A medical degree earned outside the United States opens the door to American residency training through one credential: certification by the Educational Commission for Foreign Medical Graduates. The Accreditation Council for Graduate Medical Education requires it of every international medical graduate entering an accredited residency or fellowship programme. Two examinations, one English test, a verified diploma and one directory listing stand between a graduate and that certificate.

The directory listing is the part people discover last and should check first. The World Directory of Medical Schools lists more than 4,000 medical schools in operation worldwide, and each listing carries a Sponsor Notes tab. Whether the Educational Commission for Foreign Medical Graduates has placed a note there, and which graduation years it covers, decides whether a school's graduates may apply at all.

A separate accreditation policy took effect in late 2024 and has been widely misread. It adds information to a school's listing. It does not remove anyone's eligibility, and Intealth says so in plain words.

The 2026 procedure below is the one the licensing bodies publish: the directory check, the accreditation policy, the two USMLE Steps, the Occupational English Test, the fees in US dollars, and the calendar that carries a graduate to Match Day. Last reviewed: September 2026.

What is ECFMG certification, and why does a United States residency require it?

ECFMG certification is the credential that allows a graduate of a medical school outside the United States to enter American residency training. The Educational Commission for Foreign Medical Graduates calls it “the standard for evaluating the qualifications of international medical graduates (IMGs) entering the U.S. health care system”, and the Accreditation Council for Graduate Medical Education requires it of international graduates entering accredited residency or fellowship programmes.

That single rule explains why certification comes before everything else. A programme director in Ohio or Texas cannot enrol an uncertified international graduate in an accredited post, whatever the applicant's record looks like.

Four things go into it, and they run in parallel rather than one after another. The medical school must appear in the World Directory of Medical Schools with an ECFMG Sponsor Note covering the applicant's graduation year. The applicant files the Application for ECFMG Certification through the MyIntealth portal. USMLE Step 1 and Step 2 Clinical Knowledge must both be passed. The clinical and communication skills requirement is met through an ECFMG Pathway with a satisfactory score on the Occupational English Test Medicine, or by an earlier passing performance on the former Step 2 Clinical Skills component.

Credentials are then verified at source. ECFMG requires at least four credit years, which its 2026 Information Booklet defines as “academic years for which credit has been given toward completion of the medical curriculum”, plus the final medical diploma. Start with the directory.

How do you check a medical school in the World Directory of Medical Schools?

Open www.wdoms.org, choose “Search the World Directory”, type the school's name, tick the box marked “Include Non-UN Member States”, open the listing and click the “Sponsor Notes” tab. If a note from the Educational Commission for Foreign Medical Graduates appears there, the school's students and graduates may apply for ECFMG certification. That is the whole procedure, and ECFMG publishes it in exactly those steps.

The World Directory of Medical Schools is run jointly by the World Federation for Medical Education and FAIMER, a division of Intealth. The current version launched on 4 April 2014, merging the Avicenna Directory opened in 2008 with FAIMER's International Medical Education Directory, public since April 2002. It lists more than 4,000 medical schools in operation.

Two details save people time. The “Include Non-UN Member States” box matters because a school in a territory outside United Nations membership will otherwise not appear in the results. And the Sponsor Notes tab sits apart from the general information, so a listing that looks complete at first glance may not be the page you need. Click through.

Anyone weighing up a university abroad can run this check before applying, before paying a deposit, before booking a flight. It is free and public. What does an ECFMG Sponsor Note actually say?

An ECFMG Sponsor Note states that students and graduates of the school “are eligible to apply for ECFMG Certification provided that all other eligibility requirements are met”, and it specifies the graduation years for which the school meets those requirements, along with other provisions. The graduation years are the part to read closely, because eligibility attaches to a year rather than to the school in the abstract.

The absence of a note carries an equally clear meaning. ECFMG states it plainly: “If there is no information provided by ECFMG on the Sponsor Notes tab of the school's World Directory listing, students and graduates of this school are not eligible to apply for ECFMG Certification.” Binary, published, checkable.

A Sponsor Note is not an endorsement or a quality ranking. ECFMG says it “does not recognize, accredit, or endorse medical schools”. The note records an eligibility determination for one purpose, and a school without one may be an excellent school with a long history.

What is the ECFMG accreditation requirement that took effect in 2024?

From 2024, Intealth reports whether a medical school is accredited by an agency that is itself recognised by the World Federation for Medical Education or by the National Committee on Foreign Medical Education and Accreditation. This is Intealth's Recognized Accreditation Policy, introduced in 2010 and implemented from November 2024. It adds a notation to a school's World Directory listing.

