Studying medicine in Varna: the curriculum year by year, clinical rotations and electives in Germany, city life and safety.

Anyone interested in studying medicine in Varna usually has three questions left after the first overview: What do I learn, and when? Can I complete parts of my clinical training in Germany? And just how safe is the city where I'll be living for six years?

This article covers the curriculum year by year, the question of clinical rotations (PJ) and electives in Germany, and the safety and crime figures you'd otherwise have to piece together yourself. For the full overview of fees, entrance exams and recognition, see our foundational article on Varna, or the short version with all key facts on the Varna location page.

The Curriculum in Varna, Year by Year

The medicine programme at the Prof. Dr. Paraskev Stoyanov Medical University of Varna runs over six years and 360 ECTS, following the classic European structure: pre-clinical years first, then clinical years, ending with a continuous practical year. Dentistry runs in parallel but moves into hands-on work considerably earlier.

Years 1 and 2: the pre-clinical phase

Anatomy, histology, embryology, physiology and biochemistry shape the timetable, alongside medical physics, biology and ethics. Anatomy is the subject that makes or breaks the first year: lectures, seminars and dissection courses tie together, and you're examined both orally and practically. Bulgarian language classes run in parallel – not a side subject to tick off, but preparation for patient contact later on.

Years 3 to 5: the clinical phase

From the third year, the focus shifts: pathology, pharmacology and microbiology form the bridge, followed by internal medicine, surgery, paediatrics, gynaecology and obstetrics, neurology, psychiatry, dermatology, ENT and ophthalmology. Much of the teaching takes place at St. Marina University Hospital, the largest hospital in Bulgaria's Black Sea region. From here on, you're talking to patients on the ward who don't speak English – the moment where how seriously you took your language courses pays off, or comes back to bite you.

Year 6: the state exam year

The sixth year consists of continuous ward rotations in the major specialties and matches the structure and demands of the German Praktisches Jahr. It ends with the state final exams. The degree awarded is a Master of Medicine and falls under EU Directive 2005/36/EC.

Completing Your Clinical Year (PJ) in Germany

For many, this is the decisive question, and the answer is: yes, the sixth year can be completed wholly or partly at German teaching hospitals. The process is formal, but manageable.

In practice, it works like this: you submit the hospital and planned specialty to the university well before the rotation begins, the faculty approves the section, and the hospital confirms the duration and content at the end in the format required by the university. Approval happens before you start, not after – get the order wrong and you risk an entire section not counting. German teaching hospitals are familiar with this process from students from other EU countries and are set up for it.

Two things are worth seeing realistically. First: you apply to the hospitals yourself, and popular ones are booked out a year in advance. Second: the degree remains a Bulgarian one, and your final exams are taken in Varna. Completing your clinical year in Germany is an option, not an automatic step, and it doesn't replace transferring to a German faculty. But if you want to build contacts in the German hospital world while you're still studying, this is the most direct route.

Electives in Germany

The same applies to electives during the semester breaks. They can be completed in Germany, but they need to match the Bulgarian curriculum and be recognised later during the licensing process. Recognition is handled by the state examination office or licensing authority in your German federal state, not the university. Clarify recognition before the elective, not after.

The side effect is underrated: anyone who's done three or four electives in Germany doesn't walk into a job interview after graduating as a complete unknown.

Varna as a City: What Everyday Life Looks Like

With around 340,000 residents, Varna is Bulgaria's third-largest city and its biggest port city. The university is located within the city, and St. Marina University Hospital is in the northern part – both are easy to reach by bus or marshrutka, and many students walk or cycle.

  • Climate: mild Black Sea climate, warm summers, mild winters with rarely prolonged frost
  • Housing: shared flats and small apartments are the norm, alongside university dormitory places
  • Costs: around €500 to €800 a month depending on location and standard
  • Travel: Varna has an international airport with connections to Central Europe, busier in summer than in winter
  • Season: the city has two faces – a tourism season from June to September, and a quieter university routine the rest of the year

The seasonal effect hits you directly through rent prices. Near the beach, prices rise noticeably during summer season; a few streets inland, the picture looks quite different. Anyone searching in spring rather than late September pays less and has more choice.

Safety in Varna: Crime and Safety Index

MetricVarnaSofia
Crime index34.239.2
Safety index65.860.8
Ratinglowlow

More interesting than the overall scores are the individual figures for Varna. They show what people locally actually worry about – and what they don't.

