Not Accepted for Medicine in South Africa? Your 5 Options for 2027

Not Accepted for Medicine in South Africa? Your 5 Options for 2027

Short answer

Rejected from medicine in South Africa? Five options for 2027: reapply, upgrade matric, graduate entry such as the Wits GEMP, or study medicine abroad from €3,500.

At a glance

  • About 58 applicants competed for each MBChB place at Stellenbosch University for the 2026 intake
  • Option 1: reapply next year
  • Option 2: upgrade your matric subjects
  • Option 3: start a BSc and apply for graduate entry, for example the GEMP at Wits
  • Option 4: choose another health profession
  • Option 5: study medicine abroad in English and start this year
  • First step: free consultation with Medschool Experts

The email is short. Your application for the MBChB was unsuccessful. You met the requirements, and so did thousands of others.

You have five options now. Four of them cost you at least a year. One does not.

Last reviewed: October 2026.

Rejected from medicine

In short: A rejection from a South African medical school in 2026 is mostly a matter of seats. Stellenbosch University had 17,363 applications for 300 MBChB places. Applicants who meet the medicine requirements are turned away at every one of the ten medical schools.

Read the letter once, then put it away. It does not measure whether you can become a doctor. I started dental medicine in Sofia in 2015, at 24, later than most, and graduated in 2021. Nobody has asked me since why I started late.

Reapply

In short: You can apply again for the next intake at the South African medical schools. Applications for the following year close around the middle of the year. Your matric marks stay the same unless you upgrade them, and the number of places stays the same too.

Reapplying makes sense when you were close and something changes: better marks, a stronger admission test, a completed first year in another degree. Reapplying with an identical file gives an identical ratio.

Upgrade matric

You can rewrite matric subjects to raise your marks in Mathematics, Physical Sciences, Life Sciences and English. This takes a year. Faculties treat upgraded results differently, so ask each one how it counts them before you invest that year.

Graduate entry

In short: Graduate entry means finishing a first degree, usually a BSc, and applying to medicine afterwards. The best-known programme is the Graduate Entry Medical Programme (GEMP) at the University of the Witwatersrand, where successful applicants join the third year of the MBBCh.

The route works, and it is long. Three years of BSc plus four years of medicine make seven, and the GEMP is selective in its own right. If you already hold a science degree, a transfer abroad with credit for your earlier studies can be shorter. Which universities credit your degree depends on your profile, and we check that in a free consultation.

Alternatives to medicine

Physiotherapy, pharmacy, nursing, dentistry and medical laboratory science are professions of their own. Choose one because you want that work. As a waiting room for medicine they are expensive, and their places are contested as well: the BNursing at Stellenbosch University had 22,556 applications for 50 places for 2026.

Study medicine abroad

In short: South African students study medicine in English at universities in Europe and the Caribbean. Tuition in Europe ranges from €3,500 to €10,000 a year in 2026. The academic year starts in September or October, so an applicant rejected in January can start the same year.

After graduation you can work in Europe, the UK or North America, or return home through the HPCSA board examination, internship and community service. Countries, costs and the return are in our guide to studying medicine abroad from South Africa. The ten universities and their places are in the overview of medical schools in South Africa.

Global view

Applicants without a place in other countries make the same move. German school leavers below the national grade threshold study in Bulgaria and Romania. British applicants without an offer study in Central and Eastern Europe. US Americans who miss a seat at home study in the Caribbean and return for residency. English-taught medical programmes exist for exactly these students, and South Africans are part of that group.

The author's view

I started late myself: at 24, in Sofia, in 2015. I graduated in 2021. The year you begin matters far less than the fact that you begin.

My view: give yourself one week to be angry, then compare the five options on paper, with years and costs. For most applicants who write to us from South Africa, the comparison ends the discussion.

Summary

A rejection from medicine in South Africa reflects the number of seats. You can reapply, upgrade your matric, take the graduate route such as the GEMP at Wits, or choose another profession. Each of these costs at least one year.

Studying medicine abroad in English starts in September or October of the same year and takes six years.

Marcel Kloos

Founder and owner of Medschool Experts, federally recognised dentist

Marcel Kloos is the founder and owner of Medschool Experts, a placement agency for medicine, dentistry, veterinary medicine and pharmacy degrees abroad. Since 2021 the team has secured more than 500 study places at 25+ partner universities in Europe and worldwide.

Study medicine abroad: Marcel's own path

Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. He is a federally recognised dentist.

What Marcel does at Medschool Experts

Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.

Topics Marcel writes about

  • Study medicine abroad independent of your high school diploma grades: universities, costs and admission
  • Study dentistry, veterinary medicine or pharmacy in Europe in English
  • Tuition fees and living costs at medical schools in Europe and worldwide
  • Recognition of foreign medical degrees in the EU, Switzerland, the UK and the US
  • Transferring between medical schools and returning home into a higher semester
