How is medical school in the Philippines actually structured in 2026?
You do not enter medicine straight from school. A pre-medical bachelor's degree comes first, then the Doctor of Medicine runs four years, and a post-graduate internship year follows the degree. Only after that internship does a graduate sit the Physician Licensure Examination. Seven years, in the standard formulation.
The University of the Philippines Manila publishes the clearest version. Its INTARMED programme is a seven-year track for direct entrants: two years of pre-medical courses, four years of medicine, one year of clinical internship. Students who already hold a baccalaureate degree enter laterally at learning unit three and complete a five-year sequence of basic sciences, clinical sciences and internship.
At the University of Santo Tomas Faculty of Medicine and Surgery the Doctor of Medicine is four years: three years of coursework moving from biochemistry, anatomy and physiology through pathology, pharmacology, medicine and surgery into paediatrics, obstetrics and dermatology, then a fourth year given over to clinical clerkship. Roughly 112 credit units in total. Cebu Institute of Medicine describes the same shape in its own words: four years of academic study followed by one year of internship.
Teaching is in English throughout. That is not a marketing line, it is how Philippine higher education works, and it is the main reason the country draws applicants from India, Nigeria and increasingly from Europe.
What does a Doctor of Medicine in the Philippines cost in 2026?
Between roughly PHP 50,000 and PHP 350,000 per academic year in published tuition, depending entirely on whether the school is a state university or a private one. The University of the Philippines College of Medicine sits at the bottom of that range, the University of Santo Tomas near the top, Cebu Institute of Medicine in between.

Two caveats matter. The Universal Access to Quality Tertiary Education Act of 2017 covers tuition for qualifying Filipino citizens at state institutions, so the University of the Philippines schedule is not what every student there pays. Private schools publish separate foreign-student schedules; the University of Santo Tomas tables for 2026–2027 carry an out-of-state component for international applicants in the region of USD 15,000, a figure to confirm with the faculty before it enters any budget.
How much does it cost to live in Manila and Cebu as a medical student?
Around PHP 20,000 to PHP 40,000 a month in Manila once rent, food, utilities and transport are counted, and roughly PHP 13,000 to PHP 22,000 a month in Cebu. Over a twelve-month academic year the Manila range works out at PHP 120,000 to PHP 200,000, which is EUR 1,765 to EUR 2,940 at the September 2026 working rate.

Cebu is cheaper on rent and comparable on food. Manila has more hospitals and more competition for clerkship slots. Students at Cebu Institute of Medicine sit inside the Cebu City hospital network, which shapes daily life more than the rent difference does.

What is the Physician Licensure Examination and how demanding is it?
It is the national medical licence exam, administered by the Board of Medicine under the Professional Regulation Commission, usually twice a year across thirteen test centres from Metro Manila to Zamboanga. Candidates sit it after the post-graduate internship. Results come fast. The Commission released the March 2026 results four working days after the last exam day.

The gap between the March and October sittings is the thing to understand. October draws the fresh graduating cohorts and runs high; March draws more repeat candidates and runs lower. School-level numbers are published alongside. In October 2025 Pamantasan ng Lungsod ng Maynila passed 104 of 105 candidates, the University of Santo Tomas reached 98.17 per cent and the University of the Philippines Manila 97.42 per cent. In March 2026 Far Eastern University – Dr. Nicanor Reyes Medical Foundation led that category with 165 of 174, or 94.83 per cent.
One structural point for non-Filipinos. The Medical Act of 1959, Republic Act 2382, ties licensure in the Philippines to citizenship or to reciprocity with the candidate's own country, so a foreign graduate normally seeks the licence in the country where they intend to practise. That is why the accreditation question matters more to international students than the local pass rate does.
Are Philippine medical schools WFME-accredited, and what does that mean for ECFMG certification in 2026?
Yes, through the national agency. The Philippine Accrediting Association of Schools, Colleges and Universities holds WFME Recognition Status, listed by the World Federation for Medical Education as valid through April 2033 and covering both the Philippines and Samoa. Schools accredited by PAASCU sit inside the recognised-accreditation framework that Intealth applies.
The detail people get wrong is what that status does. Intealth implemented its Recognized Accreditation Policy on 20 November 2024, and the policy is a reporting mechanism, not a gate. Intealth states it plainly: international medical graduates can continue to apply for and pursue ECFMG Certification even if their medical school does not currently meet the policy's requirements. Where a school does meet them, a notation appears on the Sponsor Notes tab of its World Directory of Medical Schools entry, and from the 2026 application season a general statement about the policy is carried on ECFMG Status Reports used for United States graduate medical education applications.
The practical position in 2026: a school listed in the World Directory of Medical Schools with a valid ECFMG Sponsor Note supports a graduate's path to ECFMG Certification and the United States Medical Licensing Examination sequence. PAASCU recognition adds the accreditation notation on top. Cebu Institute of Medicine, the first medical school in the Visayas and Mindanao to gain PAASCU accreditation and named a Commission on Higher Education Center of Excellence in Medicine in April 1996, is listed in the directory. What are University of the Philippines, University of Santo Tomas and Cebu Institute of Medicine like?
Three institutions with three different characters. The University of the Philippines College of Medicine is the state flagship and the cheapest by a wide margin. The University of Santo Tomas Faculty of Medicine and Surgery is the oldest, founded on 28 May 1871 as the Facultad de Medicina y Farmacia. Cebu Institute of Medicine is the small problem-based-learning school outside the capital.
Santo Tomas takes roughly 480 students into first-year medicine each year and reports foreign students at about five to seven per cent of that cohort. Its LEAPMed track, introduced in 2018, runs a two-year Bachelor of Science in Basic Human Studies straight into the four-year Doctor of Medicine, six years in total. The faculty holds Level III re-accreditation from the Philippine Accrediting Association of Colleges and Universities and Commission on Higher Education Center of Excellence status.
Cebu Institute of Medicine began in June 1957 as the Cebu Institute of Technology College of Medicine and took its own name in 1966. It converted to a fully problem-based curriculum by school year 2003–2004: small-group case work rather than lecture halls, from year one. The Commission on Higher Education granted it autonomous status in September 2001. Cohorts are small.

