Failed exams in Medicine in Italy? What sbarramento, fuori corso, decadenza and a failed semestre filtro mean, what to do now and how a transfer saves your degree.

At a glance

  • No national limit on exam attempts in Italy: one failed exam does not end your medical studies for good
  • What really blocks you: esame di sbarramento, propedeuticità, ripetente and fuori corso
  • End point: decadenza dagli studi, classically after eight years without exams, at some universities after a fixed maximum such as 13 years in Palermo
  • Semestre filtro since 2025/2026: only 9 to 17 percent passed Physics at the first appello, depending on the university
  • Outside the semestre filtro: English-taught courses and transfers into later years
  • Serious problems passing exams: switch early, because Medicine gets harder in the higher years, not easier
  • Way out: transfer to another European medical school, often into a higher year
  • Rientro in Italia: more than 80 percent of the Italian students we place switch back to Italy during their studies
  • First step: free consultation with Medschool Experts

If you have failed exams in medical school in Italy and are asking yourself what to do now, start with one fact. You have almost certainly not failed Medicine. You have failed one exam, perhaps four times in a row, and the Italian system has quietly parked you. Nobody sends a letter saying it is over. The calendar keeps moving. Your career does not.

Below you find what each Italian rule really means for you, from the esame di sbarramento to the semestre filtro, why so many capable students get stuck, why an early switch of university saves years when exams keep going wrong, and how a transfer to another European medical school brings your degree back on track.

Last reviewed: October 2026.

Can you fail medical school in Italy for good?

In short: No. In Italy no national rule ends your medical studies after a fixed number of failed attempts, and an exam can normally be repeated at every appello. What stops students is the calendar: esami di sbarramento between years, repeated years as ripetente or fuori corso and, at the very end, decadenza, which closes the student career at that university.

Germany works the other way round. A German student may repeat a part of the state exam twice, and a third fail closes Medicine at every university in the country. Italy counts years instead of attempts. That sounds gentler, and often it is. But one exam can keep you in the same year for twelve months, and we speak to many Italian students for whom those twelve months turned into three years.

How Medicine in Italy gets blocked: five mechanisms

The exact rules depend on your university and your cohort, but these five are what students run into in 2026.

01

Esame di sbarramento

What triggers it

A listed exam is not passed by the deadline, which is often September.

What it means for you

You are enrolled again in the same year as ripetente.

02

Propedeuticità

What triggers it

An earlier exam in the chain is still missing.

What it means for you

Later exams cannot be booked until that one is passed.

03

Fuori corso

What triggers it

You have passed the legal duration of six years.

What it means for you

Teaching stops, only exam sessions remain, and fees often rise.

04

Decadenza

What triggers it

Too many years without exams, or the maximum duration is reached.

What it means for you

Your student career at that university is closed.

05

Semestre filtro

What triggers it

Too few exams passed or no useful place in the national ranking.

What it means for you

There is no place for you in the second semester.

All of this is written in the regolamento didattico of your degree course, and it changes from cohort to cohort. Two students in the same lecture hall can face different deadlines simply because they enrolled a year apart. So the first thing to read is not a forum thread. It is your own regolamento.

Failed an esame di sbarramento in Medicine: what happens now?

In short: An esame di sbarramento is an exam you must pass by a fixed date before you can enrol in the next year of Medicine in Italy. If you miss it, the university enrols you again in the same year as ripetente. Each Italian university decides, cohort by cohort, which exams block which year.

The University of Genoa shows how concrete this gets. To move from the second to the third year of Medicine and Surgery, students need Fisiologia umana 1 and Primo soccorso. To move from the third to the fourth year, they need Fisiologia umana 2, Fisiopatologia generale, Semeiotica e metodologia clinica and Eziologia e patogenesi delle malattie, and since 2024/2025 the deadline has been September.

The University of Brescia phrases it differently for the 2026 cohort. Nobody may sit an exam of the third year or later until every exam of the first two years is passed, with a single exception, and nobody may sit a sixth-year exam until everything up to the fifth year is done. Decree 58/2019 adds one more condition: the tirocinio pratico-valutativo for the state exam can only begin once all core exams of the first four years are complete.

One exam can freeze a whole year.

If you have just missed a sbarramento, three things are worth doing in the first week. Check whether one more appello still falls before the deadline. Ask the segreteria studenti how you will be enrolled next year. And request an updated exam certificate, because that one sheet of paper is what every next step, including a transfer, starts from.

