About the Faculty
Cologne is one of the faculties where patient contact does not begin with the clinical phase. The model study programme integrates preclinical and clinical content from the start, and StudiPat adds a longitudinal component that few other faculties offer: the same GP practice, the same patient, across eight semesters. The Uniklinik Köln provides the clinical teaching base with 1,522 beds and 59 clinics and institutes, covering virtually the full specialty spectrum — including smaller disciplines that often remain invisible during a standard curriculum.
StudiPat and SkillsLab
StudiPat places students in one of over 250 partner GP practices from the first semester. They accompany the same patient longitudinally, observing the course of illness, treatment decisions, and the GP–patient relationship over time, and document their observations in a portfolio. This complements the SkillsLab, where clinical skills are trained on models, simulators, and with standardised simulation patients before students see real patients.
Admission
Places are allocated via hochschulstart (both winter and summer semesters). In the main quota (60% of places): Abitur grade and TMS score each count for 45 of 100 points; completed vocational training adds up to 10 points. In the additional aptitude quota (10% of places), the TMS result is the sole criterion. No tuition fees are charged.

Model Programme with Competence Fields
Cologne has taught exclusively in its model study programme since winter semester 2003/04; the classical Regelstudiengang is no longer offered. The three-phase structure of preclinical years, clinical years, and Practical Year is retained, but teaching is integrated from the outset.
The core element is the Kompetenzfelder (competence fields): interdisciplinary courses in which common clinical presentations and leading symptoms — such as diabetes, heart failure, or hypertension — are examined from multiple angles. These blocks begin in the first semester; eighteen of them run in the first four semesters alone. Alongside these, students train practical skills in the SkillsLab using models, simulators, and standardised patients; follow the longitudinal general medicine module (StudiPat); complete a first scientific project; and take at least one elective per semester. Students interested in research can additionally follow the English-language Research Track and receive a certificate of completion.
Assessment differs from the standard curriculum. Instead of a single Physikum, staged equivalence examinations are held in Anatomy, Biochemistry, Physiology, and Medical Psychology and Sociology, each after the relevant semester, supplemented by an oral component. The Second Section of the Medical Examination is taken in written form; the Third Section, after the Practical Year, is oral–practical.
The Practical Year is completed at the Uniklinik Köln or at affiliated teaching hospitals in the Rhineland; GP practices are also available. The Staatsexamen leads to the nationally valid Approbation; a doctorate can be pursued alongside or after the degree.
Pros & Cons
Campus & Life
A Large-City Faculty in Lindenthal
Cologne is Germany’s fourth-largest city, and that shows in the experience of studying there. The Uniklinik lies in the Lindenthal district, directly beside the main university campus, so preclinical institutes, lecture halls, cafeterias, and clinics are connected on foot or by a few minutes’ cycling. The student districts of Sülz, Klettenberg, and the Zülpicher Viertel surround the campus.
The housing market is the downside: Cologne is expensive, competition for affordable flat shares is intense, and there are waiting lists for student hall places. Anyone starting in autumn should begin their search well before the semester and consider suburbs served by rail. In return, you get a city that never stands still: the Rhine embankment and Grüngürtel for study breaks, the bars and clubs on Zülpicher Straße, museums, cinemas, and in winter the carnival that shapes the lecture timetable. The tram, bus, and night-bus network runs around the clock. Cologne is an ICE hub; Bonn, Düsseldorf, and the Ruhr are all within easy reach.
Frequently Asked Questions
What sets the Cologne model programme apart from the standard curriculum?
Preclinical and clinical content are integrated earlier. Kompetenzfelder cover common clinical presentations from the first semester in an interdisciplinary way, and practical skills are trained in parallel in the SkillsLab. The faculty no longer offers the classical Regelstudiengang.
Is there a Physikum in Cologne?
Not in the conventional form. Instead of one large examination after the fourth semester, you sit staged equivalence examinations in Anatomy, Biochemistry, Physiology, and Medical Psychology and Sociology, each supplemented by an oral component. The Second and Third Sections of the Medical Examination then follow in the standard way.
How early is patient contact?
Very early. In addition to the Kompetenzfelder, the longitudinal StudiPat module places you in a GP practice from the first semester, where you accompany the same patient over eight semesters. SkillsLab sessions with simulation patients add structured procedural training from the start.
Can I do scientific work during my studies?
Yes. A first scientific project is a compulsory part of the curriculum. Students who want more can take the English-language Research Track, which includes lecture series from research groups and leads to a certificate — a useful stepping stone to a doctorate.
How do I apply for a place?
Applications go through hochschulstart in the three national quotas. Abitur grade and TMS score are the key criteria; completed vocational training can add points. Cut-off values change each year, so always check current figures. Medschool Experts advises on NC-free routes into medical studies and does not handle applications to German state faculties.