The requirement was first announced for 2023 and moved to 2024 because the COVID-19 pandemic disrupted accreditation work worldwide. The wording at the time was that a school “must be accredited by an agency recognized by the World Federation for Medical Education”. As of May 2020 that body had recognised 21 accrediting agencies, with 13 more in progress.

Here is the part that gets lost. In its update of 22 December 2023, ECFMG wrote that international medical students and graduates “can continue to apply for and pursue ECFMG Certification, even if their medical school currently does not meet the requirements of the policy”, and that the policy “will not impact an individual's eligibility for ECFMG Certification”. Intealth's own accreditation page repeats the position: schools that do not meet the policy face no penalty regarding ECFMG certification eligibility.

The accreditation policy and the eligibility gate are two different things on the same directory page. The notation tells you how a school's national accrediting agency is regarded internationally. The Sponsor Note tells you whether you may apply. Read both; act on the second.

If accreditation does not change eligibility, why should a prospective student care?

Because accreditation decides which ECFMG Pathway is open, and the Pathways are how the clinical and communication skills requirement is met. There are six in the 2027 cycle. Pathway 3 is for applicants whose school holds accreditation from a body recognised by the World Federation for Medical Education. Pathway 4 covers countries with a comparability determination from the National Committee on Foreign Medical Education and Accreditation.

The other four exist precisely so that accreditation is not the only door. Pathway 1 is for applicants already licensed to practise medicine without supervision in another country. Pathway 2 is for graduates of schools whose regulator requires an objective structured clinical examination that the applicant has passed. Pathway 5 covers a joint degree with a school accredited by the Liaison Committee on Medical Education in the United States. Pathway 6 uses ECFMG's Mini-Clinical Evaluation Exercise, a physician evaluation of real patient encounters, for applicants not eligible for the first five.

Every one of the six requires a satisfactory score on the Occupational English Test Medicine, without exception. Applicants who qualify for Pathway 1 must apply to Pathway 1.

The consequence for someone choosing a university in 2026 is modest. A school with recognised accreditation gives its graduates a straightforward Pathway 3 route; a school without it leaves Pathway 6 and, depending on circumstances, the licensure-based routes. Both lead to the same certificate.

What do USMLE Step 1 and Step 2 CK cover, and how long are they?

USMLE Step 1 tests the basic sciences and the ability to apply those concepts to the practice of medicine. It runs approximately 280 multiple-choice questions in seven 60-minute blocks across a single day, about eight hours in total. Step 2 Clinical Knowledge tests the application of medical knowledge and clinical science to patient care, with approximately 318 questions in eight 60-minute blocks, roughly nine hours in one day.

Attempt limits apply to both. An examinee who has attempted a Step four or more times without passing is ineligible to apply for any Step, and no Step may be taken more than three times in a 12-month period. Eligibility to sit either Step depends on being an enrolled student or graduate of a medical school outside the United States listed in the World Directory as meeting ECFMG requirements. The directory check comes first here too.

What changed for international graduates when Step 1 became pass/fail?

Since 26 January 2022, USMLE Step 1 has been reported as pass or fail only. The passing standard remains 196 on the three-digit scale, but that number no longer appears on score reports or transcripts. The USMLE programme called the change “an important first step toward facilitating broader, system-wide changes to improve the transition from undergraduate to graduate medical education”.

For an international medical graduate the effect is concentration. Step 2 Clinical Knowledge now carries the only three-digit USMLE score on the file, with a minimum passing score of 218 for exams taken on or after 1 July 2025. A strong Step 1 result can no longer be shown as a number.

That reweighting changes how a graduate plans the two exams. Step 1 has to be passed; Step 2 CK is where a measurable result is produced. Many international applicants therefore leave more preparation time before Step 2 CK than they once did, and sit it early enough that the score is on file when programmes begin reading in late September. It also shifts weight onto the parts of an application that were always qualitative: clinical experience, letters, research. Those are built over years.

Who registers an international graduate for the USMLE in 2026?

Since 12 January 2026, medical students and graduates of schools outside the United States register for USMLE Step 1, Step 2 CK and Step 3 through the Federation of State Medical Boards, at usmle.fsmb.org. Registration for international applicants previously ran through ECFMG. United States students and graduates moved to the National Board of Medical Examiners on 26 January 2026.

This is a change of counter, not of substance. ECFMG still determines whether a graduate is eligible for certification and shares that determination with the USMLE co-sponsors for registration. The USMLE programme confirmed that Step exam costs remained the same after the transition.