Typical points where a Medicine degree in Italy stops moving, 2026. The exact exams and deadlines differ by university and by cohort.

Fuori corso or ripetente in Medicine: what does it mean for you?

In short: Ripetente means you are enrolled again in the same year of Medicine because a blocking exam or an attendance requirement is missing. Fuori corso means you have gone past the legal duration of six years without graduating. Your career stays open in both cases, but you lose time, often your scholarship, and frequently pay higher fees.

The six-year degree in Medicine and Surgery carries 360 CFU. Once those six years are over, a student who is fuori corso usually has no scheduled lectures left, only exam sessions. Many universities charge fuori corso students higher contributions, and some private universities add an explicit penalty for every year beyond the regular duration.

The regional scholarships, the borse di studio DSU, are tied to the credits you earn each year. A blocked year therefore tends to cost the grant as well. The rent in Bologna, Milan or Rome does not care. It arrives on the first of the month whether or not there is an appello.

Decadenza dagli studi: when does Italy close your medical career?

In short: Decadenza dagli studi is the loss of student status at an Italian university. The classic rule from Royal Decree 1592 of 1933 applies after eight consecutive years without passing an exam. Many universities now set their own limits, for example twice the legal duration plus one year, which comes to 13 years for Medicine.

The University of Palermo uses exactly that formula and allows 13 years from matriculation for six-year degrees such as Medicine and Dentistry. After decadenza the career is closed. You can enrol again, and the Consiglio di corso can recognise exams you passed earlier, but the new start follows whatever access rules apply at that moment.

Very few students ever reach decadenza. Far more lose years long before it, in the grey zone between ripetente and fuori corso, without anyone ever saying the word out loud.

Failed the semestre filtro: what can you do next?

In short: Since 2025/2026 Italy allocates places in Medicine, Dentistry and Veterinary Medicine through the semestre filtro, an open first semester with national exams in Chemistry, Biology and Physics followed by a national ranking. Students without enough passed exams or a useful ranking position get no place in the second semester. You can try again, switch to a related degree or start Medicine at a European university abroad.

The first round was hard on a great many families. Across both appelli, roughly 11,000 students passed Physics, while more than 21,000 passed Biology and more than 24,000 passed Chemistry. On 22 December 2025, Decree 1115 changed the rules for 2025/2026, splitting the ranking into nine bands and letting students with at least one passed exam enter it and recover the missing credits later.

Semestre filtro 2025/26: who reached 18/30Chemistry24,000+Biology21,000+Physicsabout 11,000For comparison: 50,859 students registeredfor the November 2025 exams.
Students who reached at least 18/30 per subject after both appelli of the first semestre filtro, academic year 2025/2026.

For 2026/2027 the exams are set for 10 December 2026 and 11 January 2027, and the national ranking with three bands follows on 22 January 2027. Two details matter if you are planning ahead. The semestre filtro does not apply to Medicine courses taught in English, and it does not apply to transfers into years after the first. That second point keeps the road back into Italy open: a student who starts abroad today can apply for a higher year at an Italian university later.

Why do medical students in Italy get stuck? The real reasons

In short: Most medical students in Italy who stall do not lack talent. The usual reasons are a death or serious illness in the family, depression or anxiety, an undiagnosed learning disorder such as dyslexia, exam anxiety in oral exams, too little time to study because of work, and one particular exam that will not move.

A death or a serious illness in the family

A father in hospital, a grandmother who needs daily care, a funeral right in the middle of the exam session. Many students carry this alone and keep enrolling as if nothing had happened. The sbarramento deadline knows nothing about any of it.

Depression and anxiety

Medicine is long, and a lecture hall with three hundred people can be a lonely place. Depression often starts small: books opened and closed again, an appello skipped almost without thinking. It is treatable, and most Italian universities offer free psychological counselling to their students.

Dyslexia and other learning disorders (DSA)

Some students read slowly without ever having had a spelling problem, so nobody noticed at school. In a long multiple-choice test such as the semestre filtro, every question becomes a race against the clock. Law 170 of 2010 gives students with a diagnosed specific learning disorder the right to compensatory measures at university, including extra time, and the diagnosis can be made in adulthood. Medical schools abroad offer similar support.

