Accreditation of Medical Universities: Everything You Need to Know

What does accreditation mean, who grants it and which quality seals really count? The comprehensive guide for prospective medical students.

Imagine studying medicine abroad for six years — only to find out at the end that your degree is not recognized in Germany. No medical license application, no admission to medical practice. Six years wasted.

That is exactly what happens to people who study at non-accredited institutions. Accreditation is therefore not a bureaucratic side issue — it is the difference between a recognized medical career and a worthless diploma. Who grants it, how it works, and which quality seals actually matter makes all the difference.

What does accreditation actually mean?

Accreditation means: an independent review body confirms that a university or degree programme meets established quality standards. Similar to a technical inspection for cars — but for medical training institutions.

Those who study at an accredited institution can assume that the faculty are qualified and regularly trained, that clinical internships take place at suitable hospitals, that the curriculum reflects the current state of medicine, and that the degree is recognized in many countries. Those who do not verify this are gambling with their professional future.

How does accreditation work?

Step 1: Self-evaluation. The university first writes up how it operates: What learning objectives does it pursue? What does the curriculum look like? How is clinical training organized? This so-called self-study document forms the basis for everything that follows.

Step 2: External review team. Independent experts — professors, experienced physicians, sometimes student representatives — visit the institution for several days. They inspect laboratories, libraries, and clinical facilities, and conduct interviews with faculty and students.

Step 3: Report and decision. The review team produces a report with strengths, weaknesses, and recommendations. The accreditation authority then decides: full accreditation, conditional accreditation with requirements — or rejection.

Step 4: Re-accreditation. Accreditation is not a permanent free pass. After five to seven years, the institution is reviewed again. Universities that fall short lose their recognition.

Who accredits in Germany, Austria, and Switzerland?

In Germany, the Medical Licensing Regulations (Approbationsordnung für Ärzte, ÄAppO) set out the minimum requirements for medical studies as federal law. In addition, since 2017 the Accreditation Council (Akkreditierungsrat) has served as the central body for system accreditation: this evaluates the entire quality management system of the institution — not just individual degree programmes.

In Austria, AQ Austria fulfils this role. This is particularly important for private universities: Paracelsus Medical Private University Salzburg or Sigmund Freud PrivatUniversität cannot award state-recognized degrees without AQ Austria accreditation.

In Switzerland, the Swiss Accreditation Council (SAR) is responsible. The major medical faculties in Zurich, Bern, Basel, Geneva, and Lausanne are state-recognized and have been established for decades.

USA, UK, and Australia: The international standards

The Liaison Committee on Medical Education (LCME) — operated by the American Medical Association and the Association of American Medical Colleges — is regarded worldwide as the strictest and most prestigious accreditation body for human medicine. LCME accreditation is a prerequisite for professional licensure in the USA. Even outside the USA, an LCME-accredited institution is considered particularly high-quality.

In the United Kingdom, the General Medical Council (GMC) provides oversight. It sets out in documents such as Outcomes for Graduates what graduates must be able to do. The Australian Medical Council (AMC) accredits programmes in Australia and New Zealand and is an important reference in the Asia-Pacific region.

The WFME: The global umbrella organization behind it all

Behind all these national systems stands an international umbrella organization: the World Federation for Medical Education (WFME), founded in 1972 and today based at the University of Copenhagen.

The WFME does not accredit universities directly. Instead, it does something more fundamental: it accredits the accreditation agencies themselves. Simply put: the WFME checks whether the inspector is working reliably — before the inspector checks anything.

Since 2023, the American certification body ECFMG has required that international physicians wishing to practice in the USA come from a university accredited by a WFME-recognized agency. This has created pressure worldwide — many countries have since reformed or newly established their accreditation systems.

ASPIRE: When minimum standards are not enough

Alongside classical accreditation, there are awards for exceptional quality. The best known is the ASPIRE programme of the Association for Medical Education in Europe (AMEE). While classical accreditation asks 'Does this school meet the minimum requirements?', ASPIRE asks: 'What does this school do particularly well?' Universities are recognized for excellence in student engagement, social responsibility, clinical training, or research. ASPIRE is not a mandatory programme, but it is a prestigious quality seal.

