You spend your working day with your hands on musculoskeletal anatomy. You take a history, you test, you form a working diagnosis, you treat, you reassess. At some point a patient walks in with something that is not a physiotherapy problem at all, and you find yourself wishing you could order the scan, read the bloods and prescribe.

That thought is what brings physiotherapists to medicine. The question that follows is always the same one: does the physiotherapy degree count, or does medicine start again from zero?

It counts. A European physiotherapy programme carries between 180 and 300 ECTS depending on the country, and a serious part of it — anatomy, applied physiology, biomechanics, neurology, clinical examination, rehabilitation and a thousand-plus hours of supervised patient contact — is material a medical faculty examines its own students on. How much is credited, and how far up the programme you enter, is decided by the receiving faculty on your transcript.

Can a physiotherapist become a doctor in Europe?

Yes. No European university runs a course that converts a physiotherapy degree into a medical degree, and none is needed. What exists is credit recognition: a faculty commission reads your transcript, decides which of its own examinations your completed modules replace, and places you at the corresponding point in its six-year programme.

That is the whole mechanism. No bridging diploma, no national conversion scheme. Which means the choice of faculty does most of the work: two commissions reading the same physiotherapy transcript under the same European framework can arrive at very different entry points, because recognition practice is written into each faculty's own regulations.

How is physiotherapy education structured across Europe?

Europe has no single model. The United Kingdom trains physiotherapists in a three-year BSc with 1,000 hours of practice-based learning; Germany still trains most of them through a three-year vocational route of 4,500 hours; the Netherlands runs a four-year 240 ECTS bachelor; Flanders requires a bachelor plus a master, 300 ECTS.

In the United Kingdom the BSc (Hons) Physiotherapy is validated by the Health and Care Professions Council and accredited by the Chartered Society of Physiotherapy, and completion is what makes you eligible to apply for HCPC registration. Cardiff University's programme for 2026/27 entry runs three years and 360 UK credits, with 1,000 hours in a practice setting delivered as seven four-week blocks. Two UK credits convert to one ECTS credit, so that transcript reads as 180 ECTS to a continental faculty.

Germany is mid-change. The vocational route under the Masseur- und Physiotherapeutengesetz produces a state-licensed Physiotherapeut after 4,500 hours but awards no academic credit, because it is not a degree. Alongside it sit primary-qualifying bachelor programmes: the Alice Salomon Hochschule Berlin runs seven semesters and 210 ECTS, with the state examination in semester six and the Bachelor of Science in semester seven. The Modellklausel that let those run as pilots expired on 31 December 2024 and was replaced by permanent provisions. The federal government told the Bundestag in 2025 that the 1994 law no longer meets the requirements of modern training.

The American Doctor of Physical Therapy is worth holding against these models, because it looks like a medical qualification and is not one. CAPTE recorded 292 accredited and 25 candidate DPT programmes in December 2024, a mean length of 123.1 weeks and a mean 35.0 weeks of full-time clinical education. To a European medical faculty a clinical doctorate in physical therapy reads as a large, well-documented body of ECTS-convertible coursework with substantial supervised practice. The title is not what gets assessed. The transcript is.

What does a physiotherapy degree already contain that a medical faculty assesses?

Seven blocks travel well: musculoskeletal and functional anatomy, applied physiology, biomechanics and kinesiology, neurology and neurorehabilitation, clinical examination, rehabilitation medicine, supervised patient contact. Those are examined subjects on both sides, close enough in learning outcomes to be compared line by line.

Anatomy is the strongest single card. A physiotherapist knows origin, insertion, innervation and action across the musculoskeletal system as working knowledge rather than as revision. A medical faculty also teaches the viscera, the dissection room and systematic neuroanatomy, which is why anatomy is usually a partial rather than a full credit.

Clinical examination is the block that surprises commissions. The Health and Care Professions Council's standards of proficiency for physiotherapists, in force since 1 September 2023, require a registrant to "undertake and record a thorough, sensitive and detailed assessment", to "form a working diagnosis on the basis of physiotherapy assessment", and to "apply problem solving and clinical reasoning to assessment findings". A regulator stating in writing that a physiotherapist diagnoses. Medical faculties teach the same reasoning under the heading of propaedeutics.

Then there is the patient contact. A Cardiff graduate has 1,000 supervised hours; a Dutch HAN graduate two twenty-week placement periods worth 30 EC each; a German Physiotherapeut 1,600 hours across seven clinical specialties. Very few applicants to a medical faculty have anything comparable.

Which subjects will a physiotherapist study for the first time?

