NHS Medical Doctor Degree Apprenticeship 2026: Current Status and the Alternatives

NHS Medical Doctor Degree Apprenticeship 2026: Current Status and the Alternatives

Short answer

The NHS medical doctor degree apprenticeship in 2026: first pilot in 2024, paused in 2025, version 1.0 retired in March 2026, and the European route to the NHS.

Since the autumn of 2024 the same question has reached our office in Bern from British families again and again: the NHS now trains doctors as paid apprentices, with no tuition fees, so should a son or daughter wait for that instead of studying abroad? My answer in 2024 was already cautious. One pilot, very few places, a long queue.

Two years later the answer is clearer. The medical doctor degree apprenticeship was meant to be a new way into medicine in England, earning a salary while studying. The first pilot cohort started in September 2024. In 2025 the government paused the programmes, and in March 2026 version 1.0 of the apprenticeship standard was retired.

For applicants in 2026 that means the apprenticeship is not a route they can plan on. A European medical degree is, and at the 25+ partner universities of Medschool Experts the place does not depend on a test score or on high-school diploma grades.

What was the NHS medical doctor degree apprenticeship?

The medical doctor degree apprenticeship was a scheme in England to train doctors as employees of the NHS. Apprentices would study for a full medical degree at a university while working for an NHS employer, receive a salary, and pay no tuition fees. The apprenticeship standard, called Doctor (degree) at Level 7, was approved in 2022 with a typical duration of five years.

The idea came from the NHS Long Term Workforce Plan, which aimed to widen access to medicine for people who could not afford five or six years without income. Apprentices would graduate with the same medical degree as other students and then enter the same foundation training. The curriculum was the university's normal medical curriculum. The employer paid the wage.

Apprenticeship funding came from the apprenticeship levy, with a maximum of 27,000 £ for the training costs listed in the standard.

What is the status of the medical doctor degree apprenticeship in 2026?

In 2026 the medical doctor degree apprenticeship is not open as a national route. The first pilot cohort started in September 2024, a second small pilot cohort followed in 2025, the government paused the programmes in 2025, and NHS England confirmed it would not fund an expansion. Version 1.0 of the apprenticeship standard was retired on 25 March 2026.

Apprentices already enrolled keep their funding and continue to qualification. New applicants have no funded national intake to apply for in 2026 or 2027. A version 2.0 of the standard is listed as approved, but an approved standard alone does not create places. Places need employers, universities and funding, and in 2026 those are not in place for a new intake.

Level 7 levy funding from January 2026Limited to apprentices aged 16 to 21

A wider funding change matters too. From 1 January 2026, government levy funding for Level 7 apprenticeships in England is limited to apprentices aged 16 to 21, with small exceptions. Most medical schools take students older than that once a gap year or a first degree is counted.

How many places did the pilot offer?

The pilot offered a small number of places, and thousands of people applied. Anglia Ruskin University ran the first cohort from September 2024, with the University of Central Lancashire and the University of Plymouth named as pilot sites. The exact number of apprentices was never large enough to change the national picture.

Apprentices in the pilot worked in hospitals outside term time without direct patient responsibility, and received a salary that was reported between 14,606 £ and 22,038 £ a year. Subsidised accommodation was part of some offers.

Apprentice pay, pilot figures 2024/25Lower end£14,606Upper end£22,038

That pay was the attraction. It also made each place expensive for the NHS, which is one reason the scheme did not grow.

Why was the apprenticeship paused?

The medical doctor degree apprenticeship was paused in 2025 after the government reviewed apprenticeship funding and NHS England decided not to expand the pilot. Doctors' organisations had also raised concerns about workload, supervision and how apprentices would fit into medical school teaching, and a student survey across the three pilot sites in 2025 showed little appetite among existing students.

The money question was simple. Paying a salary for five years on top of the training costs made each apprentice far more expensive than a regular medical student. With levy funding for Level 7 narrowed from January 2026, the economics became harder still. Plans can change again. For an applicant deciding in 2026, they have not.

What does the pause mean for applicants?

For applicants in 2026 the pause means the apprenticeship is not a route to plan around. Applicants who want to study medicine in the United Kingdom go through the regular UCAS route with the UCAT, school grades and interviews, or look for a degree abroad that leads back to the NHS. Waiting for a new apprenticeship intake is a plan without a date.

UK medical schools remain very competitive. Many strong applicants receive no offer, and a second attempt costs a year. That is the gap the European route fills, because the study place there does not depend on a test score or on school grades.

How does the European route lead back to the NHS?

A graduate of a European partner university reaches an NHS post in four steps in 2026: a six-year medical degree, English evidence, GMC registration and an application to an NHS employer. Holders of a relevant European qualification from an EU medical school can register with the GMC without PLAB under the recognition arrangement the UK kept in place in 2023. Graduates from partner universities outside the EU take PLAB 1 and PLAB 2.

The English requirement is IELTS Academic 7.5 overall with 7.0 in each part, or OET Medicine with grade B in all four parts. PLAB 1 has 180 questions, PLAB 2 has 16 clinical stations in Manchester, and both follow the same content map as the UKMLA that UK graduates sit since the 2024/25 academic year. The details are in our article on the UKMLA and PLAB.

Medschool Experts does not place students at British or Irish universities. The route goes through Europe and comes back to the UK with a recognised degree.

What does the European alternative cost?

Six years of medicine at the Medical University of Plovdiv in Bulgaria cost about 106,000 euros in 2026, living costs and flights included. Tuition is 10,000 euros a year, so 60,000 euros over six years. Living costs of around 600 euros a month add 43,200 euros, and four return flights a year to the UK add about 2,880 euros.

