC examinations. You need an assessment component and an experience component — for Ireland, an accredited programme plus an Irish internship with certificate of experience.
What English score does the Medical Board of Australia require in 2026?
For tests taken on or after 23 April 2026: IELTS Academic 7.0 overall with 7.0 in listening, reading and speaking and 6.5 in writing; OET 350 in listening and writing and 360 in reading and speaking; PTE Academic 63 overall; TOEFL iBT 91 overall. Results stay valid two years before you lodge, and different providers cannot be combined.
How long is the supervised practice period before general registration?
On the competent authority pathway, provisional registration is granted for twelve months of supervised practice with a minimum of 47 weeks of full-time service, and an orientation report plus a work performance report fall due after three months. You work only in the position your registration names, under an approved supervision plan.
I have already completed specialty training in Europe. What do I do?
You take the specialist pathway, and the relevant Australian specialist medical college carries out the assessment, not the Australian Medical Council. Three routes exist: specialist recognition, an area of need position, and the expedited specialist pathway for qualifications the Medical Board judges substantially equivalent to an approved Australian one. Check the Board's accepted qualifications list first.
Which visa do doctors use to work in Australia?
Most start on the subclass 482 Skills in Demand visa, where an employer sponsors a skilled worker for a position they cannot fill locally. Permanent options are the subclass 186 Employer Nomination Scheme, usually via the Temporary Residence Transition stream, and the points-tested subclass 189 Skilled Independent visa. Medical practitioners hold 34 occupations on the 2026 Core Skills Occupation List.
