How Many Medical Schools Should You Apply To? A Strategy for 2026

Short answer

How many medical schools to apply to in 2026: 4 UK medicine choices, 15 to 20 US applications, the real cost of spreading and the placement route.

When I applied for dentistry in 2015, I sent one set of documents to one university: the Medical University of Sofia. Friends of mine in Germany were doing the opposite at the same time, filling in central allocation forms, ranking cities and waiting for a grade cut-off they could not influence. I started in Sofia that autumn and graduated in 2021.

The question "how many medical schools should I apply to" has two very different answers in 2026. In systems that select by grades and test scores, the answer is "as many as the rules allow", because each application is a lottery ticket with a known price. On a placement route that is independent of high-school diploma grades, the answer can be one.

Below are the numbers for both strategies: what applicants in the United Kingdom, the United States and Germany do, what it costs, and how Medschool Experts turns one profile check into a secured place at one of our 25+ partner universities.

How many medical schools should you apply to in 2026?

In 2026, applicants in grade-based systems apply to as many medical schools as the rules allow: four medicine choices in the United Kingdom, typically 15 to 20 schools in the United States, and a full ranked list in Germany. On a placement route abroad that is independent of high-school diploma grades, one well-matched university is enough, because the place is secured after a single profile check.

The two answers do not contradict each other. They follow from two different mechanics. In one, many applicants compete for a fixed number of seats and the medical school picks. In the other, the student and an expert pick the university first, and the place follows.

Why do applicants in grade-based systems apply so widely?

Applicants in grade-based systems apply widely because each medical school decides independently and most applications end in rejection. When places are scarce, a candidate with good grades still faces many competitors with similar grades. Spreading applications raises the chance that at least one school says yes. It is a rational response to scarcity, not a sign of a weak candidate.

Look at the United States. In 2025, 54,699 people applied to US medical schools and 23,440 started in the first year. Fewer than half got a seat. An applicant who sends only three applications in that system takes a large risk. One who sends seventeen takes a smaller one and pays for it.

US medical schools in 2025: 54,699 applicants and 23,440 new first-year students. Fewer than half of applicants started medical school that year.

The United Kingdom works the other way round. The UCAS form for 2026 entry allows five choices in total, but no more than four in medicine, dentistry or veterinary medicine. Applicants cannot spread further, so they spread across those four as carefully as they can and keep the fifth choice for a related subject.

What does applying to many medical schools cost?

Applying to many medical schools costs between a few hundred and several thousand euros per cycle in 2026, depending on the country. A US applicant with 17 schools pays about 3,000 to 5,000 US dollars for application fees, secondary applications, the MCAT and interview travel. A UK applicant pays the UCAS fee of 28.95 pounds and an admissions test fee, plus preparation and travel.

Here is a worked example for one US applicant in the 2026 cycle. It uses the fixed AMCAS fees of 175 dollars for the first school and 47 dollars for each additional school.

Money is only one part. Each secondary application asks for new essays, often three to five per school. Seventeen schools can mean fifty essays written between June and September. Students write them next to a full semester or a job. That time has a price too. If the cycle ends without a seat, most of the cost repeats the next year.

Is applying to more schools always better?

Applying to more medical schools raises the chance of at least one offer only up to a point. Beyond that, each extra application adds cost and essay work but little probability, because the same grades and test scores are judged again by schools with similar standards. Quality of each application drops when the number gets too high.

Advisers in the United States usually recommend a balanced list across several types of school rather than the maximum number. In the United Kingdom the cap of four removes the question. In Germany the central system does the ranking. In all three, the grade remains the main input. More applications do not change the grade. They only spread the same profile wider.

This is the real limitation of the wide strategy. It manages the risk. It does not remove it.

How does central allocation in Germany change the strategy?

In Germany, medical study places are allocated centrally in 2026. Applicants submit one application and rank their preferred universities. Thirty per cent of places go by school grade alone, ten per cent through an additional aptitude quota that does not use the grade, and sixty per cent through university selection procedures that combine the grade with other criteria. The number of applications is fixed at one.

German applicants therefore cannot spread their risk by sending more forms. Their strategy is the ranking itself and waiting semesters. Many German students who do not receive a place look abroad, where the decision does not depend on the school-leaving grade. That is why a large share of our students at Medschool Experts come from Germany, Austria and Switzerland.

How much time does a wide application strategy cost?

A wide application strategy in a grade-based system usually takes 12 to 18 months from the first test to the first day of medical school, and an unsuccessful cycle adds another full year. In the United Kingdom, applicants submit in October and start the following September. In the United States, the primary application opens in May and studies begin more than a year later.

Put two unsuccessful cycles into a timeline and the picture changes. A student who leaves school at 18 and tries twice starts medicine at 20 or 21, graduates at 26 or 27 and begins specialist training even later. Nothing is wrong with a later start. I began at 24 myself. But the years between should be a choice, not a waiting room.

The placement route compresses that timeline. The consultation, the profile check and the place usually fit inside one season, so a student who finishes school in June can sit in an anatomy lecture in the autumn.

How is a placement route different from applying to many schools?

A placement route replaces the many-applications strategy with one matched placement. Medschool Experts checks the student profile once, recommends the partner university that fits budget, language and goals, and secures the place there. Placement is independent of high-school diploma grades: neither the grade average nor an entrance test decides the place. There is no list of seventeen schools and no cycle of waiting.

One profile check leads to one secured place. Placement is independent of high-school diploma grades.

