Short answer

How to become a doctor in South Africa: matric subjects, six years of MBChB, two years of internship, one year of community service, plus the GEMP and the route abroad.

At a glance

  • Nine years from first lecture to independent practice: six years of MBChB, two years of internship, one year of community service
  • Matric subjects: Mathematics, Physical Sciences, English, often Life Sciences
  • Ten public universities offer the degree, each with about 250 to 300 first-year places
  • Route two: a first degree and the Graduate Entry Medical Programme at Wits
  • Route three: study medicine abroad in English and register through the HPCSA board examination
  • First step for route three: free consultation with Medschool Experts

The path to the title Dr is fixed by law and by the Health Professions Council of South Africa. The hard part is the first step, the place at medical school.

Here is the path step by step, with the years, and the two other routes that lead to the same registration.

Last reviewed: October 2026.

How to become a doctor in South Africa

In short: To become a doctor in South Africa you complete matric with Mathematics and Physical Sciences, a six-year MBChB or MBBCh degree, a two-year internship and one year of community service. After these nine years the Health Professions Council of South Africa registers you for independent practice.

StepDurationWhat happens
1. MatricGrade 10 to 12Mathematics, Physical Sciences, English, often Life Sciences, at high marks
2. Medical degree6 yearsMBChB at nine universities, MBBCh at Wits. Registration as a medical student with the HPCSA
3. Internship2 yearsRotations in accredited public hospitals
4. Community service1 yearWork in a public facility, often rural
5. Independent practicefrom year 10Registration as medical practitioner, then general practice or specialist training as a registrar

Matric and admission

Medical schools ask for Mathematics, Physical Sciences and English, several also for Life Sciences. The University of Pretoria sets an APS of at least 35. The details per university are in our guide to medicine requirements in South Africa.

Then comes the selection. Stellenbosch University had 17,363 applications for 300 MBChB places for the 2026 intake. Most applicants who qualify on paper do not receive an offer in their first year of trying.

Internship and community service

In short: Every medical graduate in South Africa completes a two-year internship and one year of community service in the public health system. Both are paid posts. Only after the community service year does the HPCSA register a doctor for independent practice.

The three years after graduation are part of the training, and they apply to everyone who wants to practise independently in South Africa, wherever the degree was earned.

Graduate entry

The second route starts with another degree. The Graduate Entry Medical Programme at the University of the Witwatersrand admits graduates into the third year of the MBBCh, so medicine takes four more years. The minimum is an average of 70% in the first degree, and the threshold for acceptance is usually between 76 and 79%. Requirements, the WAPT and the dates are in our article on the GEMP at Wits.

Study medicine abroad

In short: South Africans who study medicine abroad return through the HPCSA. The university must be listed in the World Directory of Medical Schools. The Council verifies the degree, the graduate passes the board examination with a theory part and a practical part, and internship and community service follow in South Africa.

English-taught medical schools in Europe charge between €3,500 and €10,000 tuition a year in 2026. The degree takes six years and starts in September or October. The board examination costs R12,000 per attempt. Our HPCSA guide explains each step, and the guide to studying medicine abroad from South Africa covers countries and costs.

RouteYears of studyExtra step
MBChB in South Africa6Selection at one of ten universities
First degree plus GEMP at Wits3 + 4WAPT and ranking by academic rating

All three end in the same internship, the same community service year and the same registration. South African applicants reach us fastest through our South Africa page.

The author's view

I became a dentist abroad: Sofia, 2015 to 2021, in the English programme. I started at 24, and it was the route that had a start date.

My view: all three routes in this article end in the same registration. Choose the one with a confirmed first day of lectures.

Summary

Becoming a doctor in South Africa takes nine years after matric: six years of degree, two of internship, one of community service. The bottleneck is the place at one of ten medical schools.

Graduate entry at Wits and a medical degree from abroad lead to the same HPCSA registration.

Marcel Kloos is the founder and owner of Medschool Experts, a placement agency for medicine, dentistry, veterinary medicine and pharmacy degrees abroad. Since 2021 the team has secured more than 500 study places at 25+ partner universities in Europe and worldwide.

Study medicine abroad: Marcel's own path

Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. He is a federally recognised dentist.

What Marcel does at Medschool Experts

Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.

Topics Marcel writes about

  • Study medicine abroad independent of your high school diploma grades: universities, costs and admission
  • Study dentistry, veterinary medicine or pharmacy in Europe in English
  • Tuition fees and living costs at medical schools in Europe and worldwide
  • Recognition of foreign medical degrees in the EU, Switzerland, the UK and the US
  • Transferring between medical schools and returning home into a higher semester