Biochemistry, pharmacology, pathology and microbiology. Those four are new ground for most physiotherapy graduates, and saying so plainly is a description of the curriculum. They sit in the preclinical and early clinical years, and they are taught on the course.

The German training regulations show why. The Ausbildungs- und Prüfungsverordnung für Physiotherapeuten of 6 December 1994 lists every subject and every hour, and biochemistry, pharmacology and microbiology do not appear at all. What appears instead is 390 hours of general and special pathology — disease teaching organised by specialty rather than by mechanism.

Those 1,600 practical hours split as 240 each in surgery, internal medicine, orthopaedics and neurology, 160 in paediatrics, 80 each in psychiatry and gynaecology, 80 elsewhere. Read that next to the clinical rotations of a medical degree; the resemblance is not accidental. British and Dutch graduates differ in one useful way — their programmes carry ECTS from the start and examine research methods and statistics, which map onto the biostatistics and epidemiology of a medical curriculum.

How does ECTS credit recognition actually work?

Recognition rests on two European instruments. The ECTS Users' Guide, 2015 edition, defines 60 credits as one full-time academic year of 1,500 to 1,800 hours of student workload, and states that recognition should rest on compatibility of learning outcomes rather than identical course content.

That sentence is what makes a physiotherapy file workable. A physiotherapy anatomy module and a medical one use different textbooks and different emphases, but where the learning outcomes and the credit volume are comparable, the receiving faculty has a written European standard telling it to compare outcomes rather than chapter headings.

The second instrument is the Lisbon Recognition Convention of 1997, adopted jointly by the Council of Europe and UNESCO. It places the burden of proof on the institution assessing your file, which has to demonstrate that a substantial difference exists rather than you proving that none does.

German vocational graduates face one extra step, because 4,500 hours carry no ECTS. The hours are converted into a credit-equivalent volume using the 1,500-to-1,800-hour benchmark, documented subject by subject from the Anlage 1 table above. Faculties accept this. The file has to be assembled properly for them to do it.

What does EU directive 2005/36/EC actually govern here?

Directive 2005/36/EC governs recognition of a finished professional qualification across the European Union. It does not govern admission to a medical programme with prior credit. Those are two separate questions, and confusing them is the commonest error made on this route.

Article 24 sets what basic medical training has to contain: at least five years of study and at least 5,500 hours of theoretical and practical training, guaranteeing adequate knowledge of the sciences on which medicine is based, sufficient understanding of the structure and function of healthy and sick persons, adequate knowledge of clinical disciplines, and suitable clinical experience in hospitals under supervision. Any faculty granting you credit still has to deliver a programme meeting that standard. The constraint is curricular, not a numerical rule.

The directive's automatic recognition regime, the one that makes a medical degree portable across the Union, covers seven professions: doctors, nurses responsible for general care, dental practitioners, midwives, pharmacists, veterinary surgeons and architects. Physiotherapist is not among them. Physiotherapy qualifications move between member states under the general system of Title III Chapter I instead, where a host state may impose compensation measures.

Which produces a fact worth sitting with. As a physiotherapist your qualification is recognised across Europe case by case, with possible aptitude tests or adaptation periods. As a doctor holding a degree from an EU or EEA university listed in Annex V, recognition is automatic — regardless of which semester you entered.

How far up a medical programme can a physiotherapist enter?

Far enough that nobody with a physiotherapy degree should plan on starting in semester one. The entry point is decided per faculty, on your individual file, by a commission comparing your modules against its curriculum. Three things move it. Which faculty reads the file, because recognition practice is written into each faculty's own regulations. Whether the clinical blocks — pathology, clinical examination, rehabilitation, the supervised hours — are credited alongside the basic sciences, since those sit in the middle years and lift the entry point further than preclinical credit does. And how the target curriculum is sequenced: a faculty teaching anatomy in year one and pharmacology in year three reads a physiotherapy transcript differently from one doing the reverse.

None of that is visible from a university website. It comes from having placed people into those faculties and seen what each commission credits.

Can you keep your practice or caseload while studying medicine?

Many do, and it is one of the practical advantages of this route over most career changes. Physiotherapy is portable work: the skills transfer across borders, demand exists everywhere, and clinics in university cities employ qualified physiotherapists on part-time and hourly contracts.

The arrangements fall into a few patterns. Some keep a practice running with an employed colleague covering the caseload and return during semester breaks. Some take weekend or evening hours in a clinic in the city where they study, which builds the language faster than any course does. Some hold a small private caseload and see it in concentrated blocks.