The work moves from writing many applications to choosing well. Which country, which city, which budget over six years, which route back home. Those questions get answered once, in a consultation, and the result is a single university. The details of each university's own procedure depend on the profile, so we handle them in the consultation rather than in a checklist. The full scope is on the services page.

What does one placement cost compared with many applications?

The consultation with Medschool Experts is free, and a placement replaces the repeated application fees, essays and travel of a wide strategy. The main cost on the placement route is the tuition itself: at the Medical University of Sofia, the Medical University of Plovdiv and the Medical University of Varna it is 10,000 euros a year in 2026, at the Medical University of Pleven 9,000 euros a year, and considerably less in Bosnia and Herzegovina, Serbia and Romania.

The larger saving is time. A student who goes through two unsuccessful application cycles at home loses two years before medical school starts. At the end of a career, those are two years of a doctor's salary. Starting in the next intake abroad keeps them. The universities are listed on the universities page.

International perspective: how this works in other countries

Every country shapes the "how many" question differently in 2026. In the United Kingdom, the cap is four medicine choices on one UCAS form, with the medicine deadline on 15 October of the year before entry. Applicants usually sit an admissions test such as the UCAT, and a strong candidate can still receive four rejections in a competitive year.

In the United States, medicine follows a bachelor's degree, and applicants send a primary application through AMCAS and then secondary applications to each school. Lists of 15 to 20 schools are common. Canada has fewer medical schools and many applicants apply in several provinces. In Italy, the semestre filtro has replaced the entrance test since 2025/26: in the first round there were 17,278 places in medicine and 22,688 students passed both exams, so about 5,000 eligible students had to choose a related course.

Germany, Austria and Switzerland combine grades or tests with fixed quotas. In the Gulf states, admission to public medical schools follows the national school-leaving average. The pattern is the same everywhere: where grades decide, students spread applications. Where a placement decides, they choose one university with care.

A practical strategy for 2026

A practical strategy for 2026 combines both routes: apply at home within the rules, and plan the placement abroad in parallel, so that a rejection does not cost a year. Students who start the conversation with Medschool Experts before home results arrive can keep a place for the next intake while they wait.

Three steps make this concrete. Write down your budget for six years. Decide which country you want to work in later. Then book a free consultation and send your documents once. Many families also read the student stories before deciding.

The author's view

In 2015 I watched friends in Stuttgart rank universities on a central form and wait months for a cut-off. I sent one application to Sofia and spent those months learning the first words of Bulgarian. Neither path was wrong. One just left a lot less to chance.

From Bern, the families I speak to often arrive after two cycles of applications at home. They have spent money, but mostly they have spent years. When I show them that one profile check can lead to a secured place, the relief is usually about time, not money.

My assessment: apply at home if you want to, within the rules. Plan the alternative at the same time. The worst outcome is not a rejection. It is a rejection with no plan for the next morning.

Summary

  • The UK allows 4 medicine choices on one UCAS form for 2026 entry, with a fee of 28.95 pounds.
  • US applicants typically apply to 15 to 20 schools, and in 2025 there were 54,699 applicants for 23,440 first-year places.
  • A US list of 17 schools costs about 3,000 to 5,000 US dollars in fees, tests and travel in 2026.
  • Germany allocates medical places centrally with one application, 30 % of places by grade alone.
  • More applications spread the same profile wider but do not change the grade.
  • A placement route independent of high-school diploma grades needs one profile check and one university.

Conclusion

How many medical schools to apply to depends on the system. In grade-based systems the arithmetic is "as many as you can afford and write well". On a placement route it is one, chosen carefully.

Frequently asked questions about how many medical schools to apply to

How many medical schools should I apply to in the US?

Applicants to US medical schools typically apply to 15 to 20 schools in 2026. In 2025, 54,699 people applied and 23,440 started medical school, so a wider list lowers the risk of no offer. Each school adds 47 dollars in AMCAS fees plus a secondary fee, so the list should balance chance and cost.

How many medical schools can I apply to through UCAS?

For 2026 entry, UCAS allows five choices in total, of which a maximum of four can be in medicine, dentistry or veterinary medicine. The fifth choice is usually used for a related subject. The application fee is 28.95 pounds and the deadline for medicine is 15 October of the previous year.

How many applications can I send for medicine in Germany?

In Germany, medical places are allocated centrally, so each applicant submits one application and ranks preferred universities. Places are split into quotas: 30 per cent by school grade, 10 per cent through an aptitude quota without the grade and 60 per cent through university selection procedures that combine the grade with other criteria.

How much does it cost to apply to medical school?

In 2026 a US applicant with 17 schools pays about 3,000 to 5,000 US dollars for AMCAS fees, secondary applications, the MCAT and interview travel. A UK applicant pays 28.95 pounds to UCAS plus an admissions test fee and preparation costs. Repeating a cycle repeats most of these costs.

Is it better to apply to more medical schools?

Up to a point. More applications raise the chance of at least one offer in grade-based systems, but each extra school adds cost and essay work while the grades and test scores stay the same. A balanced list of well-prepared applications usually works better than the maximum number of rushed ones.

Do I need to apply to several universities abroad?

Not on a placement route. Medschool Experts checks the student profile once and secures a place at one of its 25+ partner universities in Europe and the Caribbean. Placement is independent of high-school diploma grades, so neither the grade average nor an entrance test decides the place.

Can I apply at home and abroad at the same time?

Yes. Many students apply within their home system and plan a placement abroad in parallel, so that a rejection at home does not cost a year. Medschool Experts can prepare the placement for the next intake while home results are still pending, which keeps both options open.
