Study medicine in Cuba or Europe? How the Cuban programme works for South Africans, Spanish language, costs, HPCSA recognition and the English-taught alternative.
Study medicine in Cuba: how it works for South Africans
In short: South Africans study medicine in Cuba through the Nelson Mandela-Fidel Castro Medical Programme. Provincial health departments such as Gauteng Health select the students, the National Department of Health pays the fees, and teaching is in Spanish. For the 2025 intake Gauteng asked for at least 50% in Physical Sciences, Life Sciences, Mathematics and English.
The programme is a government arrangement between South Africa and Cuba. It is built to train doctors for the public health system, which is why the selection criteria look the way they do: South African citizens, living in the province, aged 18 to 30, without a place at another institution and without another bursary.
The number of seats has moved a lot. Gauteng sent 264 students in 2019, and the figure fell to 25 in 2021. A student planning around a Cuban place should treat it as a government allocation, not as an application that any qualified school leaver can make.
Cuba vs Europe compared
| Question | Cuba | English-taught medicine in Europe |
|---|---|---|
| Language | Spanish, with a preparatory Spanish course first | English from the first lecture |
| How you get a place | Selection through the government programme, a limited number of seats | Application through Medschool Experts, with a start date at our partner universities |
| Who pays | The National Department of Health, R146,580 a year per student | The family, tuition from €3,500 to €10,000 a year |
| Entry criteria | Gauteng 2025: 50% in four subjects, South African citizen living in the province, age 18 to 30 | School certificate with the sciences, checked in the consultation |
| Way back to South Africa | HPCSA recognises the qualification, return supported by provincial coordinators | HPCSA verification and board examination, then internship and community service |
Language: Spanish or English
Lectures, clinical teaching and examinations in Cuba are in Spanish. Students who do not speak it start with a preparatory Spanish course before the medical years begin. Patients speak Spanish too, so the language has to be good enough for history taking and for examinations.
At the English-taught medical schools in Europe, the degree is delivered in English. Patient contact in the clinical years happens in the local language, and the universities teach it alongside the degree. Which language suits you is a practical question: a Spanish year is an investment of time, and some students prefer to spend it in the lecture hall.
Costs: government programme or family budget
In short: Under the Cuban programme the National Department of Health carries the training fee of R146,580 per student per year. In Europe the family pays tuition of €3,500 at Foča, €6,000 at Niš and Galați, €9,000 at Pleven and €10,000 at Sofia, Plovdiv and Varna.
For a family, the Cuban route costs little, and the price is the set of conditions that come with a government place. In Europe the costs are visible from the start, and so is the start date.

In short: The Department of Health states that the Health Professions Council of South Africa recognises Cuban medical qualifications. Graduates of the government programme are supported on their return by provincial coordinators and local universities.
Recognition of the Cuban degree is settled for the government programme. For a degree from a European university the steps are the same ones every South African foreign graduate follows: verification, board examination, internship, community service. Each step, with the fees, is in our HPCSA board exam and registration guide.
Study medicine in Europe in English
Students who do not receive a Cuban place, or who prefer English, study at English-taught medical schools in Europe. The degree takes six years, the academic year starts in September or October, and our partner universities decide on the school certificate, not on a national ranking list. Countries, costs and the timetable from January to October are in our guide to studying medicine abroad from South Africa.

The author's view
I started dental medicine in Sofia in 2015, at 24, in English, and graduated in 2021. I have never studied in Cuba, and I do not claim to know its lecture halls.
My view: the useful question for a South African is not Cuba or Europe as places, but which seat exists for you. The Cuban seats are tied to a government programme with few places. A fee-paying seat in Europe is the one you can book today.
Summary
Cuba trains South African doctors through a government programme with Spanish as the language of teaching, a limited number of seats and fees paid by the National Department of Health. HPCSA recognises the qualification.
English-taught medical schools in Europe take fee-paying students from €3,500 a year, with a confirmed start date.
Conclusion
Apply to the Cuban programme if you qualify and live in a participating province, and secure a university in Europe for the same year. Request your free consultation.

Study medicine abroad: Marcel's own path
Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. In Switzerland he completed the federal recognition procedure and holds a Swiss dental licence.
What Marcel does at Medschool Experts
Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.
