USMLE Step 1 and Step 2 CK While Studying Medicine in Europe

When can a student in a six-year European programme first sit USMLE Step 1?

Not at a fixed semester, but at the point where the medical school can certify that you have completed the basic medical science component of its curriculum. In a six-year European programme that condition is normally satisfied at the end of year three, once anatomy, physiology, biochemistry, pathology, pharmacology and microbiology are behind you. The summer after third year is therefore the first realistic Step 1 window.

Two published rules set the boundary. The USMLE programme opens Step 1 and Step 2 Clinical Knowledge to "a medical student officially enrolled in, or a graduate of, a medical school that is outside the U.S., listed in the World Directory of Medical Schools as meeting ECFMG eligibility requirements". The 2026 Information Booklet of the Educational Commission for Foreign Medical Graduates adds that you hold an accepted Application for ECFMG Certification, and that the school certifies you "will have completed the basic medical science component" of the curriculum by the beginning of your exam eligibility period.

Note what that does not say. It names no semester and no year number. A programme that finishes pathology in the winter of year three opens the window earlier than one that runs it to the following June. The dean's office, not a study forum, is the authority on when your own curriculum crosses that line. A second rule bites on the day itself: the conditions must hold both when you apply and on your examination date.

The American student reaches Step 1 roughly twenty-two months into the degree. The European student reaches the same certification point around month thirty-four. Same exam, later date.

What did the pass or fail change on 26 January 2022 move onto Step 2 CK?

It moved the only three-digit USMLE number on a residency application. Step 1 has been reported as pass or fail for examinations taken on or after 26 January 2022; the passing standard stayed at 196 on the three-digit scale but no longer appears on score reports or transcripts. Step 2 Clinical Knowledge is still reported as a three-digit score, with a minimum passing score of 218 for examinations taken on or after 1 July 2025.

For scheduling inside a six-year degree the consequence is direct. Step 1 is a gate to clear. Step 2 CK is the result programmes read, so it needs the better preparation block and it needs to be on file at the right moment rather than merely passed at some point.

That moment is September. Residency applications are submitted in early September, and a score arriving in November arrives after most programmes have formed their interview lists. The USMLE programme advises allowing at least eight weeks for notification when choosing a test date, although results are typically available two to four weeks after the test.

Eight weeks is the number to plan against. Work backwards from early September and a Step 2 CK date in June or early July of the application year is comfortable; a date in August is not.

Where does Step 2 CK fit in a six-year curriculum?

In the summer between year five and year six, for most students who want to enter the Match in the same season they graduate. By the end of year five a European programme has usually delivered internal medicine, surgery, paediatrics, obstetrics and gynaecology and psychiatry — the clinical content Step 2 CK is built from. Sitting it that summer puts a score on file roughly fourteen months before graduation.

The alternative is to graduate in June of year six and sit Step 2 CK afterwards. That works, and thousands of graduates do it. It also means the score reaches the file in the autumn at the earliest, so the realistic Match entry is the following year. One exam date, one year of a career. Eligibility is not the constraint: a graduate is eligible in the same clause as an enrolled student. Timing is.

A student who sits Step 1 after year three and Step 2 CK after year five arrives at final year with both examinations behind them, which frees year six for clinical electives, letters of recommendation and the application itself. Those parts of a file take months to assemble and cannot be compressed the way a question bank can.

What has to be true about your medical school before any of this counts?

The school must appear in the World Directory of Medical Schools with a Sponsor Note from the Educational Commission for Foreign Medical Graduates covering your graduation year. Without that note, no eligibility exists to schedule around, and the check is free, public and takes a few minutes at www.wdoms.org. The World Directory is run jointly by the World Federation for Medical Education and FAIMER, a division of Intealth.

A second policy sits on the same listing and is regularly misread. Intealth's Recognized Accreditation Policy, implemented from November 2024, records whether a school is accredited by an agency recognised by the World Federation for Medical Education or by the National Committee on Foreign Medical Education and Accreditation. ECFMG stated in December 2023 that students and graduates "can continue to apply for and pursue ECFMG Certification, even if their medical school currently does not meet the requirements of the policy".

What does a year-by-year USMLE plan across a six-year degree look like?

It has four active periods and two quiet ones. Years one and two carry no USMLE work beyond the directory check and steady basic-science study. Year three ends with the first examination. Year five ends with the second. Year six is application year. The table below sets out the months.

