What does a physician associate qualification actually contain?
Anatomy, physiology, pathology, pharmacology, clinical examination and a long block of supervised patient contact. A United Kingdom physician associate master's carries 180 credits and a minimum of 1,600 clinical placement hours across two years. The German Bachelor of Science sits at 180 ECTS, or 210 ECTS for students entering without a healthcare qualification. The content overlaps heavily.
The Keele University programme specification for 2024/25 shows where the time goes. Year one runs 29 weeks of taught sessions alongside roughly 17 weeks of placement; year two runs 20 weeks taught and 22 weeks of full-time clinical placement. The rotation blocks are itemised down to the day: 360 hours of general medicine, 216 and later 288 hours of general practice, and 120 hours each of general surgery, acute and emergency medicine, obstetrics and gynaecology, mental health and paediatrics, plus 140 hours of additional clinical experience.
The Royal College of Physicians curriculum published in September 2023 sets the frame rather than the hours. It requires placements covering acute and emergency medicine, general medicine, surgery, mental health, and primary and community care, and leaves course length and entry requirements to each higher education institution. That is why two British physician associate transcripts rarely look identical.
Germany runs the same profession on a different chassis. The position paper issued by the Bundesärztekammer together with the Deutsche Gesellschaft für Physician Assistants in April 2025 sets a floor of 180 ECTS where the student already holds a healthcare qualification and at least 210 ECTS where they do not. The Deutsche Gesellschaft für Physician Assistants counts 22 universities offering the degree in Germany, 12 of them private and 10 state-funded, over six or eight semesters. The Landesärztekammer Hessen described the graduate population in June 2026 as several thousand and still growing.
The United States has by far the largest cohort. The National Commission on Certification of Physician Assistants recorded 201,038 board-certified physician assistants at the end of 2025, passing 200,000 for the first time and more than doubling the 95,583 counted in 2013. The qualification there sits at master's level and typically runs two to three years, with supervised clinical hours usually quoted in the region of 2,000, which is an approximate 2026 figure rather than a single regulated number.

Why does a physician associate qualification not count as a medical degree in Europe?
Because European law defines the medical degree by volume and content, and defines it narrowly. Article 24 of Directive 2005/36/EC requires basic medical training of at least five years of study and at least 5,500 hours of theoretical and practical training provided by, or under the supervision of, a university. A two-year associate programme sits below that line whatever its quality.
The directive also states what those hours have to produce: knowledge of the sciences on which medicine rests and of scientific method, understanding of the structure, functions and behaviour of healthy and sick people, and clinical competence covering prevention, diagnosis, treatment and reproduction. Medicine is one of the sectoral professions in the directive, which is why a qualification listed in Annex V is recognised automatically across the European Union without a further equivalence check. Physician associate is not on that list.
That is a fact about paperwork. The clinical instinct built on 1,600 ward hours does not disappear when someone re-enrols; it simply has no legal box of its own, so it gets converted into credits instead.
How do European medical faculties assess prior credits under ECTS?
Module by module, against learning outcomes rather than course titles. The European Commission's ECTS Users' Guide is direct about this: recognition should rest on compatibility of learning outcomes rather than equivalence of course contents. One ECTS credit represents 25 to 30 hours of student work, and a full academic year is 60 credits, which the guide puts at 1,500 to 1,800 hours.
What a faculty needs is a document set that makes those outcomes visible. Medical University of Sofia publishes its procedure for students arriving from another institution: an application to the Rector, an academic transcript naming the institution, the level of qualification and the certified semesters or periods of study, certified translations, copies of the discipline programmes wherever the transcript itself does not show hours and credits, and a document proving the accreditation of the previous institution.
The decision mechanics are public too. The Dean's office determines course placement individually. A student may enrol in a higher course where recognised hours and passed exams cover two thirds of the disciplines required at that level, and disciplines whose hours fall short are made up through compensatory coursework and exams. That is the machinery. It rewards a well-built file, and it punishes a thin one.

