Physician Associate to Doctor: MD Conversion Routes in Europe

Physician Associate to Doctor: MD Conversion Routes in Europe

Physician associates who want a medical degree: how European faculties weigh ECTS credits, English-taught MD routes and 2026 tuition from EUR 9,950 a year.

A physician associate spends two years on anatomy, physiology, pharmacology and clinical examination, then more than 1,600 hours on hospital wards and in general practice. That is a medical education by any ordinary meaning of the phrase. What it is not, under European law, is a primary medical qualification, and that single distinction is what sends physician associates looking for a route into a full medical degree.

The route exists and it is well travelled. Article 24 of Directive 2005/36/EC fixes what a European medical degree must contain: at least five years of study and at least 5,500 hours of theoretical and practical training under university supervision. Every English-taught medical faculty in the European Union builds to that standard, and every one of them has a published mechanism for weighing what an applicant has already studied.

Marcel Kloos studied dentistry in Sofia between 2015 and 2021 and is today a federally recognised dentist in Switzerland and the founder of Medschool Experts, which has since placed over 500 students into medical and dental degrees, and a growing share of those placements arrive with a transcript already full of credits. Physician associates and physician assistants are now among the most frequent.

Credit recognition decides how the next six years look. It is worth understanding first. Last reviewed: September 2026.

What does a physician associate qualification actually contain?

Anatomy, physiology, pathology, pharmacology, clinical examination and a long block of supervised patient contact. A United Kingdom physician associate master's carries 180 credits and a minimum of 1,600 clinical placement hours across two years. The German Bachelor of Science sits at 180 ECTS, or 210 ECTS for students entering without a healthcare qualification. The content overlaps heavily.

The Keele University programme specification for 2024/25 shows where the time goes. Year one runs 29 weeks of taught sessions alongside roughly 17 weeks of placement; year two runs 20 weeks taught and 22 weeks of full-time clinical placement. The rotation blocks are itemised down to the day: 360 hours of general medicine, 216 and later 288 hours of general practice, and 120 hours each of general surgery, acute and emergency medicine, obstetrics and gynaecology, mental health and paediatrics, plus 140 hours of additional clinical experience.

The Royal College of Physicians curriculum published in September 2023 sets the frame rather than the hours. It requires placements covering acute and emergency medicine, general medicine, surgery, mental health, and primary and community care, and leaves course length and entry requirements to each higher education institution. That is why two British physician associate transcripts rarely look identical.

Germany runs the same profession on a different chassis. The standard issued by the Bundesärztekammer together with the Deutsche Gesellschaft für Physician Assistants in April 2025 sets a floor of 180 ECTS where the student already holds a healthcare qualification and at least 210 ECTS where they do not. The Deutsche Gesellschaft für Physician Assistants counts 22 universities offering the degree in Germany, 12 of them private and 10 state-funded, over six or eight semesters. The Landesärztekammer Hessen described the graduate population in June 2026 as several thousand and still growing.

The United States has by far the largest cohort. The National Commission on Certification of Physician Assistants recorded 201,038 board-certified physician assistants at the end of 2025, passing 200,000 for the first time and more than doubling the 95,583 counted in 2013. The qualification there sits at master's level and typically runs two to three years, with supervised clinical hours usually quoted in the region of 2,000, which is an approximate 2026 figure rather than a single regulated number.

Why does a physician associate qualification not count as a medical degree in Europe?

Because European law defines the medical degree by volume and content, and defines it narrowly. Article 24 of Directive 2005/36/EC requires basic medical training of at least five years of study and at least 5,500 hours of theoretical and practical training provided by, or under the supervision of, a university. A two-year associate programme sits below that line whatever its quality.

The directive also states what those hours have to produce: knowledge of the sciences on which medicine rests and of scientific method, understanding of the structure, functions and behaviour of healthy and sick people, and clinical competence covering prevention, diagnosis, treatment and reproduction. Medicine is one of the sectoral professions in the directive, which is why a qualification listed in Annex V is recognised automatically across the European Union without a further equivalence check. Physician associate is not on that list.

That is a fact about paperwork. The clinical instinct built on 1,600 ward hours does not disappear when someone re-enrols; it simply has no legal box of its own, so it gets converted into credits instead.

How do European medical faculties assess prior credits under ECTS?

Module by module, against learning outcomes rather than course titles. The European Commission's ECTS Users' Guide is direct about this: recognition should rest on compatibility of learning outcomes rather than equivalence of course contents. One ECTS credit represents 25 to 30 hours of student work, and a full academic year is 60 credits, which the guide puts at 1,500 to 1,800 hours.

