A dentistry degree in Spain, Italy or Portugal can cost about €3,500 in tuition across the whole programme, or €133,000. Same European Union directive behind the diploma, same right to work as a dentist afterwards. The difference is which door you walk through.

Public universities in all three countries charge fees set by a ministry or a regional government, and those fees are low by any international standard. Universidade de Lisboa charged €697 a year in 2025/2026. Universidad Complutense de Madrid works out near €1,015 for a 60-credit year at the 2025/2026 Comunidad de Madrid rate. Private faculties sit elsewhere: Universidad Europea de Madrid published €26,720 a year for 2026/2027.

Below is the full arithmetic for 2026: tuition, rent in Madrid, Milan, Rome and Lisbon, the instrument kits dentistry adds on top, and where each diploma stands under European Union directive 2005/36/EC.

What does a dentistry degree in Spain, Italy or Portugal actually cost in 2026?

Tuition across the whole degree ranges from roughly €3,500 at a Portuguese public university on the home and European Union rate to roughly €133,600 at the most expensive Spanish private faculty. Living costs then add €60,000 to €85,000 over the programme, and instruments and clinical materials add another €2,000 to €3,000. Those three lines are the whole picture.

The spread inside one country is wider than the spread between countries. A Spanish public place and a Spanish private place differ by a factor of twenty-six on tuition alone. Italy behaves differently again: what you pay at a state university depends on your family's assessed income, not on a price list. Portugal sits in the middle, with a legally capped public fee, a separate and much higher international-student fee, and private faculties between them.

How much is tuition at a Spanish public university like Universidad Complutense de Madrid or Universitat de Barcelona?

Spanish public tuition is priced per ECTS credit by the regional government, not by the university. For 2025/2026 the Comunidad de Madrid set the first-enrolment price at €16.92 per credit, and gave the worked example itself: 60 credits comes to €1,015.20. Universitat de Barcelona published €17.69 per credit for 2026/2027 under the Catalan price decree. A Spanish dentistry degree is 300 ECTS.

Multiply it out and the whole Grado en Odontología at Universidad Complutense de Madrid costs a little over €5,000 in tuition if every credit is passed at first attempt. At Universitat de Barcelona it is around €5,300, plus roughly €140 a year in file-management and support fees that the university lists separately.

The repeat-credit rule matters more than the headline number. Madrid's second-enrolment price for 2025/2026 was €25.38 per credit and the third was €28.76 — a repeated clinical subject is an expense, not just a delay. Pass first time and the degree stays cheap.

What do Spanish private dentistry faculties charge?

Private Spanish faculties publish an all-in annual price, and for dentistry it is among the highest of any degree in the country. For 2026/2027, Universidad Europea de Madrid listed €26,720 a year for Odontología, Universidad Alfonso X el Sabio €24,050, and Universidad Europea de Valencia €23,421. Universidad CEU Cardenal Herrera's English-taught Dentistry programme in Valencia came to €23,785 for 2025/2026 once the place reservation, registration and record fees were added to the nine monthly instalments.

The same faculty charges very differently by language. At Universidad CEU Cardenal Herrera the Spanish-taught Odontología route totalled €16,145 for 2025/2026 against €23,785 for the English-taught route — a gap of €7,640 a year, or €38,200 across five years, for the same degree in a different language of instruction.

Private faculties revise prices each year, and Universidad CEU Cardenal Herrera says so in its own fee schedule. Budget for increases, not a flat line.

How does Italy's ISEE system decide what you pay?

Italian state universities do not quote a single fee. They quote a ceiling, then cut it according to your ISEE — the Indicatore della Situazione Economica Equivalente, an official assessment of household income and assets. Below a national threshold you pay nothing but the regional tax and stamp duty. Above it, the discount tapers away.

For 2025/2026 the national no-tax area sat at an ISEE of €22,000. Between €22,000 and €24,000 the contribution is cut by 80 per cent, then 50 per cent to €26,000, 25 per cent to €28,000 and 10 per cent to €30,000. Above that you pay the full amount. Universities may set a more generous threshold than the national floor, and many do.

What is the full amount? Sapienza Università di Roma published a maximum ordinary contribution of €2,924 for 2025/2026 in the group containing Medicine, Pharmacy, Engineering and the sciences, plus a regional student-support tax of €140 and €16 of stamp duty. That is the case for a student who files no ISEE at all. Inside the no-tax area the same student pays the €156 and nothing more.