Oral exams and exam anxiety

A large share of Italian exams are oral. You sit in front of the professor and a commission, with a room full of colleagues waiting behind you. For some students that format alone wipes out everything they know. They study for months and freeze within ten minutes.

Not enough time to study

Students who work in the evenings, or commute two hours a day, lose exactly the quiet hours that Physiology and Biochemistry demand. That is a question of time, not of intelligence.

One exam, one professor

Sometimes the whole problem is a single subject. Physics, Physiology or Biochemistry failed four times while everything else is passed, and because of propedeuticità that one exam then blocks every exam behind it.

None of these reasons means you chose the wrong profession. I started dental medicine in Sofia in 2015, at 24, years after most of my classmates, and graduated in 2021. A slower start says nothing about the doctor you will become.

Serious problems passing exams? Why switching university early makes sense

In short: If you keep failing exams in medical school in Italy, switching university is often the better move, and the earlier the better. Medicine rarely gets easier in the higher years: clinical subjects bring more material, longer chains of propedeuticità and tighter deadlines. A transfer abroad in the first years saves time, stress and money, and your passed exams carry more weight while they are recent.

Many students hope the hard part is over once Physics or Biochemistry is finally passed. Usually the opposite happens. From the third year on come Pathology, Pharmacology, Internal Medicine and the clinical subjects, with more material per exam and less time between appelli. At Brescia, as shown above, one missing exam from the first two years blocks every exam from the third year on. Whoever is already struggling in year two carries that weight into every year that follows.

That is why at Medschool Experts we always advise switching early. A student who changes after the second year arrives abroad with a clean record of recent exams and years of study still ahead. A student who waits until fuori corso arrives later, with older exams and several years already spent. Both can make it. The first one simply saves years.

A new start at a European medical school often changes more than the address. A different exam format, in which written and multiple-choice tests play a bigger role at many universities, smaller groups and a fixed timetable in which you move up year by year with your class help many students who got lost in the Italian system. And the road back to Italy stays open.

Signs that a switch makes sense now

If two of these four apply to you, book a free consultation now rather than after the next appello.

01

You have failed the same exam three times or more.

02

Two or more exams of your year are still missing before the sbarramento.

03

You have already repeated a year as ripetente.

04

You study for months but freeze in the oral exam.

Blocked in medical school in Italy: what to do in the next four weeks

In short: Before you drop out of medical school in Italy, read the regolamento didattico of your cohort, note every sbarramento and deadline, talk to your tutor or course president, contact the DSA office and the counselling service, and request your exam certificate and course programmes. Those same documents also prepare a transfer to a medical school abroad.

This four-week plan costs nothing and keeps every door open, including the one to stay.

Week 1

Download the regolamento didattico of your cohort and write down every sbarramento with its deadline.

Week 1 to 2

Ask the tutor or the president of your degree course for a personal appointment.

Week 2 to 3

If reading speed, anxiety or low mood play a part, contact the DSA office or the free counselling service.

Week 3 to 4

Request your certificato degli esami sostenuti and the programmi of every course you have passed.

Before the deadline

Book a free consultation to compare staying with a transfer while every option is still open.

The last step matters most. A transfer is far easier to plan while your career is still active and your exams are recent than after the September deadline has passed.

Transfer from an Italian medical school to a university abroad: how it works

In short: A transfer from an Italian medical school to another European university is possible, and Medschool Experts organises it. Italian CFU and European ECTS credits follow the same logic, so the exams you have passed can be assessed for recognition. Which universities fit you depends on your individual profile.

A study transfer is a complicated undertaking. Every university checks hours, content and syllabi in its own way, deadlines differ from country to country, and the documents have to arrive in exactly the right form. Our experts have almost ten years of experience with study transfers worldwide, so they plan and carry them out with confidence. The consultation costs you nothing.

Medschool Experts is the leading specialist for this worldwide and regularly achieves placements into higher years, at times Year 4 or Year 5, at strong universities. Which universities accept your previous exams depends on your specific profile, and that is what we look at together in the free consultation.

Rientro in Italia: can you return to an Italian medical school later?

In short: A medical degree from another EU country is recognised automatically in Italy under Directive 2005/36/EC. The Ministry of Health issues a decree and you register with the Ordine dei Medici. A return to a higher year at an Italian university during your studies is not guaranteed, but with some diligence it can be planned.