What a faculty needs is a document set that makes those outcomes visible. Medical University of Sofia publishes its procedure for students arriving from another institution: an application to the Rector, an academic transcript naming the institution, the level of qualification and the certified semesters or periods of study, certified translations, copies of the discipline programmes wherever the transcript itself does not show hours and credits, and a document proving the accreditation of the previous institution.

The decision mechanics are public too. The Dean's office determines course placement individually. A student may enrol in a higher course where recognised hours and passed exams cover two thirds of the disciplines required at that level, and disciplines whose hours fall short are made up through compensatory coursework and exams. That is the machinery. It rewards a well-built file, and it punishes a thin one.

How a transcript becomes recognised creditStep 1 Transcript of Records, module descriptions, contact hours and ECTS credits per subjectStep 2 The faculty compares learning outcomes, not course titlesStep 3 Matching modules are carried over, gaps arescheduled as coursework and examsStep 4 The applicant is placed into a study yearand continues the degree from there
Four-step flow: transcript and module descriptions, faculty comparison of learning outcomes, recognised modules carried over and gaps scheduled, placement into a study year.

Medschool Experts is the world's leading specialist in credit-recognition placements for healthcare graduates and regularly achieves year 4 or year 5 entry at top universities worldwide. What any particular physician associate record produces depends on the modules behind it, the hours attached to them and the faculty reading them, which is assessed case by case rather than from a table. Send the transcript and we look at it directly in a free consultation.

Which English-taught medical programmes in Europe accept applications from healthcare graduates?

Most of the established English-language faculties do, and several publish a separate procedure for applicants who arrive with completed university study behind them. Medical University of Sofia in Bulgaria, the Lithuanian University of Health Sciences in Kaunas, Pavol Jozef Šafárik University in Košice in Slovakia, Rīga Stradiņš University in Latvia, Semmelweis University in Budapest and Poznań University of Medical Sciences in Poland all run six-year medical degrees taught in English.

Requirements vary by university and by individual profile. The modules completed, the hours behind them, the country that issued the qualification and the year it was finished all move the answer, so a generic checklist would mislead more than it helps. Medschool Experts reviews the record and speaks to the faculty on the applicant's behalf. Book a free individual consultation and we assess the case directly.

The faculties Medschool Experts places students at are listed on the universities page, with the country, the language of instruction and the degree awarded for each.

What does the language side of an English-taught degree really look like?

Teaching, examinations and written work stay in English for all six years at these faculties. The local language arrives through patients, not through lectures. From roughly the third year, clinical teaching moves onto real wards, and those wards speak Bulgarian, Lithuanian, Slovak, Latvian, Hungarian or Polish depending on the city.

Faculties handle this with compulsory language modules in the early years, pitched at history-taking and ward communication rather than literary fluency. Physician associates usually underrate their own advantage here. They already know the shape of a history and the order of a systems review, so the work is vocabulary rather than concept. That difference shows quickly.

How is a European medical degree structured year by year?

Six years, twelve semesters, 360 ECTS. Semmelweis University publishes exactly that shape for its English-language General Medicine programme, which awards the Doctor of General Medicine. The first two years are preclinical: anatomy, histology, physiology, biochemistry. The middle years bring pathology, pharmacology, internal medicine, surgery and the specialist subjects. The final year is clinical.

Timeline of a six-year European medical degree: years one and two preclinical, years three to five clinical medicine and rotations, year six the final clinical year.

A physician associate transcript meets that curriculum unevenly, and the unevenness is the whole story. Anatomy, physiology and pharmacology were studied, though often with fewer contact hours than a medical faculty allocates to the same subject. Clinical rotation experience frequently runs ahead of what a third-year medical student has seen. Those two halves of the record are read separately by the faculty, which is why a single blanket rule about the outcome would describe nothing real.

What does the switch from physician associate to doctor cost in 2026?

Tuition plus living costs, over six years, in a country where both are lower than in the United Kingdom, Germany or the United States. Numbers first. A worked example at Medical University of Sofia, using the 2026/2027 tuition and September 2026 living-cost figures for the city, holds both flat for illustration.

  • Tuition: EUR 9,950 a year for six years = EUR 59,700
  • Living costs without rent: EUR 757 a month for ten months a year, six years = EUR 45,420
  • Rent, one-bedroom flat outside the centre: EUR 525 a month for twelve months, six years = EUR 37,800
  • Travel home, four return trips a year at EUR 150 = EUR 3,600
  • Books, instruments, insurance and administrative fees at EUR 700 a year = EUR 4,200

That totals EUR 150,720 for the complete degree, or about EUR 25,100 for each academic year. The per-year figure is the one worth remembering, because it is what changes when a faculty recognises prior study and places an applicant at a later year: every year removed from the front of the degree takes roughly EUR 25,100 of tuition, rent and living costs out of the total, and returns a year of medical salary at the other end.