The constraint is the timetable, not the profession. Preclinical semesters are lecture-and-lab heavy; clinical years run on hospital rotations that start early and finish when the ward finishes. Entering at a higher semester means fewer years of that.

One point on registration. Working as a physiotherapist in another EU member state means having that qualification recognised there under the general system described above, which can take a national decision before you start. Begin it alongside the university application.

What do the clinical years look like for someone who already has patient-contact hours?

Different from what they look like for a school leaver. A physiotherapist entering the clinical years already knows how to be in a hospital: how to talk to a patient in pain, how to handle a body that cannot move itself, how a ward round works, how to write a note another clinician can use. Those are the things that occupy the first clinical year for everybody else.

The transferable ground is specific. History taking is the same skill. Physical examination overlaps heavily in orthopaedics, rheumatology, neurology and rehabilitation — a physiotherapist tests cranial nerves, reflexes, power, tone and gait as routine practice. Handling patients is a physiotherapist's daily competence and a medical student's weak point.

What is new is the apparatus around the examination: imaging, laboratory values, differential diagnosis across the whole of medicine rather than the musculoskeletal and neurological systems, and prescribing. Surgery is new. Emergency medicine is new. The result is a student strong at the bedside and building the rest, and supervisors notice it in how fast responsibility gets handed over on the ward.

What does the document file cost, and how long does it take?

The document work is the one part of this route with fixed, checkable prices. For a German-trained physiotherapist assembling a transcript file, the itemised calculation runs as follows, at official 2026 rates.

The €1.95 rate applies where the text is not supplied in editable electronic form, the normal case for certificates; supply an editable file and it drops to €1.80, with a minimum of €20 per assignment. Rates agreed privately outside court proceedings vary, so these are a reference rather than a quotation.

Timing matters more than the money. Tuition varies by country and by faculty, and the entry point changes the total because it changes how many years you pay for. Entering higher removes whole years of tuition and living costs — usually the largest financial effect of a good recognition decision. What that means for you depends on the faculty, and we work it out in a free consultation.

What does the twelve months before your first semester look like?

Work backwards from the semester start. Most European medical faculties begin in late September or early October, with a smaller number offering a February intake. Everything else hangs off that date. Apostilles from a German Landgericht are issued within days to a few weeks; a sworn translation of a full physiotherapy transcript takes one to three weeks; the item that takes months is the visa or residence permit.

Twelve months out, the transcript review: module descriptions, hours, credit volumes and examination records for every subject, plus the Anlage 1 hour conversion for German vocational graduates. Nine months out, the file goes to the faculties whose regulations suit it. Six months out, apostilles and sworn translations together so nothing waits on anything else. Language preparation runs underneath all of it. A physiotherapist has an advantage few applicants have: clinical vocabulary in any European language is largely Latin and Greek, and you already own the anatomical half of it. What has to be built is the conversational layer and the examination register. Starting twelve months out with two sessions a week is comfortable.

Where does the physiotherapy route lead after graduation?

To the same medical licence as everybody else. A medical degree from an EU or EEA university listed in Annex V of Directive 2005/36/EC is recognised automatically across the Union, and the diploma records the degree, the university and the date of award — not the semester you entered. Outside the Union, the General Medical Council in the United Kingdom, the Medical Council in Ireland and ECFMG certification for the United States assess the awarding institution and the graduate. The physiotherapy qualification stays registrable in the country that awarded it, and most European systems permit both registrations at once.

What the combination is worth professionally is not hypothetical. Physicians in rehabilitation medicine, sports medicine, orthopaedics, neurology and pain medicine work at the boundary the physiotherapist already lives on, and a consultant who has delivered the rehabilitation prescribes it differently. The World Health Organization's 2026 national health workforce report raised its projected global shortfall to 11 million health workers by 2030, up from the 10 million estimated in 2022.

Recognition also depends on the university appearing in the right registers — the World Directory of Medical Schools for ECFMG purposes, and the national databases in Europe. The author's view

I studied dentistry in Sofia between 2015 and 2021 and I now practise as a licensed dentist in Bern. What I remember from those years is how much of the first two semesters was spent teaching people to touch a patient. Physiotherapists do not need that part. You have been doing it for years, and it shows in the clinic before it shows in the exam results.

The thing I would push back on is the idea that the missing subjects are a problem. Biochemistry and pharmacology are new for you — so what. They are a block of work, they sit in a known place in the curriculum, and every physiotherapist who has gone through this route before you has cleared them. What actually decides the outcome is which faculty reads your transcript and how that transcript is put together.