Individual figure (Varna)ValueRating
Safety walking alone during the day84.1very high
Safety walking alone at night61.9high
General level of crime32.7low
Worry about being mugged or robbed28.9low
Worry about attack due to origin, skin colour or religion15.8very low
Violent crime as a problem23.1low
Theft from vehicles41.0moderate
Drugs as a problem45.0moderate

The picture is clear: violent crime plays a minor role in daily life, and the very low level of concern about attacks based on origin or skin colour is, for international students, the most important figure in this table. Where things get trickier is elsewhere: minor crime around vehicles and drugs in nightlife – both issues you'll recognise from any larger tourist city, and both easily managed with the usual precautions.

Bulgaria in International Comparison

At the national level, Bulgaria's crime index sits at around 35.5, putting it in the low range; the figure has been falling for several years. On the Global Peace Index, which rates 163 countries on safety, conflict and militarisation, Bulgaria ranks 26th – in the top sixth worldwide, ahead of a number of larger European countries. Terrorist attacks play statistically no role.

In other words: if you're wondering whether Bulgaria is a safety risk as a study destination, the answer based on the available data is no. The realistic issues are minor crime, bureaucracy and adjusting to a foreign language – not personal danger.

What the Indices Don't Show

Three things appear in no table, yet still shape your first semester. First, location: a central room with a short walk to the university is worth more than a cheap room on the edge of town that you can't easily get back from at night. Second, language: anyone who still can't speak Bulgarian after two years will run into a real problem during the clinical phase, no matter how safe the city is. Third, integration: the English-language cohorts in Varna are international, but if you only ever move within the German-speaking group, you'll never really get to know the city.

These are exactly the points our team at Medschool Experts supports you with locally – with finding accommodation, with enrolment, with administrative procedures, and afterwards as a point of contact if something comes up in your third semester.

Author's Opinion

I studied in Bulgaria myself, in Sofia, from 2015 to 2021. Back then, the safety question came from my parents, and today it comes up in almost every consultation from parents too – almost never from the applicants themselves. My honest assessment after six years in the country and many visits since: the safety question is the most overestimated concern about studying medicine in Bulgaria, and the language question is the most underestimated one.

Varna has an advantage as a location that you can't see on paper: the city is big enough for a real university life and small enough that you start recognising faces after a few months. On top of that, there's a hospital that's a genuine heavyweight in the region. If you need structure, you'll settle in here more easily than in a big metropolis.

And on doing clinical rotations in Germany: I recommend that anyone who wants to work in Germany later on complete at least part of it there. Not for the recognition – that's already covered by the EU directive – but for the contacts. The difference between a cold application and a call from a senior consultant who already knows you is bigger than any transcript.

Frequently Asked Questions About the Curriculum, City and Safety in Varna

Can I do my clinical year (PJ) from Varna in Germany?

Yes, the sixth year of study can be completed wholly or partly at German teaching hospitals. The requirement is that the university approves the hospital and rotation beforehand, and that the certificates match the curriculum. The order matters: first the faculty's approval, then the hospital's. Leaving it until the fifth year gets tight.

How many hours of anatomy will I face in the first year?

As at all Bulgarian faculties, anatomy is the dominant subject of the first two years in Varna, taught through lectures, seminars and dissection courses. The workload is significantly higher than anything you'll know from school. Anyone who builds a solid study system in the first few weeks gets through well. Anyone who waits for exam periods doesn't.

Is Varna safer than Sofia?

Yes: Varna's crime index sits at around 34, compared with about 39 for Sofia. The gap is real but not dramatic – both cities fall into the low-crime category. The difference is more noticeable day to day: Varna is smaller, distances are shorter, and the centre is easy to navigate.

How safe is it to walk alone at night in Varna?

The safety score for walking alone sits at around 84 out of 100 during the day and around 62 at night. That matches what students report: unremarkable during the day, varying by neighbourhood at night. During summer season, the area around the beach promenade is noticeably busier, with everything that comes with nightlife in a tourist city.

How do administrative procedures work in Varna?

Expect enrolment, residence permits and registration formalities to take longer and be more formal than you're used to from Germany. Bring one document too many rather than one too few. For enrolment and your residence permit, our team at Medschool Experts will support you in person.

How do I get from Germany to Varna?

Varna has an international airport with connections to Central Europe, noticeably busier in summer than in winter. Alternatively, you can fly to Sofia and continue by bus, train or domestic flight. Plan trips home during the winter semester earlier, since the flight schedule thins out.

How expensive is housing in Varna really?

For rent, groceries, local transport and insurance, budget around €500 to €800 a month. The biggest lever is location: near the beach, prices rise noticeably during summer season, while a few streets inland the picture looks different. Searching in spring costs less than searching in September.
