The Fachsprachprüfung certifies medical German at level C1. It is held by the state chambers of physicians, takes around 60 minutes and consists of a doctor-patient conversation, written documentation and a doctor-to-doctor conversation.

The three parts mirror everyday clinical work: taking a history from a simulated patient, writing a physician's letter and presenting the case to a colleague.
It applies to everyone whose language of training was not German – regardless of whether the degree comes from the EU. If you come from a German-speaking country and studied abroad, you are usually exempt, because German is your native language.