Families whose income sits abroad are assessed through a parified ISEE or a country-of-origin calculation. Get that paperwork right before enrolment week; the difference is thousands of euros a year.

Can you study dentistry in English in Italy, and how long does it take?

Yes — and the programme is longer than people expect. Università degli Studi di Siena runs Dentistry and Dental Prosthodontics entirely in English as a single-cycle master's degree in class LM-46, worth 360 credits. Not 300. Italy's Odontoiatria e Protesi Dentaria runs six years, not five, and that applies to the English-taught route as well as the Italian one.

That extra year is the single most under-priced fact in any Spain-Italy-Portugal comparison. Six years of rent in Milan or Rome instead of five is roughly €14,000 of additional living cost even before tuition, and it is a year of earnings you do not have.

Italian-taught dentistry runs at Università degli Studi di Milano, Sapienza Università di Roma, Università degli Studi di Padova and many others, all on the same 360-credit structure. What does dentistry cost in Portugal at Lisboa, Porto and Egas Moniz?

Portugal has the cleanest public price in Europe. The annual propina for a licenciatura or integrated master's was €697 for 2025/2026 at both Universidade de Lisboa and Universidade do Porto, plus €2 of school insurance at Porto. That is the national cap, unchanged for several years, and it applies to Portuguese students and to European Union citizens treated as equivalent.

Students admitted under the international-student route pay a different fee, set by each faculty against the real cost of the course. Faculdade de Medicina Dentária da Universidade do Porto published €8,000 a year for the Mestrado Integrado em Medicina Dentária for 2025/2026, payable in ten monthly instalments, with eight places in each of the first two admission phases and no reduction for students from Portuguese-speaking countries. Universidade de Lisboa set €12,500 a year for Medicina Dentária on its international-student schedule.

Instituto Universitário Egas Moniz, the private school of health and science at Monte de Caparica outside Lisbon, runs the Mestrado Integrado em Medicina Dentária over five years and 300 credits, taught in English or in Portuguese. Its published fee is €841.05 a month over twelve months — €10,092.60 a year, or about €50,500 across the degree. For a five-year English-taught dentistry programme inside the European Union, that is the middle of the market, not the top.

How much do you need to live on in Madrid, Milan, Rome or Lisbon?

Rent is the number that moves. The Spanish consumer organisation OCU analysed 6,580 listings on 31 August 2026 and put the average student room in Madrid at €689 a month, against €735 in Barcelona. Immobiliare.it Insights put a single room in Milan at €704 in 2026, down 3.8 per cent year on year, and Rome at €615, up 7 per cent. Lisbon rooms in shared flats ran €300 to €700.

Everything else is smaller and steadier. A Navegante Metropolitano transport pass in Lisbon costs about €40 a month and covers metro, bus, tram and regional trains. Groceries for a student cooking at home ran €180 to €280 a month in Lisbon in 2026.

Smaller university cities cut this hard. A room in Pisa averaged €364 in 2026 and one in Genoa €408. Rome instead of Milan saves about €1,070 a year in rent; a smaller Italian city saves up to €3,500.

What do instruments and clinical materials add on top of tuition?

This is the line that gets left out of almost every cost comparison, and for dentistry it is real money. Escuela Universitaria ADEMA, which runs a Grado en Odontología in Spain, publishes the figure openly in its economic conditions document: approximately €2,500 of individual clinical material and instruments across the whole degree, not included in tuition, with purchasing beginning in second year.

That is the shape of it everywhere. First year is books and a lab coat. From second year the phantom-head lab starts and you buy your own hand instruments; by the clinical years it is burs, matrices, impression material, endodontic files and composite, plus whatever you break or blunt. In our own placements, the students who budget nothing for this get a shock in the third semester.

I bought my first full instrument kit in Sofia in 2016, and I remember handing over more than a month's rent for a tray of stainless steel. A mirror, probe, tweezers and excavator sounds trivial. A prosthodontics and endodontics kit is not. Instruments that live in a student's bag get lost and dropped, so replacement is a running cost.

Budget this separately, from year two

Plan on €2,000–€3,000 across the degree for instruments, consumables, a loupe if your faculty expects one, and replacements. Some faculties bundle a starter kit into the first clinical year; most do not. Ask before you enrol.

Is the degree recognised across Europe under directive 2005/36/EC?

Yes. Dentistry is one of the sectoral professions in European Union directive 2005/36/EC, so the qualification is recognised automatically rather than case by case. The directive sets a minimum of five years of full-time theoretical and practical study for basic dental training, lists each member state's qualifying diploma in Annex V point 5.3.2, and obliges the others to give it the same effect as their own.