CitySingle person, monthly, without rentOne-bedroom flat outside the centreData month

Budapest, HungaryHUF 275,044, roughly EUR 830HUF 209,583, roughly EUR 630September 2026

Sharing a flat moves the rent line substantially, and most students do. Medschool Experts sets out what its own placement work covers on the services page.

What does the year before the first semester look like?

It is a sequence, and it runs on the academic calendar rather than on a fixed date. Most European faculties open the winter semester in September or early October, and several also run a February intake. Working backwards from that, a realistic year looks like this. Start with the paperwork.

Months 1 to 2

Assemble the full academic record: Transcript of Records, module descriptions with contact hours and learning outcomes, syllabus copies, the degree certificate. Book the first consultation.

Months 2 to 4

Certified translations and legalisation of the certificate and transcript. This is the step that most often stretches, because it depends on third parties.

Months 4 to 6

The faculty reads the record and the credit decision is taken. Language preparation for the clinical years can start in parallel and costs nothing to begin early.

Months 6 to 9

Place confirmed, accommodation arranged, residence registration prepared. The last four to six weeks are arrival, enrolment and orientation on site.

Physician associates already in post usually keep working through the first half of that sequence, which is one reason the switch is less disruptive financially than it looks from the outside.

Where can the qualified doctor register afterwards?

In the European Union, the United Kingdom and the United States, by three different routes. Three systems, three routes. A medical degree from a European Union faculty listed in Annex V of Directive 2005/36/EC is recognised automatically in every member state, which is the legal reason the six-year structure is built the way it is.

The General Medical Council operates two doors. Doctors holding a relevant European qualification apply through a dedicated route for full registration. Everyone else applies as an international medical graduate: an acceptable primary medical qualification, both parts of the PLAB test passed within the previous two years, a completed internship, evidence of English language ability, a certificate of good standing from the previous regulator, a five-year activity history and a fitness to practise declaration, followed by an identity check.

For the United States, the 2026 Information Booklet from the Educational Commission for Foreign Medical Graduates sets out what certification takes: the medical school must appear in the World Directory of Medical Schools carrying an ECFMG Sponsor Note, the applicant must have at least four credit years from that school and the final medical diploma, and must pass Step 1 and Step 2 Clinical Knowledge of the USMLE. Clinical and communication skills are demonstrated through an ECFMG Pathway, which includes the Occupational English Test, or through a previous pass in the former Step 2 Clinical Skills. Intealth implemented its Recognized Accreditation Policy in late 2024, and checking a faculty's World Directory entry before enrolling is the single most useful thing an applicant with American plans can do.

What happens to the credits a faculty does not recognise?

They change the workload, not the destination. A module that is not carried over is scheduled as coursework and an exam at the new faculty, usually alongside the year the applicant has been placed in, and Medical University of Sofia describes precisely this as compensatory coursework where the hours fall short of the national requirement. Nothing is wasted.

The part that decides how much gets carried over is the one applicants prepare last: the module description. A transcript listing "Human Anatomy, 8 credits, pass" gives a faculty committee almost nothing to compare. The same module accompanied by its syllabus, its contact hours split into lectures, seminars and dissection, and its stated learning outcomes gives them everything. Physician associate schools issue these on request and rarely volunteer them, and the request takes an email.

Timing matters as much as content. A record assembled months before an application is read differently from one arriving in fragments, because a committee that can see the whole curriculum at once can map it against its own. Marcel Kloos built exactly this kind of file for his own move to Sofia in 2015, before Medschool Experts existed, and the placements the team has run since then are what turned it into a repeatable process. Accounts from students who made the switch are collected on the student stories page.

The author's view

I moved to Sofia in 2015 to study dentistry, finished in 2021, and I am a federally recognised dentist in Switzerland and I run Medschool Experts from Bern. What I remember most clearly about the beginning is not the anatomy. It is the folder of documents I carried into the faculty office and how little I understood about which pages in it actually mattered.

That is the thing I would tell every physician associate who writes to us. You are not starting from zero, and you are not starting from where a school leaver starts either. You have studied the sciences, you have taken histories, you have stood in theatre. The problem is almost never the substance of what you know. It is that the substance is invisible to a faculty committee unless somebody puts it on paper in the form they read.

In Medschool Experts' own placement work, the healthcare graduates who do best are the ones who ask their old school for the module descriptions early and unglamorously, six months before anyone talks about applications. It is dull work. It is also the highest-leverage hour anyone in this position will spend, and it is the first thing I ask for when someone books a consultation.