One line in that table does more work than the rest. Appointments at Prometric may not be available more than six months in advance, which is why registration for a July examination belongs in January, not in May. Students who register late do not usually lose the exam; they lose the city, and end up travelling.

Where can you actually sit the exam in Europe, and what does the testing region cost?

Step 1 and Step 2 Clinical Knowledge are delivered at Prometric test centres, and Europe has them in most capitals and many second cities. The practical constraint is not whether a centre exists but how many USMLE seats it releases on the dates you want. A candidate in a smaller market plans a train journey; a candidate in a larger one usually does not.

Cost follows the testing region rather than the individual centre. Testing outside the United States and Canada attracts an international test delivery surcharge on top of the application fee: for 2026 that is $210 for Step 1 and $235 for Step 2 Clinical Knowledge, added to a $695 application fee for each Step in 2026 and 2027. The region is chosen at registration and changing it afterwards carries a separate charge.

Rescheduling is where budgets quietly leak. The fee depends on how many days remain before the appointment, measured in local time at the regional registration centre for your Prometric testing region.

Those bands apply to all testing regions equally for Step 1 and Step 2 Clinical Knowledge. Move a date six weeks out and you pay nothing. The same move four days out costs $158 in 2026 pricing.

What does the whole USMLE budget come to in 2026?

For one candidate on the six-year plan, testing outside the United States and moving one appointment with more than a month's notice, the published fees come to $3,725. That total spreads across three of the six years rather than landing at once, which is the part a single headline figure hides.

Call this the six-year-plan budget, at 2026 prices. Every line is a published charge from the USMLE application fee schedule for 2026 and 2027, the ECFMG fee schedule effective 1 April 2026, or the USMLE rescheduling schedule effective 1 July 2026.

Several charges sit outside that figure. The Occupational English Test is booked and paid directly with OET. Travel to a Prometric centre in another city counts, as does the hotel night before a nine-hour examination, and residency application fees arrive in year six on top. When should the Occupational English Test be taken, and why does it not belong in the exam summer?

Autumn of the final year, after Step 2 Clinical Knowledge and before the Pathway deadline. The Occupational English Test Medicine is required in every ECFMG Pathway: a minimum of 350 on Listening, Reading and Speaking and 300 on Writing, all in one test administration and attained on or after 1 January 2025.

Two features of that rule shape the timing. Because all four sub-test scores must come from a single sitting, a retake means the whole test rather than one paper. And because the 2027 Pathways application and supporting documentation are due by 31 January 2027, applicants are advised to test by the last available date in December 2026.

Which leaves autumn as the sensible slot. Stacking OET into the same summer as Step 2 CK puts two very different preparations into the same eight weeks, and the clinical exam is the one that carries the score. The test is profession-specific — consultations to listen to, a referral letter to write, role-played patient encounters — so a student who has spent two years on European wards has done all of it in practice. The format is what needs rehearsing.

What does generic USMLE guidance leave unanswered for a six-year student?

Three things, and they are all consequences of the curriculum shape rather than of the exam.

The first is the gap between Step 1 and Step 2 CK. In a four-year programme the two examinations sit roughly eighteen months apart. On the six-year plan described here they sit two full years apart, and basic-science recall decays across that gap while clinical rotations run. The answer is not to move Step 1 later; it is to keep a light, deliberately spaced review of physiology and pharmacology running through year four, because Step 2 CK questions lean on both.

The second is what "dedicated preparation time" means when your university sets the calendar. American guidance assumes a protected block carved out of the curriculum. A European six-year programme does not offer one, so the block is the summer break, fixed by the faculty's examination period at one end and the next academic year at the other. Ten weeks across July, August and September is a common shape.

The third is what happens to a plan that slips. Attempt limits are strict: an examinee who has attempted a Step four or more times without passing is ineligible to apply for any Step, and no Step may be taken more than three times in a twelve-month period. Building the first attempt into year three leaves room that a late start does not.

How does the plan change if you decide in year five rather than year one?

It compresses into a graduate route, and that route is well travelled. A student who reaches year five having sat nothing takes Step 1 in the summer after year five, graduates in year six, sits Step 2 Clinical Knowledge that summer, and enters the Match the following September. One extra year between graduation and residency, spent on clinical experience and letters rather than lost.

Nothing in the eligibility rules penalises that order, and source verification of a diploma is simpler once the diploma exists. What changes is the buffer: with two examinations in eighteen months there is no room for a retake without pushing the Match year again.