Anyone returning to the process after a break should expect the registration portal to be somewhere new. The certification application itself still lives in MyIntealth. Two portals, two jobs.

What is the OET Medicine requirement?

The Occupational English Test Medicine is the language and communication assessment used in every ECFMG Pathway. For the 2027 Pathways an applicant must attain at least 350 on the Listening, Reading and Speaking sub-tests and at least 300 on Writing, all in one test administration, achieved on or after 1 January 2025.

Two features of that rule catch people out. The scores must come from a single sitting, so a strong Speaking result in one administration cannot be combined with a strong Writing result from another. And the Writing threshold sits lower at 300, which reflects how the test is scaled rather than any suggestion that writing matters less.

OET Medicine is profession-specific. The tasks are built around clinical work: listening to consultations, reading medical texts, writing a referral letter, speaking through role-played patient encounters. Familiar ground, unfamiliar format.

Timing is fixed by the Pathway deadline rather than by the exam. For the 2027 Pathways, applicants are advised to test by the last available OET date in December 2026, with the application and supporting documentation due by 31 January 2027.

What does ECFMG certification cost in 2026?

Take one graduate sitting both Steps outside the United States in 2026 and applying to a Pathway. The published fees add up to $3,690. Every figure below is a fixed charge from the ECFMG fee schedule effective 1 April 2026 or the USMLE application fees for 2026 and 2027, not an estimate.

Costs outside that total are worth naming. The Occupational English Test is booked and paid separately through OET. ECFMG charges $52 per page in 2026 for transcript translation, $50 for an exam history transcript and $70 for a Certification Verification Service report. Candidates choosing India as their testing region also pay an 18 percent goods and services tax collected by the Indian government, in force since 4 December 2024.

How long does it take from the first exam to Match participation?

A graduate who begins preparing in 2026 and moves steadily can plan on about two years from the World Directory check to Match Day. There is no fixed national sitting; both Step 1 and Step 2 Clinical Knowledge are taken on dates the candidate books, so the calendar is largely the candidate's own to set.

A workable sequence runs like this. Week one, the directory check, because everything downstream depends on it. Then the MyIntealth account and the Application for ECFMG Certification. Register for Step 1 with the Federation of State Medical Boards, prepare over roughly six months, sit it. Move to Step 2 Clinical Knowledge with a longer window, since it carries the three-digit score. Book the Occupational English Test so the result lands before the Pathway deadline of 31 January in the application year. Send the diploma and transcript for verification early, because that stage waits on a third party.

The residency year itself runs on published national dates, opening in June, with applications submitted from early September and Match Day the following March. The 2026 Match Day fell on 20 March 2026.

What happens after certification, at the residency Match?

A certified international medical graduate enters the National Resident Matching Program's Main Residency Match, the algorithm that places applicants into first-year residency posts across the United States. The 2026 Main Residency Match was the largest in the organisation's history, with more than 53,000 applicants registered, 48,050 active applicants, 44,344 positions offered and 41,482 filled.

International medical graduates made up a substantial part of that. In 2026 there were 4,210 active applicants who were United States citizen international medical graduates, of whom 2,947 matched, a rate of 70 percent and the highest on record for that group. Among non-United States citizen international medical graduates, 6,738 of 11,944 matched, a rate of 56.4 percent.

Those categories follow citizenship rather than the country the medical school sits in. Two graduates of the same faculty in the same year can appear in different columns of the same table.

Certification is what makes an applicant eligible to appear in those tables at all. Whether a particular profile fits a particular route is decided case by case, and Medschool Experts assesses each file directly in a free individual consultation.

The author's view

I work in visa and immigration law, and the habit that job teaches you is to find the one document that everything else hangs on. In this process it is not an exam result. It is a tab on a web page. I have watched people plan six years of their life around a medical degree without ever opening the World Directory of Medical Schools, and then meet the Sponsor Note question at the worst possible moment, after graduation. It takes four minutes. Do it before you pay a deposit.

The 2024 accreditation policy caused more anxiety than any other development I have dealt with in this area, and most of that anxiety was unnecessary. People read “requirement” and heard “bar”. What Intealth actually published is that students of schools which do not meet the policy can continue to apply for and pursue certification. I have had that conversation, in some form, with applicants writing from Belgrade where I now live, from Dubai and from India where I lived before. The wording had travelled further than the correction.

What I would tell anyone starting in 2026: the sequence is public, the fees are published to the dollar, and the dates are fixed a year ahead. That is a good position to plan from. Conclusion

The route is administrative rather than mysterious. Every gate on it has a published rule, a published fee and a date, and the first gate is a directory page anyone can open today.