Other certifications that matter

ISO 9001 is familiar to most from industry — some universities use this certificate for their quality management system. It demonstrates systematic processes, but says nothing about the substantive quality of teaching.

For university hospitals, Joint Commission International (JCI) accreditation is relevant: it accredits hospitals worldwide against patient safety standards. A JCI-accredited university hospital is a strong indicator that the clinical training environment is professionally managed.

Within the European framework, ENQA/EQAR membership of an accreditation agency ensures that its certifications are recognized across the EU. In the Caribbean, CAAM-HP ensures that graduates can meet the requirements for licenses in North America.

The problem with non-accredited schools

Unfortunately, there are institutions worldwide that issue medical diplomas with minimal effort — so-called 'diploma mills'. Graduates then discover that their home countries do not recognize the degree. In Germany, the licensing authority can refuse recognition if the foreign university is not listed in a recognized database. In the USA, professional licensure is impossible without ECFMG certification. Checking the World Directory of Medical Schools (wdoms.org) is therefore not optional — it is mandatory before enrolling.

Where is accreditation heading?

Three trends are currently changing the field. First, the focus is shifting from input to outcome: previously the question was 'How many teaching hours are there?', today it is 'What can graduates actually do?' Competency-based models such as the Canadian CanMEDS framework set the standard here. Second, social responsibility is gaining weight: Does a university train for the needs of the local population? Third, all agencies must adapt their standards to digital teaching formats — online lectures, simulation training, and digital patient cases pose new questions for traditional evaluation criteria.

The author's view

No other mistake costs medical students as much — time, money, and life plans — as choosing an unrecognized institution. And it is an avoidable mistake. The World Directory of Medical Schools can be checked in five minutes. Anyone who skips this step because the application process at a questionable school runs so smoothly, or because the tuition fee offer is particularly tempting, should recognize exactly that as a warning sign.

Conclusion: Accreditation protects your future

Accreditation is not a bureaucratic appendage — it is the fundamental quality certification on which student decisions, professional licensing, and international recognition rest. Anyone who chooses a university without first verifying whether it is accredited is gambling with their career.

The good news: the check is simple. The World Directory of Medical Schools at wdoms.org is free, up to date, and the world's authoritative reference work. Anyone who searches for a university there and finds no recognized accreditation agency should look very carefully — before signing their enrollment.

Frequently asked questions about accreditation of medical universities

What is the difference between accreditation and state recognition?

State recognition allows a university to award degrees that lead to medical licensure. Accreditation is an additional, independent quality certification. Both together is the ideal case.

How do I check whether a foreign medical university is recognized?

The quickest way: go to the World Directory of Medical Schools (wdoms.org), search for the university, and check whether a WFME-recognized agency has confirmed its quality. This step should always be completed before applying.

What does WFME recognition actually mean?

The WFME does not recognize universities directly, but rather the national accreditation agencies. If an agency is recognized, degrees from its accredited universities are considered quality-assured worldwide.

Is LCME accreditation relevant outside the USA?

Yes — especially if a career in the USA or Canada is planned. It is also regarded worldwide as the most prestigious quality indicator in medical education.

What happens if I studied at a non-accredited institution?

In the worst case, the degree cannot be used internationally. In Germany, the licensing authority can refuse recognition; in the USA, professional licensure is impossible without ECFMG certification.

Marcel Kloos

Swiss-licensed dentist, founder and owner of Medschool Experts

Marcel Kloos is the founder and owner of Medschool Experts, a placement agency for medicine, dentistry, veterinary medicine and pharmacy degrees abroad. Since 2021 the team has secured more than 500 study places at over 25 partner universities in Europe and worldwide.

Study medicine abroad: Marcel's own path

Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. In Switzerland he completed the federal recognition procedure and holds a Swiss dental licence.

What Marcel does at Medschool Experts

Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.

Topics Marcel writes about

  • Study medicine abroad without an entrance exam: universities, costs and admission
  • Study dentistry, veterinary medicine or pharmacy in Europe in English
  • Tuition fees and living costs at medical schools in Europe and worldwide
