OET or IELTS in 2026 — which one does a foreign-trained health professional actually need?
For most doctors and nurses heading to the United Kingdom, Ireland, Australia or New Zealand, either test is accepted and the choice is yours. For dentists applying to the General Dental Council, the published guidance names IELTS only. For the Educational Commission for Foreign Medical Graduates in the United States, only the Occupational English Test counts.
The first question is not which test is easier. It is which one your regulator names.
The second question is what the test asks you to do. IELTS Academic asks you to describe a bar chart and write an essay. The Occupational English Test asks you to write a referral letter and take a patient history. Same level of English, different task. Clinicians who have spent six years reading case notes often find the second closer to home.
The third question is arithmetic: fees, validity, attempts allowed.
What minimum scores does each regulator require in 2026?
The General Medical Council asks for IELTS Academic 7.0 in every section with 7.5 overall, or grade B in all four sub-tests of the Occupational English Test (Medicine). The Nursing and Midwifery Council accepts a writing score one notch lower. The General Dental Council asks for 6.5 in each IELTS component and 7.0 overall. The numbers below are the published minimums, each with its source and year.

Two details in that table cost people money every year. The General Medical Council wants grade B in all four Occupational English Test sub-tests, writing included, while the Nursing and Midwifery Council accepts C+ in writing. And Ireland requires a single sitting, so a candidate who cleared reading in March and speaking in June holds two half-certificates.
How does the Occupational English Test differ from IELTS Academic?
The listening and reading sub-tests of the Occupational English Test are shared across all professions and set in healthcare contexts. Writing and speaking are profession-specific: a pharmacist writes a letter a pharmacist would write, a physiotherapist role-plays a physiotherapy consultation. IELTS Academic uses general academic material for all four skills, the same paper for an engineer and a midwife.
That is the whole design difference, and it decides more than people expect.
Scoring language differs too, and mixing the two causes real confusion on application forms. IELTS reports bands from 0 to 9 in half-point steps. The Occupational English Test reports 0 to 500 per sub-test with a letter attached: grade B covers 350 to 440, grade C+ covers 300 to 340. Australia moved to the raw numbers for tests taken from 23 April 2026, which is why its table now reads 360 for reading and speaking.
Which regulators accept PTE Academic or TOEFL iBT instead?
Among the regulators covered here, only the Australian Health Practitioner Regulation Agency accepts a wider list. Its English language skills registration standard names five tests: IELTS Academic, the Occupational English Test, PTE Academic, TOEFL iBT and Cambridge C1 Advanced or C2 Proficiency. The United Kingdom, Ireland and New Zealand regulators stay with IELTS and the Occupational English Test.
Australia also rewrote its numbers. For tests taken on or before 22 April 2026, PTE Academic required 66 overall with 56 in writing and TOEFL iBT required 94 overall. From 23 April 2026, PTE Academic reads 63 overall with 58 listening, 59 reading, 76 speaking and 60 writing, and TOEFL iBT reads 91 overall. The agency called this score concordance work and alignment with the Department of Home Affairs, not a change in the level required.
Note the speaking jump. PTE Academic speaking moved from 66 to 76, so a candidate who passed comfortably in March 2026 might not clear the same bar in May 2026 on an identical performance.
Nursing and midwifery in Australia sit under their own National Board, which publishes a separate English language skills standard rather than the common one. Check the board, not the country.
How long are results valid, and can I combine two sittings?
Two years is the common answer. Every regulator in the table above works to a two-year window in its 2026 guidance. What differs is the anchor date and whether two sittings can be stitched together.
The Nursing and Midwifery Council sets out the conditions precisely: both sittings within 12 months of each other, both within the two years before assessment, all four skills attempted each time, no score below the minimum in either sitting, and the same test type throughout. The Australian regulator allows two sittings in a 12-month period and will not mix providers; it extended that window from six months to 12, which is why older advice still says six.
The IELTS One Skill Retake is a separate mechanism. You sit the full computer-delivered test, then repeat one skill within 60 days, once. The General Medical Council accepts it. So does the General Dental Council, alongside IELTS Online. The Nursing and Midwifery Board of Ireland does not, and also excludes IELTS Online and the at-home Occupational English Test. Australia treats a One Skill Retake as part of the original sitting rather than an additional one, which preserves your second full attempt.
What do the tests cost in 2026, and what does one re-sit add?
The Occupational English Test published a full-test fee of AU$587 in 2026, and US$455 for the computer-based version at a venue and for the at-home version in the United States. IELTS in India cost INR 19,000 in 2026 for both the paper and computer formats, with the One Skill Retake at INR 12,650. Fees are set locally; your centre will differ.

A worked example, using those published 2026 figures. A nurse in India sits IELTS Academic on computer for INR 19,000 and scores 7.0 in listening, reading and speaking but 6.0 in writing. The Nursing and Midwifery Council wants 6.5 in writing, so the certificate fails on one number. She books the One Skill Retake inside 60 days for INR 12,650 and repeats writing only. Total outlay: INR 31,650, roughly 1.7 times a single test fee, with no second exam day for the other three skills.
Now run the same failure in Dublin's rulebook. The Nursing and Midwifery Board of Ireland does not accept the One Skill Retake and does not combine sittings, so the same nurse pays a second full fee of INR 19,000 and sits all four skills again. Total outlay: INR 38,000, and a second full test day. Same candidate, same weak skill, two different bills, purely because of where she is applying.