A Philippine medical degree is a third-country qualification in European terms. EU directive 2005/36/EC governs automatic recognition of qualifications obtained inside the European Union and European Economic Area, and a Philippine Doctor of Medicine falls outside that automatic mechanism. Each member state runs its own recognition procedure for third-country qualifications, and those procedures exist and are used every year.
In the United Kingdom the route is registration with the General Medical Council, for which graduates of overseas medical schools typically evidence their knowledge and skills through the Professional and Linguistic Assessments Board examination alongside an approved English language test. The General Medical Council publishes its acceptable-primary-medical-qualification criteria and its list; that list, not a school's marketing, is the thing to check.
A large share of European students who choose the Philippines are aiming at the United States rather than at home. English teaching, an American curriculum model, and the ECFMG pathway above. That is a coherent plan. Decide on it before you enrol.
What does the route look like for students from India?
India has the most specific rulebook of the three regions. The National Medical Commission (Foreign Medical Graduate Licentiate) Regulations, published on 18 November 2021, require a foreign medical degree from a course of minimum fifty-four months' duration, followed by an internship of minimum twelve months in the same foreign medical institution.
Set that against the Philippine structure and the arithmetic is visible: the standalone Doctor of Medicine runs forty-eight months, with the internship year following it. The regulations do not apply to candidates who obtained their qualification before 18 November 2021 or who were already enrolled at that date, and in December 2023 the Commission granted a one-time relief to students who had already begun pre-medical Bachelor of Science courses. In August 2025 the Association of Philippine Medical Colleges issued an advisory on internship places for foreign graduates. Philippine schools have been restructuring their combined pre-medical and medical offerings in response.

What does the route look like for students from Africa?
African applicants come to the Philippines for the same three reasons Europeans do: English-medium teaching, an American-style curriculum, and cost. The regulatory route home runs through each country's own council. The Health Professions Council of South Africa and the Medical and Dental Council of Nigeria both operate registration pathways for graduates of foreign medical schools, generally including a qualifying or assessment examination and a period of supervised practice.
A second pattern is worth naming. Many West and East African graduates do not return immediately; they sit the United States Medical Licensing Examination sequence, or register in a Gulf state where Philippine qualifications are well known because so much of the region's health workforce is Filipino.
Visa sequencing is the part that most often goes wrong, and it is not a medical question at all. Can a prior degree in a health subject count toward the medicine course?

What does the timeline look like, month by month?
Working backwards from a June or September intake, here is the sequence that holds. Dates vary by school, so treat the spacing rather than the calendar as the useful part.
12–10 months out
Decide the destination licence first. Everything downstream follows from that choice.
10–8 months out

Assemble and legalise academic documents. Apostille takes weeks, not days, in most countries.

2 months to arrival
Housing. Six to eight weeks is realistic for a viewing trip or a vetted booking near campus.

What most coverage of Philippine medical education leaves out
Three things, consistently.
The first is that the internship year is a separate legal object from the degree. People read "four-year MD" and plan a four-year budget, then meet a seventh year of living costs with no tuition offset. The second is that pass-rate tables are not comparable across sittings. Quoting a 77 per cent October figure next to a 50 per cent March figure as though they measure the same population produces a meaningless average; the cohorts differ, and the Professional Regulation Commission publishes them separately for that reason.
The third is the most consequential. Accreditation status and licensing eligibility are two different systems, and conflating them causes real planning errors. WFME recognition applies to PAASCU as an agency. The ECFMG Sponsor Note applies to the school. Intealth's Recognized Accreditation Policy, live since 20 November 2024, reports on schools but does not restrict who may pursue ECFMG Certification. Assuming it closed a door in 2024 is an outdated picture; assuming accreditation alone guarantees licensure anywhere is an optimistic one.

I studied dentistry in Sofia between 2015 and 2021, and the thing nobody told me in year one was how much of the outcome is decided before you enrol. Not by grades. By whether you picked a programme whose structure matches the licence you actually want at the end. The Philippines is a good example, because the seven-year shape is genuinely different from the European six-year one, and people budget for the wrong thing.
I like the Philippine option and I say so to families who ask. English throughout, an American curriculum, a national accrediting agency with WFME recognition running to 2033, and tuition that at the University of the Philippines is under a thousand euros a year on the published schedule. That combination is rare. What I insist on is that people decide the destination licence first, then choose the school, rather than the other way round.
