What distinguishes a referral programme from affiliate marketing?
The two terms are often confused, but they describe two different business models. With affiliate marketing you get a link, your community buys something, and you receive a percentage of the basket. The trigger is always a payment. With a referral programme you nominate a person, and the bonus is tied to an event that happens with that person: a contract signed, a job started, a probationary period passed.
In practice this means: affiliate programmes are immediately accessible but pay little per transaction. Referral programmes pay considerably more, but are tied to conditions you often cannot fulfil — for example, that you yourself are employed by the provider.
Which referral programmes are open to medfluencers?
The landscape falls into three groups. The order below is sorted by level of compensation, not suitability — that is a distinction I will come back to further down.
Staffing agencies pay the highest bonuses
Pacura doc pays up to 5,000 euros gross for a successful referral, split into 1,000 euros on the start date and 4,000 euros after the six-month probationary period. Only current employees may refer, primarily doctors who refer doctors or psychotherapists. The signed referral confirmation must be received before the referred person starts work; for part-time positions the bonus is reduced proportionally.
DOCSTR pays 2,000 euros gross once the referred doctor has worked at least 160 hours. The hours may be spread across several individual assignments; below this threshold nothing is paid. Here too, referrers must themselves be working through the agency.
MEDWING Friends is tiered by position: 500 euros for nursing staff, 1,000 euros for resident doctors and management positions, 1,500 euros for specialists and senior physicians, 2,000 euros for leading medical positions. The new position must be a permanent contract; payment follows after the probationary period is passed. You must yourself be registered as a professional; mutual referrals are excluded.
doctari pays 500 euros after the referred person's first assignment and a further 1,500 euros if this leads to a permanent contract with a passed probationary period. The assignment or permanent employment must begin within twelve months; the bonus is paid only once per person.
The common pattern is unmistakable: these are employee programmes. They are aimed at professionals who refer colleagues — not at reach.
Learning platforms and shops pay on a percentage basis
Lecturio runs a classic partner programme with 15 percent per sale or alternatively 5 euros per lead, with a 30-day cookie window. Approval as a partner takes a maximum of two days according to the provider; banners, text links, deep links and voucher codes are provided. Search engine advertising on the brand name is prohibited.
Fernarzt pays 10 euros per valid order, meaning per order that has been medically approved and not cancelled. Cookie window likewise 30 days; the average basket is around 30 euros. For members of a healthcare profession the topic is sensitive, as it involves prescription products.
Amazon PartnerNet is the standard tool for textbooks, stethoscopes, lab coats and small items. Commission ranges between 1 and 12 percent depending on the product category and is adjusted regularly. The lowest entry barrier of all — and the lowest earnings per referral.
The major medical learning platforms have no open programme
AMBOSS, Meditricks and via medici from Thieme are the names most frequently asked about in this target group. None of the three runs a publicly accessible partner programme. Collaborations are handled through individually negotiated arrangements with personal discount codes; terms are not published. Anyone who enquires should include figures on reach and audience composition, otherwise the request tends to go nowhere.
How much does a referral actually pay?
Let us calculate this for a realistic account. Suppose a channel reaches 4,000 people with a story, of whom two percent click the link — that is 80 people. On a well-performing shop link perhaps three percent of those buy, so two to three people. With a textbook at 40 euros and a commission of three percent that comes to around 3 euros. For a story that took half an hour to make.
The learning platform partner programme looks better: 15 percent on a three-figure course yields a two-figure sum per sale. With two sales from 80 clicks that is around 30 euros. That is a factor of ten compared with the book link — but still far from the four-figure bonuses of the staffing agencies.
And those four-figure bonuses are not attainable for most medfluencers. Anyone with an audience of students and prospective students cannot refer someone who is starting a permanent senior physician post next week. The calculation that looks best on paper is the one that never happens.
How is a referral attributed?
Three methods are common, and they differ more than they appear to.
- Link with cookie. The standard in affiliate marketing. Expires after 30 days, does not work reliably if someone opens the link on their phone and buys later on a computer, and is lost as soon as another partner link intervenes.
- Personal voucher code. Considerably more robust. The code travels in the user's mind, survives device and channel changes, and works even if someone googles your name weeks later.
- Form submitted before first contact. Common in high-bonus referral programmes: you enter a name and contact details before the person gets in touch themselves. Cleanly verifiable, but unsuitable for reach — you do not know your followers by name.
For a channel with reach, the voucher code is the only method that truly works. Anyone promoting a programme that only knows forms or cookies loses a significant portion of their own referrals without ever noticing.

What is the legal position in other countries?
The disclosure requirement is the same idea everywhere, but the implementation differs in detail — relevant for anyone who also posts in English or works with international brands.

The practical difference: the British authority is strictest on wording and placement, the American on the question of what must be disclosed at all — there even a free product sent to you is explicitly covered. Anyone following the British approach is on the safe side in all three countries.
What additional rules apply to members of a healthcare profession?
Anyone who is a doctor, dentist, pharmacist or healthcare professional and appears publicly has two further sets of rules alongside the general disclosure requirement: the Heilmittelwerbegesetz and the professional code of their chamber. Both target advertising for medicines, treatments and medical devices. Recommendations for learning platforms, books or study consultancy are generally not affected — but products, diagnostics and treatment promises very much are. If in doubt, clarify a planned collaboration with your chamber before you sign, not after.
The second point concerns tax: a prize or gift is also income. Anyone who regularly receives bonuses or commissions should discuss this with a tax adviser early rather than improvising at the end of the year.
How do you recognise a programme that suits your community?
Five questions that should be answered before any commitment:
- What does your community have to do? Buy, register or book a free consultation are three completely different thresholds.
- How much is one conversion worth? Calculate how many clicks you need for an amount that justifies the work.
- How is attribution handled? Without a code, or with a 30-day cookie, you structurally lose a portion of your referrals.
- When is payment made? With staffing agencies there are regularly six months of probationary period between referral and money.
- Can your audience even trigger the event? Most high-value programmes require that you yourself are employed by the provider.
A programme paying 500 euros that your community can trigger is worth more than one paying 5,000 euros that it can never reach.
Why a free consultation is the lowest threshold
Between the sales programmes with a few percent and the staffing bonuses for permanent employees there is a gap. It could be closed by a programme that simultaneously meets three criteria: for the referred person it is free, there is real value behind each conversion, and attribution runs via a code rather than a cookie.

What bothers me about the whole debate is that almost everyone only talks about amounts. A bonus of 5,000 euros sounds better in any headline than a skeleton or a paid clinical placement — but for the vast majority of medfluencers it simply comes to nothing, because they can never meet the condition.
I regard the trigger as the only metric that truly matters. Anything that costs your viewers money filters out ninety percent of them before you see any benefit. A free conversation filters out nobody. That is why we built our programme this way, and it is why I advise every creator who asks me to check this one question first and only then look at the number.
And one more thing, because it is rarely said in this context: anyone who speaks to young people about medicine has influence over decisions that bind six years and a large five-figure sum. That responsibility is greater than any commission. Only recommend what you have used or verified yourself — and write what it costs and what it cannot do.
