Study medicine abroad from South Africa: English-taught medical schools in Europe from €3,500 a year, the Caribbean route and how you return via the HPCSA.
Study medicine abroad from South Africa
In short: South African students study medicine abroad at English-taught universities in Europe and the Caribbean. The degree takes six years in Europe, the university is listed in the World Directory of Medical Schools, and Medschool Experts arranges the place at one of more than 25 partner universities.
The numbers at home explain the interest. The University of Pretoria can take 300 first-year MBChB students. The University of KwaZulu-Natal has had about 12,000 applications for 250 places. The Faculty of Health Sciences at the University of Cape Town counted 18,931 applications for 460 places across all its programmes for 2026.
Abroad, the bottleneck is different. Universities with English-taught programmes plan seats for international students every year, and a complete application is what counts. We place students there reliably.
Countries and costs
In short: Tuition for English-taught medicine in Europe ranges from €3,500 a year in Bosnia and Herzegovina to €10,000 a year in Bulgaria in 2026. Living costs in these university cities are about €5,000 to €8,000 a year. Six years of tuition in Foča add up to €21,000.
| Country | University | Tuition per year (2026) | Six years of tuition |
|---|---|---|---|
| Bosnia and Herzegovina | University of East Sarajevo, Foča | €3,500 | €21,000 |
| Serbia | University of Niš | €6,000 | €36,000 |
| Romania | University of Galați | €6,000 | €36,000 |
| Bulgaria | Medical University Pleven | €9,000 | €54,000 |
| Bulgaria | Medical Universities Sofia, Plovdiv, Varna | €10,000 | €60,000 |
The table shows tuition only. Add rent, food, flights home and the student visa. Rent in a university town such as Foča or Niš is low by European standards, and that carries a large part of the budget.

Request a free consultationStudy medicine in Europe in English
In short: European medical schools with English programmes teach all six years in English. Lectures, textbooks and examinations are in English. Students learn the local language alongside, because in the clinical years they talk to patients on the wards.
I studied dental medicine in the English programme of the Medical University of Sofia from 2015 to 2021. The lectures and examinations were in English, and Bulgarian was a subject on the timetable, because the clinical years bring patients.
For South Africans the fit is practical. School was in English, and the academic year in Europe starts in September or October. Matric results arrive in January. That leaves months for documents, legalisation and the visa instead of a rushed week.
Caribbean medical schools
In short: Caribbean medical schools follow the American model: basic sciences on the island, then clinical rotations in teaching hospitals in the USA. The curriculum prepares for the USMLE. Graduates apply for residency in the USA or Canada, and the degree also opens a route into Europe.
This is the route for students who see themselves in North America. Our partner university in the Caribbean is the University of Health Sciences Antigua. You study in English from the first lecture.
After graduation
In short: A medical degree from abroad leads to four destinations: Europe, the United Kingdom, North America and South Africa. Each has its own licensing step. Europe asks for the language of the country, the UK for the PLAB test, the USA for the USMLE, and South Africa for the HPCSA board examination.
| Destination | Licensing step | Authority |
|---|---|---|
| European Union | Degree from an EU university, language certificate of the country, work visa | National medical authority |
| United Kingdom | PLAB 1 and PLAB 2 | General Medical Council |
| USA | USMLE Step 1 and Step 2 CK, ECFMG certification, residency | ECFMG and state medical boards |
| South Africa | Board examination, internship, community service | Health Professions Council of South Africa |
Returning to South Africa
In short: South African citizens with a medical degree from abroad register with the Health Professions Council of South Africa. The university must be listed in the World Directory of Medical Schools. The Council verifies the degree, then follows the board examination with a theory section and a practical section, internship and one year of community service.
The board examination is demanding, and it rewards months of structured preparation. We plan for it from the first year: you keep your curriculum, transcripts and proof of clinical hours in one file, because the Council asks for exactly these documents. The details are in our guide to HPCSA registration with a foreign medical degree.
Many students decide late where they will work. Six years abroad change plans, and a doctor with a European degree and a South African passport can choose.
Timeline
- January: matric results. Free consultation and choice of university.
- February to May: application file, certified copies, legalisation of the certificate.
- June to August: admission letter, student visa, accommodation.
- September or October: first lecture.
Students who finished matric in earlier years, or who already hold a degree, start with the same first step. Which universities fit your profile is part of the consultation.

The author's view
I was 24 when I started dental medicine in Sofia in 2015, older than most first-year students, and I graduated in 2021. The late start has not mattered on a single working day since.
South African applicants who write to us often have marks that would have earned a seat in most countries. My view: a year spent reapplying is a year of medicine you do not study. If the offer has not come by January, look at the map.
Summary
South Africa has far more qualified applicants than MBChB seats. English-taught medical schools in Europe cost between €3,500 and €10,000 a year in tuition, and Caribbean schools lead towards the USA and Canada.
After graduation you work in Europe, the UK or North America, or you come home through the HPCSA board examination, internship and community service.
Conclusion
Send us your matric results, and request your free consultation. You will know within 30 minutes where you can start.

Study medicine abroad: Marcel's own path
Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. In Switzerland he completed the federal recognition procedure and holds a Swiss dental licence.
What Marcel does at Medschool Experts
Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.