What does the language side of an English-taught degree really look like?
Teaching, examinations and written work stay in English for all six years at these faculties. The local language arrives through patients, not through lectures. From roughly the third year, clinical teaching moves onto real wards, and those wards speak Bulgarian, Lithuanian, Slovak, Latvian, Hungarian or Polish depending on the city.
Faculties handle this with compulsory language modules in the early years, pitched at history-taking and ward communication rather than literary fluency. Physician associates usually underrate their own advantage here. They already know the shape of a history and the order of a systems review, so the work is vocabulary rather than concept. That difference shows quickly.
How is a European medical degree structured year by year?
Six years, twelve semesters, 360 ECTS. Semmelweis University publishes exactly that shape for its English-language General Medicine programme, which awards the Doctor of General Medicine. The first two years are preclinical: anatomy, histology, physiology, biochemistry. The middle years bring pathology, pharmacology, internal medicine, surgery and the specialist subjects. The final year is clinical.
A physician associate transcript meets that curriculum unevenly, and the unevenness is the whole story. Anatomy, physiology and pharmacology were studied, though often with fewer contact hours than a medical faculty allocates to the same subject. Clinical rotation experience frequently runs ahead of what a third-year medical student has seen. Those two halves of the record are read separately by the faculty, which is why a single blanket rule about the outcome would describe nothing real.

What does the year before the first semester look like?
It is a sequence, and it runs on the academic calendar rather than on a fixed date. Most European faculties open the winter semester in September or early October, and several also run a February intake. Working backwards from that, a realistic year looks like this. Start with the paperwork.

Where can the qualified doctor register afterwards?
In the European Union, the United Kingdom and the United States, by three different routes. Three systems, three routes. A medical degree from a European Union faculty listed in Annex V of Directive 2005/36/EC is recognised automatically in every member state, which is the legal reason the six-year structure is built the way it is.
The General Medical Council operates two doors. Doctors holding a relevant European qualification apply through a dedicated route for full registration. Everyone else applies as an international medical graduate: an acceptable primary medical qualification, both parts of the PLAB test passed within the previous two years, a completed internship, evidence of English language ability, a certificate of good standing from the previous regulator, a five-year activity history and a fitness to practise declaration, followed by an identity check.
For the United States, the 2026 Information Booklet from the Educational Commission for Foreign Medical Graduates sets out what certification takes: the medical school must appear in the World Directory of Medical Schools carrying an ECFMG Sponsor Note, the applicant must have at least four credit years from that school and the final medical diploma, and must pass Step 1 and Step 2 Clinical Knowledge of the USMLE. Clinical and communication skills are demonstrated through an ECFMG Pathway, which includes the Occupational English Test, or through a previous pass in the former Step 2 Clinical Skills. Intealth implemented its Recognized Accreditation Policy in late 2024, and checking a faculty's World Directory entry before enrolling is the single most useful thing an applicant with American plans can do.
What happens to the credits a faculty does not recognise?
They change the workload, not the destination. A module that is not carried over is scheduled as coursework and an exam at the new faculty, usually alongside the year the applicant has been placed in, and Medical University of Sofia describes precisely this as compensatory coursework where the hours fall short of the national requirement. Nothing is wasted.
The part that decides how much gets carried over is the one applicants prepare last: the module description. A transcript listing “Human Anatomy, 8 credits, pass” gives a faculty committee almost nothing to compare. The same module accompanied by its syllabus, its contact hours split into lectures, seminars and dissection, and its stated learning outcomes gives them everything. Physician associate schools issue these on request and rarely volunteer them, and the request takes an email.
Timing matters as much as content. A record assembled months before an application is read differently from one arriving in fragments, because a committee that can see the whole curriculum at once can map it against its own. I moved to Sofia in 2015 to study dentistry, finished in 2021 and practise in Switzerland today from a base in Bern. What I remember most clearly about the beginning is not the anatomy. It is the folder of documents I carried into the faculty office and how little I understood about which pages in it actually mattered.
That is the thing I would tell every physician associate who writes to us. You are not starting from zero, and you are not starting from where a school leaver starts either. You have studied the sciences, you have taken histories, you have stood in theatre. The problem is almost never the substance of what you know. It is that the substance is invisible to a faculty committee unless somebody puts it on paper in the form they read.
