Failing Medical School in South Africa? What to Do After Academic Exclusion (2026)

Short answer

Failing medical school in South Africa? Academic exclusion or readmission refused: what happens now and how a transfer abroad brings you back via the HPCSA.

At a glance

  • South African faculties of health sciences exclude students who fail a year twice or cannot finish within the permitted time
  • Exclusion can be appealed to a readmission committee, with documents and a short deadline
  • Funding from the national aid scheme is limited to the length of the degree plus extra years
  • Another South African medical school will rarely admit a student who was excluded elsewhere
  • Coming back from abroad: registration with the HPCSA after its board examination, then two years of internship and one year of community service
  • First step: free transfer consultation with Medschool Experts

You were one of a few hundred admitted out of many thousands of applicants. Now the letter speaks of academic exclusion, and the faculty has no place left for you.

Below you find how exclusion and readmission work, and how a transfer to a medical school abroad brings you back to South Africa as a registered doctor.

Last reviewed: October 2026.

Academic exclusion

In short: Each faculty sets its own readmission rules for the six-year MBChB. The common pattern: a student who fails the same year twice, or who can no longer complete the degree within the permitted number of years, is refused readmission. That is academic exclusion.

The rules are in the faculty handbook. Read the section on readmission for your year of study, because some faculties count a failed supplementary exam as a failed year and others look at the number of courses passed.

Readmission appeal

In short: You can appeal to the readmission committee of the faculty. It looks at circumstances beyond your control, such as illness or bereavement, with documents, and at a realistic plan to pass. The deadline is short and usually falls in December or January.

Funding is the second barrier. The national aid scheme pays for the length of the degree plus a limited number of extra years. A student who repeats twice can be readmitted and still be unable to register.

Why students get stuck

In short: Medical students in South Africa who struggle rarely lack ability. The usual reasons are money, housing and transport, the jump from school to a heavy first-year load, depression or anxiety, bereavement and responsibility for family at home.

None of this means you chose the wrong profession. I started dental medicine in Sofia in 2015, at 24, years after most of my classmates, and graduated in 2021. A slower start says nothing about the doctor you will become.

Transfer abroad

In short: A transfer from a South African faculty to an English-taught medical school abroad is possible, and Medschool Experts organises it. The receiving university assesses your academic record and course outlines. Which universities fit you, and which year you can enter, depends on your profile.

A study transfer is a complicated undertaking. Every university checks hours, content and syllabi in its own way, and the documents have to arrive in exactly the right form. Our experts have almost ten years of experience with study transfers, and we regularly achieve placements into higher years, at times Year 4 or Year 5.

Ask for your full academic record and the course outlines while your student account is still open. A place abroad also means a student visa, and we prepare that file with you.

Returning to South Africa

In short: Yes, you can come back, and the route is longer than for a graduate of a South African university. The Health Professions Council of South Africa verifies your degree, you pass its board examination with a theory part and a practical part, and you then complete internship and one year of community service.

  • University: it must be listed in the World Directory of Medical Schools, and your credentials are verified at source through the service the Council prescribes. The Council has turned away graduates of institutions it no longer recognises, so this is checked before you enrol.
  • Board examination: you have three attempts. A fourth is at the Board's discretion after a wait of one year.
  • Citizens not registered abroad: under the pathway the Board adopted in 2020, South African citizens who are not registered as doctors in the country where they studied pass the theory assessment, complete 12 months of clinical exposure at a South African university, which they must arrange themselves, and then take the practical examination.
  • Internship: citizens who cannot show an internship completed abroad perform it in South Africa at accredited training facilities, followed by community service.
  • Documents: keep the curriculum, transcripts and proof of clinical training from the first year on.

We choose a listed university whose clinical training can be documented for the Council, and we tell you before you enrol whether registration in the country of study is realistic for you.

Costs

In short: Tuition at European medical schools starts at about €3,500 a year, and living costs in many Central and Eastern European cities are €5,000 to €8,000 a year. That is often in the range of tuition and residence at a South African university.

How we help

Medschool Experts reviews your academic record in a free transfer consultation, finds medical schools among our more than 25 partner universities that fit your profile, prepares the application and plans your route back to South Africa. More than 500 students have been placed so far. The first step is always a free transfer consultation.

The author's view

When I began dental medicine in Sofia in 2015, I was 24, years older than most of my classmates. I graduated in 2021. South African students who write to us were often excluded for reasons that had little to do with their heads and a lot to do with money and distance from home. The board examination does not ask where you first failed. It asks what you know when you come back.

Summary

In South Africa a second failed year or a degree that can no longer be finished in time leads to academic exclusion. The appeal needs documents and a plan, and funding often ends before the appeal succeeds.

A transfer abroad is possible, and the return runs through the HPCSA: assessment, board examination, internship and community service. We plan that route home with you from the first day.

Marcel Kloos is the founder and owner of Medschool Experts, a placement agency for medicine, dentistry, veterinary medicine and pharmacy degrees abroad. Since 2021 the team has secured more than 500 study places at 25+ partner universities in Europe and worldwide.

Study medicine abroad: Marcel's own path

Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. He is a federally recognised dentist.

What Marcel does at Medschool Experts

Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.

Topics Marcel writes about