The Medical University of Pleven charges 9,000 euros a year, and partner faculties in Serbia and in Bosnia and Herzegovina charge less. A UK student at an English medical school pays about 9,500 £ a year in tuition, plus living costs in a British city, which brings five or six years to a similar or higher total. The apprenticeship would have been cheaper for the student. It is simply not available.

One cost is easy to forget. English-taught programmes include the local language for the clinical years, which means several hundred hours of Bulgarian over the first three years in Plovdiv. It is work, and it pays off on the ward.

How does a UK applicant start the European route in 2026?

A UK applicant who wants to start a European medical degree in 2026 or 2027 begins with a profile review: school results, any first degree, the preferred country and the budget. From there Medschool Experts proposes partner universities, prepares the documents and plans the start, which is usually in autumn, with some faculties also starting in spring.

British citizens need a student residence permit in EU countries since Brexit, and the paperwork differs by country. Medschool Experts prepares that file as part of the placement. Which university fits depends on your profile, so book a free consultation and we look at it together.

Timing matters more than most families expect. The autumn start fills during the summer, and a decision in October usually means a spring or next-autumn start. One phone call in time saves months.

International perspective: how this works in other countries

Paid routes into medicine are rare everywhere. In Germany, some federal states reserve medical study places for students who commit to working as rural doctors after graduation, and the German armed forces pay medical students who sign a long service contract. Both are commitments rather than apprenticeships, and both come with years of obligation.

The United States has military scholarships that cover medical school in exchange for service years, and Australia has bonded medical places that require work in under-served regions. In the Gulf, governments in Saudi Arabia, the United Arab Emirates and Kuwait sponsor citizens to study medicine abroad, often in Europe, with a duty to return and work at home.

Seen next to these schemes, the English apprenticeship was unusual because it paid a wage during the degree itself. That made it attractive and expensive at the same time. Most systems choose a cheaper model: the student studies first, and the obligation or the salary comes after the diploma. A European degree with a clear licence plan follows that model and is open every year.

Which applicants should look at Europe now?

Applicants who should look at a European medical degree in 2026 are those without a UK offer, those who were waiting for the apprenticeship, career changers with a first degree, and families who want a fixed plan rather than another application cycle. The degree takes six years, and the start can be this autumn or next spring.

Applicants with a first degree in a health or science subject may be able to start in a higher year. Medschool Experts is the leading expert worldwide in credit recognition and frequently achieves placement into year 4 or year 5 at top universities. Compare the faculties on the universities page, and see how Medschool Experts supports the whole route on the services page.

The author's view

I started dentistry in Sofia in 2015 at 24, after law, with savings and no salary. I would have loved a paid route. So I understand why the apprenticeship sounded good to the British families who asked us about it in 2024.

From Bern I have watched the scheme for two years now, and my view has not changed. A route with a handful of places and no funded intake is not a plan. A six-year degree with a clear licence path is.

My assessment: if the apprenticeship returns at scale, good. Do not build your next six years on it. Start the degree, and keep the NHS as the destination.

Summary

  • The medical doctor degree apprenticeship was designed to let apprentices earn a salary while studying medicine in England, without tuition fees.
  • The first pilot cohort started in September 2024 at Anglia Ruskin University, with pay reported between 14,606 £ and 22,038 £ a year.
  • The government paused the programmes in 2025, and version 1.0 of the standard was retired on 25 March 2026.
  • In 2026 there is no funded national intake; current apprentices continue to qualification.
  • The European route leads to the NHS via GMC registration, directly with a relevant European qualification or through PLAB.
  • Six years at the Medical University of Plovdiv cost about 106,000 euros all-in in 2026.

Conclusion

The apprenticeship was a good idea on paper and a costly one in practice. In 2026 it is a pilot with a pause, not a door that applicants can walk through.

Frequently asked questions about the NHS medical doctor degree apprenticeship

Is the NHS medical degree apprenticeship still running in 2026?

Not as an open national route. The pilot started in September 2024, the government paused the programmes in 2025 and NHS England did not fund an expansion. Version 1.0 of the standard was retired in March 2026. Apprentices already enrolled continue, but there is no funded intake for new applicants in 2026.

Which universities ran the medical apprenticeship pilot?

Anglia Ruskin University ran the first cohort from September 2024. The University of Central Lancashire and the University of Plymouth were named as pilot sites. The total number of apprentices across the pilot was small compared with the thousands of applications received.

How much did medical apprentices earn?

Apprentices in the pilot received a salary reported between 14,606 £ and 22,038 £ a year, with tuition paid through the apprenticeship levy up to 27,000 £. Some offers included subsidised accommodation. Apprentices worked in hospitals outside term time without direct patient responsibility.

Will the medical apprenticeship come back?

No national relaunch has been announced for 2026 or 2027. A version 2.0 of the apprenticeship standard is listed as approved, but places need employers, universities and funding. Since January 2026, Level 7 levy funding is limited to apprentices aged 16 to 21, which makes a large relaunch less likely.

What are the alternatives to the medical apprenticeship?

The alternatives are a regular UK medical degree through UCAS, with the UCAT, school grades and interviews, or a medical degree at a European university. At the 25+ partner universities of Medschool Experts the place does not depend on a test score or school grades, and graduates return to the NHS through GMC registration.

Can I work in the NHS with a European medical degree?

Yes. Graduates with a relevant European qualification from an EU medical school can register with the GMC without PLAB under the arrangement the UK kept in place in 2023. Graduates from other European partner universities take PLAB 1 and PLAB 2. English evidence at IELTS 7.5 or OET grade B is required.

Does Medschool Experts place students at UK medical schools?

No. Medschool Experts does not place students at British or Irish universities. It places students at 25+ partner universities in Europe and the Caribbean and plans the licence route back to the United Kingdom, the EU, the Gulf or the United States from the start.