How many weeks should I plan for preparation, booking and results?
Plan roughly 14 to 18 weeks from first practice test to certificate, assuming one re-sit. For a clinician already working in English, preparation is usually eight to ten weeks of structured work. Booking lead time, the test, results turnaround and the re-sit account for the rest.
IELTS runs year-round with multiple dates each month at most locations, and the computer-delivered version releases results faster than paper. The Occupational English Test is offered on paper at a venue, on computer at a venue, and at home for candidates in countries without a physical venue, a structure the Educational Commission for Foreign Medical Graduates sets out for its own applicants.
Seats in the busiest cities go first. In our own placements, candidates in Delhi, Manila and Cairo have looked three to six weeks ahead for a convenient weekend slot, while smaller centres often have seats two weeks out. Book before you feel ready. A date can be transferred for a fee; a full seat cannot be conjured.

This is the question that catches people, and the answer is that expiry is measured against a specific moment in the process, and that moment is not the same everywhere. The Nursing and Midwifery Council times the two years against the point at which your application is assessed. The Australian regulator times it against the date you lodge your application. The Medical Council in Ireland requires the certificate to be dated within two years of both the application and the registration date.
Read that last one again. Ireland's wording means a certificate that was fresh at application can still fall outside the window if the file is decided close to the two-year mark, so a 22-month-old certificate is a different proposition in Dublin than in Melbourne.
If more than six months of validity remain
Submit. Keep the original test report form, and respond to document requests the week they arrive rather than the month they arrive.
If three to six months remain
Ask the regulator in writing which date it measures against, and keep the answer. Book a provisional test seat you can transfer.
If under three months remain
Re-sit now and submit the fresh certificate with the file. A certificate that expires mid-assessment usually means supplying a new one, not starting again.
There are recognised ways out that do not involve another exam day. The Australian standard accepts results older than two years where the applicant has worked continuously as a registered health practitioner in an environment where English was the main language of practice, or finished approved study within 12 months of lodging. The Medical Council of New Zealand extends validity to five years for repeat candidates at its registration examination who have lived continuously in specified English-speaking countries. Both also publish exemptions based on where a qualification was taught and examined.
The practical habit that prevents most of this: date your certificate backwards from your intended registration decision, not forwards from your exam day. If your regulator's queue runs several months and your paper is 20 months old, you are already inside the risk zone. Where does the United States route sit, and why is there no IELTS option?
The United States handles English differently. There is no standalone English test in the licensing exam sequence; the requirement lives inside certification by the Educational Commission for Foreign Medical Graduates. For applicants using a Pathway, the commission requires the Occupational English Test Medicine with minimum scores of 350 in listening, reading and speaking and 300 in writing, achieved in one administration.
No exceptions are made for native speakers or for graduates of English-medium medical schools. The commission states this plainly: all Pathways applicants must satisfy the requirement regardless of first language, language of instruction at medical school or citizenship.
One group sits outside it: doctors who passed the former Step 2 Clinical Skills examination before it was discontinued in 2020. That pass still satisfies both requirements, and those doctors are not eligible to pursue a Pathway.
Timing matters here more than anywhere. For the 2027 Pathways, results had to be obtained on or after 1 January 2025, and the commission advises testing by the last scheduled December 2026 date so scores reach the Match in time. Miss that window and the application year moves.
Which test suits which profession in 2026?
Dentists applying to the General Dental Council have the narrowest choice, because the published guidance names IELTS and sets 6.5 in each component with 7.0 overall. Doctors and nurses heading to the United Kingdom, Ireland, Australia or New Zealand can choose either test. Doctors aiming at the United States take the Occupational English Test or nothing.
Pharmacists and physiotherapists are given the wrong advice most often, because their regulator sits in a different place depending on the country. In Australia, pharmacy and physiotherapy fall under the common English language skills registration standard alongside medicine and dentistry. In the United Kingdom and Ireland, each profession has its own regulator and its own numbers.
The Occupational English Test covers twelve healthcare professions, which is why a physiotherapist sitting it writes a physiotherapy letter rather than a medical one. That specificity is the argument for it. The argument for IELTS is availability, price in many markets, and one certificate that can serve a university application, a visa file and a registration file at once.

I work on visa and immigration law, and the English certificate is the document I see fail most often, not because people cannot speak English but because they misread a date. I have lived in Dubai and in India, and in both places I met nurses and doctors who had passed their test comfortably and then let the paper sit in a drawer for eighteen months while they finished a contract or waited for a family decision. By the time the registration file moved, the clock had run down.
From Belgrade, where I am based now, I deal with the same pattern from the other direction: applications arriving at a regulator with a certificate that was perfectly valid on the day it was posted and marginal by the day it was read. My habit is to write the expiry date on the front page of every client file in red, next to the police certificate date and the medical report date, because those three documents age on the same clock and nobody thinks about them until the week they matter.
If I could hand one piece of advice to every candidate, it would be this: treat the test as the first step of the registration, not as a separate errand you finish beforehand. Book it once you know your target regulator and your target start date, so the two-year window covers the whole journey rather than just the beginning of it. That single reordering saves more applications than any study plan I have seen.

The Australian regulator uses the same rule and adds that results from different test providers cannot be combined.