Spanish, Italian and Portuguese dentistry degrees are all on that list. A Grado en Odontología from Universidad Complutense de Madrid, a Laurea Magistrale in Odontoiatria e Protesi Dentaria from Sapienza Università di Roma and a Mestrado Integrado em Medicina Dentária from Universidade do Porto open the same doors in Ireland, Germany, the Netherlands or Sweden.

Registration still happens nationally. In Portugal you register with the Ordem dos Médicos Dentistas before practising; in Spain you join the provincial Colegio de Odontólogos y Estomatólogos. Italy changed its rules in 2021, making Odontoiatria a qualifying degree, with the separate state examination folded into the programme as a practical assessment and the Ministry of University and Research issuing the implementing decrees in July 2022. Take a student starting in September 2026 and count everything: tuition, twelve months of living a year, instruments, insurance and books. Five years in Spain and Portugal, six in Italy.

Two things fall out of this. Living costs dominate every public route — in Lisbon, rent and food are ninety per cent of the bill. And the Italian sixth year is worth €14,000 to €17,000 in living costs alone, which is why an Italian public place and a Portuguese private place end up closer than their tuition suggests.

What happens when? A month-by-month view of a 2026 start

Semester dates are fixed and public, and everything else hangs off them. Spanish and Portuguese universities start in mid-September; Italian single-cycle programmes generally begin in October. Working backwards gives a usable calendar.

October–December, the year before

Shortlist countries and faculties. Language of instruction drives both price and preparation, so start language work now.

January–March

Gather and legalise school certificates. Apostille and sworn translation take four to eight weeks, and nothing moves without them.

April–June

Application windows open in all three countries. Italian applicants prepare the ISEE in parallel; it decides the fee, not the place.

July–August

Results and place allocation. Non-European Union students start the visa file; consular processing runs four to twelve weeks.

September

Housing. Room prices in Madrid, Milan and Lisbon peak in the fortnight before term. Signing in July costs less.

October onwards

Enrolment, residence registration, health cover. Italian instalments fall in November and March; Portuguese propina is billed monthly.

One item is easy to miss. Non-European Union students need proof of funds for the first year, and the required figure tracks the national minimum income — arrange it months ahead of the consular appointment, not days.

Can you transfer into a Spanish, Italian or Portuguese dentistry course with credits you already have?

You can, and it is one of the most useful routes into these countries. Dental hygienists, dental technicians, biomedical scientists, physiotherapists, paramedics, pharmacists and lab technicians arrive with coursework that overlaps a dentistry curriculum, and European faculties assess it subject by subject against their own study plan.

Which route should you actually pick?

Start with language, not price. Studying in Spanish, Italian or Portuguese opens the public systems in all three countries, and the whole degree then costs between €70,000 and €106,000 including five or six years of living. If you need English, the field is Universidad CEU Cardenal Herrera in Valencia, Università degli Studi di Siena and Instituto Universitário Egas Moniz outside Lisbon — and the cheapest of the three is the Portuguese one.

Then weigh the sixth year. Italy's 360-credit programme carries the same European standing as a 300-credit one, so the extra year buys clinic time, not extra recognition. For some that is worth it. For others, earning a year sooner is worth more.

The author's view

I studied dentistry in Sofia between 2015 and 2021, and I now practise as a licensed dentist in Bern. The thing that stays with me from those six years is not tuition. It is the tray of instruments I bought in second year, the ones I still recognise the weight of, and how completely absent that cost was from every budget I had drawn up beforehand. Nobody puts it on a price list. It is real, it starts early, and it never quite stops.

The second thing I would tell a nineteen-year-old comparing Madrid, Rome and Lisbon: count the sixth year properly. Italian dentistry is six years. That is not a footnote. It is roughly €15,000 more of rent and food, and a year of earning you postpone. Sometimes the extra clinical time is worth exactly that. But decide it on purpose, not by accident.

In our own placements, the students who do well are rarely the ones who found the cheapest number. They are the ones who matched the language to their own head and the city to their own budget, and then stopped optimising. A public place in Lisbon and a private place in Valencia lead to the same register in the end. Pick the one you can actually finish.

Conclusion

The cheapest complete dentistry degree in these three countries costs about €70,500 over five years in Lisbon; the most expensive about €209,600 in Madrid. The decision is language, city, programme length and funding.

Skipping it means paying the maximum bracket, which at Sapienza Università di Roma was €2,924 plus taxes.