Italian universities publish bandi for posti negli anni successivi al primo, places in later years, and the Consiglio di corso then decides on the convalida of your exams. Because the semestre filtro does not apply to these transfers, the road back stays open. More than 80 percent of the Italian students we have placed switch back to Italy during their studies.

Others prefer to finish abroad and come home for their first job. With around four to five months of holidays a year, they spend a good part of every year in Italy anyway.

What does one more blocked year in Italy cost compared with a transfer?

In short: In 2026, one more year fuori corso in a large Italian university city costs roughly €9,000 to €16,500 including tuition and living costs, plus a year of your life. Tuition at European medical schools starts at about €3,500 a year, and living costs in many Central and Eastern European cities are lower than in Milan or Rome.

A worked example with planning values: a student in Milan, blocked between the third and the fourth year, needs two more years to clear three exams. That comes to about 2 × (€2,500 + €11,000), or €27,000. The money is the smaller part of the bill. The larger part is two years without any clinical progress.

International view: how Germany, France and the UK handle failed exams in Medicine

In short: Italy is relatively forgiving with exam attempts but slow. Germany ends Medicine nationwide after a third failed attempt at a part of the state exam. France does not allow students to repeat the first-year PASS. In the United Kingdom each medical school writes its own progression rules, usually with a limited number of resits.

In Germany the medical licensing regulation allows two repeats of a failed state exam part. The third fail means losing the right to sit that exam, and with it the right to study Medicine anywhere in Germany. Germans call this Zwangsexmatrikulation, and many of those affected continue their studies abroad.

France reformed its first year in 2020. The PASS, the main access year, cannot be repeated, so students who do not get through continue in another subject and may try again later through the LAS route.

In the United Kingdom each medical school writes its own progression rules. A resit and, at the school's discretion, a repeat year are common, and after that the course can be terminated. Italy sits between these systems, with more attempts than almost anywhere else and more years lost to waiting.

How Medschool Experts helps you save your medical degree

In short: Medschool Experts reviews your Italian exam record, CFU and course programmes in a free consultation, finds universities that fit your profile, prepares the application and the documents, and plans a later return to Italy if you want one. More than 500 students have been placed so far.

Getting started is simple. You send us your certificato degli esami sostenuti, and we check what can be recognised and where, without promises that depend on someone else's commission. Then we plan the transfer, the paperwork and the timing together. If you want to return to Italy later, we plan that route from the very first day.

You can read how other students restarted on our student stories page and look at destinations in our universities overview. The first step is always a free consultation.

The author's view

When I began dental medicine in Sofia in 2015, I was 24. Most of my classmates were 19. I remember feeling late for everything, from the first anatomy lab to the first exam session. I graduated in 2021, and today nobody asks how old I was in my first semester. I tell this because so many Italian students who write to us believe that one blocked year has already decided their life. It has not.

The students who contact us from Italy rarely lack ability. Usually there is one exam, one hard year or one family story behind the block. The Italian system gives you many attempts, but it also lets time run without anyone stopping to ask why. That is the moment to act, while your career is still active and every exam you passed still counts.

More than 80 percent of the Italian students we have placed return to Italy during their studies, and the others finish abroad and come home as doctors. Both are good endings. The only bad ending is giving up on Medicine because one regolamento said not yet.

Summary

Failing medical school in Italy rarely happens in one dramatic moment. There is no national limit on exam attempts, so students are slowed down by esami di sbarramento, propedeuticità and years spent as ripetente or fuori corso, and at the far end by decadenza, which Royal Decree 1592 of 1933 sets at eight years without exams and which universities such as Palermo, with 13 years for Medicine, define for themselves.

Since 2025/2026 the semestre filtro decides who gets a place in Medicine, but it applies neither to English-taught courses nor to transfers into later years. The reasons students get stuck are human ones, from family crises and depression to learning disorders, exam anxiety and too little time to study. A transfer to another European medical school is possible, the year of entry depends on your profile, a degree from another EU country is recognised in Italy under Directive 2005/36/EC, and a return to Italy during your studies can be planned.

Conclusion

Being blocked in an Italian medical school feels like a dead end. It rarely is one. The rules that stop you belong to one university and one cohort, and outside them the same goal can be reached on another road, often faster than by waiting for the next appello.