Summary

  • A United Kingdom physician associate master's carries 180 credits and at least 1,600 clinical placement hours over two years, itemised in the Keele University programme specification for 2024/25.
  • The German Physician Assistant Bachelor of Science requires at least 180 ECTS with a prior healthcare qualification and at least 210 ECTS without, under the Bundesärztekammer's April 2025 standard.
  • Article 24 of Directive 2005/36/EC requires a medical degree of at least five years and at least 5,500 hours, which is why the associate qualification cannot be converted directly.
  • The European Commission's ECTS Users' Guide bases recognition on compatibility of learning outcomes rather than equivalence of course contents, with one credit worth 25 to 30 hours of work.
  • Medical University of Sofia charges EUR 9,950 a year for medicine in English in 2026/2027; Rīga Stradiņš University charges EUR 13,500 and Pavol Jozef Šafárik University in Košice EUR 13,000.
  • A full six-year degree in Sofia works out at roughly EUR 150,720 including rent and living costs at September 2026 prices, or about EUR 25,100 an academic year.
  • Medschool Experts is the world's leading specialist in credit-recognition placements for healthcare graduates and regularly achieves year 4 or year 5 entry at top universities worldwide.
  • The finished degree leads to automatic recognition across the European Union, a General Medical Council route in the United Kingdom, and ECFMG certification in the United States on the 2026 requirements.

Conclusion

The physician associate route was designed as a destination, and for many people it is a good one. For those who wanted the medical degree all along, the two years of anatomy, physiology and ward time are not a detour. They are the strongest application file a European medical faculty is likely to read that year.

Frequently Asked Questions about converting a physician associate qualification into a medical degree

Does a physician associate qualification count towards a medical degree in Europe?

Not automatically, but its individual modules can. European faculties assess prior study module by module under ECTS, comparing learning outcomes rather than course titles, as the European Commission's ECTS Users' Guide directs. Anatomy, physiology, pharmacology and clinical subjects studied at university level all enter that assessment. What the assessment produces depends on the hours and outcomes behind each module.

What entry requirements apply to me as a healthcare graduate?

Requirements vary by university and by individual profile, so any single answer printed here would be wrong for most readers. The country that issued your qualification, the modules on your transcript, the hours behind them and the year you finished all change the picture. Medschool Experts assesses the case directly and speaks to the faculty on your behalf. Book a free individual consultation.

How long does a European medical degree take?

Six years and 360 ECTS at the faculties that teach in English, matching Semmelweis University's published structure of twelve semesters for General Medicine in 2026. Article 24 of Directive 2005/36/EC sets the floor at five years and 5,500 hours. Where prior study is recognised, an applicant begins at a later point on that six-year line rather than at the start.

Do I have to learn the local language?

Teaching and examinations run in English for the full degree, but clinical years put you in front of patients who speak Bulgarian, Hungarian, Latvian, Lithuanian, Slovak or Polish. Faculties build compulsory language modules into the early years for exactly that reason, aimed at history-taking and ward communication. Physician associates tend to move through them faster than school leavers do.

Which documents does a faculty need to assess my credits?

The core set is a Transcript of Records, module descriptions showing contact hours and learning outcomes, syllabus copies where the transcript is thin, certified translations and proof that the previous institution is accredited. Medical University of Sofia lists this publicly. The module descriptions do most of the work and are the part applicants most often forget to request.

What does the degree cost in 2026?

Tuition for English-taught medicine in 2026/2027 runs from EUR 9,950 a year at Medical University of Sofia to EUR 13,500 at Rīga Stradiņš University, with Pavol Jozef Šafárik University in Košice at EUR 13,000 and the Lithuanian University of Health Sciences at EUR 12,800 for the first three years. Living costs in Sofia averaged EUR 757 a month without rent in September 2026.

Can I work as a physician associate while I prepare?

Most people do. Gathering transcripts, module descriptions, translations and legalisations takes several months and none of it requires being on campus, so the income continues through that phase. Language preparation also fits around shift work. The break comes at enrolment, when the degree becomes full-time and the clinical timetable takes over.

Will I be able to register as a doctor in the United Kingdom or the United States?

Both routes exist and both are documented by the regulators. The General Medical Council registers international medical graduates who hold an acceptable primary medical qualification, have passed both parts of PLAB within two years and completed an internship. For the United States, the 2026 ECFMG requirements are a World Directory listing with a Sponsor Note, four credit years, the final diploma and USMLE Step 1 and Step 2 Clinical Knowledge.