If exams keep going wrong, do not wait for the higher years to fix it. They usually bring more material, not less, and an early switch of university saves you years and a great deal of stress. Talk to us before the next deadline. The consultation is free, and it shows you what is possible with the exams you already have. Book your free consultation.

Failed medical school in Italy: 15 questions and answers

Can you fail medical school in Italy for good?

Not because of one exam. Italian medical schools let you repeat an exam at every appello, and there is no national limit on attempts as there is in Germany. What can end your career is time: a missed esame di sbarramento keeps you in the same year as ripetente, and long periods without exams can lead to decadenza.

What happens if I fail an esame di sbarramento in Medicine?

You cannot enrol in the next year of the course, and the university enrols you again in your current year as ripetente. At some universities you also cannot book exams of later years. Which exams block which year, and by which deadline, is written in the regolamento didattico of your cohort.

I have failed the same exam four times. What should I do now?

Stop and look at the whole picture before the fifth attempt. Check how much time that one exam is costing you through sbarramento and propedeuticità, ask your tutor for an appointment, and consider whether exam anxiety or an undiagnosed learning disorder plays a part. Then compare staying with a transfer in a free consultation while your career is still active.

I have serious problems passing exams. Should I switch university, and when?

Often yes, and as early as possible. Medicine rarely gets easier in the higher years, because clinical subjects bring more material, longer chains of propedeuticità and tighter deadlines. A transfer abroad after the first or second year saves time, stress and money. Even if you are already fuori corso, a switch is usually still possible.

What is decadenza dagli studi?

Decadenza is the loss of your student status at an Italian university. Royal Decree 1592 of 1933 applies it after eight consecutive years without passing an exam, and many universities set their own limits, such as twice the legal duration plus one year. After decadenza your career is closed, and a new enrolment follows the access rules of that moment.

How long can you stay fuori corso in Medicine in Italy?

It depends on the university. Some apply the classic eight years without exams, others set a maximum total duration, such as 13 years from matriculation for Medicine at the University of Palermo. While you are fuori corso you usually only take exams, and many universities charge higher contributions.

I failed the semestre filtro. What are my options?

You can try again at the next semestre filtro, enrol in a related degree, or start Medicine at a European university abroad. A degree from another EU country is recognised in Italy, and transfers into later years at Italian universities are outside the semestre filtro. A free consultation with Medschool Experts shows which route suits you.

Does the semestre filtro apply to English-taught courses and transfers?

No. The semestre filtro applies neither to Medicine courses taught in English nor to transfers into years after the first. That is why a later rientro from a European medical school into a higher year at an Italian university stays possible.

Can I transfer from an Italian medical school to a university abroad?

Yes. Medschool Experts organises study transfers from Italian medical schools to other European universities. The receiving university assesses your exams, CFU and course programmes, and which universities accept your previous exams depends on your individual profile.

Will I lose a year if I transfer from Italy to a medical school abroad?

That depends on your exams and on the receiving university, and it is decided case by case. Medschool Experts regularly achieves placements into higher years, at times Year 4 or Year 5, at strong universities. In the free consultation we look at your record and tell you what is realistic.

Which documents do I need for a transfer from an Italian medical school?

You usually start with your certificato degli esami sostenuti and the programmi of the courses you have passed. Depending on the receiving university, further documents and translations may be needed, and Medschool Experts tells you exactly which ones in the free consultation.

Can I return to an Italian university after studying Medicine abroad?

Yes. Italian universities publish calls for places in years after the first, and the Consiglio di corso recognises your exams. A place is not guaranteed, but with some diligence the rientro can be planned. More than 80 percent of the Italian students we place return to Italy during their studies.

Is a medical degree from another EU country valid in Italy?

Yes. Under Directive 2005/36/EC, medical degrees from EU member states listed in its Annex V are recognised automatically. The Italian Ministry of Health issues a recognition decree, and you then register with the Ordine dei Medici in your province.

Do students with dyslexia get support at medical schools in Italy and abroad?

Yes. In Italy, Law 170 of 2010 gives students with a diagnosed specific learning disorder the right to compensatory measures such as extra time, and the diagnosis can be made in adulthood. Many medical schools abroad offer comparable support.

Does the consultation with Medschool Experts cost anything?

No. The first consultation is free and without obligation. We look at your situation, your exam record and your goals and explain which routes are open to you.
