Failing MBBS in India? What to Do After Four Attempts in the First Professional (2026)

Short answer

Failing MBBS in India? Four attempts used in the first professional or time limit close: what happens now and how to study abroad and return via FMGE or NExT.

At a glance

  • No student gets more than four attempts at the first professional MBBS examination
  • The whole course must be finished within nine years of admission, and a draft of May 2026 would restore ten years including internship
  • No transfer or migration to another Indian medical college is allowed under any circumstances
  • Coming back from abroad: the NMC accepts a foreign degree only if the entire course of at least 54 months and a 12-month internship were done at the same foreign institution
  • For an Indian licence that means a new start abroad, not a credit transfer
  • First step: free consultation with Medschool Experts

You cleared NEET, your family paid the fees, and now the fourth attempt at the first professional is gone. The college speaks of discharge, and another Indian college cannot take you.

Below you find the NMC limits, why a move inside India is closed, and how a medical degree abroad brings you back to India as a registered doctor if it is planned by the rules from the first day.

Last reviewed: October 2026.

Four attempts

In short: Under the Graduate Medical Education Regulations 2023, no student is allowed more than four attempts for the first professional MBBS, and these must fall within four years. A student who does not clear it is not allowed to continue the course under any circumstances.

The attempts include the supplementary examinations. A student who fails the main and the supplementary exam in the first year has used two of four within twelve months.

Time limit

In short: Since June 2023 no student may continue the undergraduate course after nine years from the date of admission. On 18 May 2026 the NMC published a draft that would restore the limit of ten years, including the compulsory rotating medical internship. The internship itself must be finished within two years.

The four-attempt rule is unchanged in the draft. Until the amendment is final, plan with nine years.

No migration

In short: No student once admitted to a medical institution in India is allowed to seek transfer or migration to any other medical institution under any circumstances. A discharged student who wants to study MBBS in India again needs a new NEET rank and a new admission.

Why students get stuck

In short: MBBS students who struggle rarely lack ability, they cleared one of the hardest entrance exams in the world. The usual reasons are the jump from coaching to self-study, anatomy, physiology and biochemistry in one year, language, homesickness, depression or anxiety and family pressure.

None of this means you chose the wrong profession. I started dental medicine in Sofia in 2015, at 24, years after most of my classmates, and graduated in 2021. A slower start says nothing about the doctor you will become.

Returning to India

In short: Yes, you can come back, if your degree abroad meets the Foreign Medical Graduate Licentiate Regulations 2021. The decisive rule: the entire course, training and internship must be done in the same foreign medical institution throughout, and no part of it in India or in another country.

  • Duration: a course of at least 54 months, plus an internship of 12 months at the same institution.
  • Language: the medium of instruction is English.
  • Registration: the degree must make you eligible to practise in the country where you studied.
  • Time: the whole course is completed within ten years of joining.
  • NEET: you need a NEET qualification to study Medicine abroad, and a result stays valid for three years.
  • After graduation: FMGE, to be replaced by NExT, and then the compulsory rotating medical internship of 12 months in India.

For a student who failed in India this has a clear consequence. Credits from your Indian college cannot be carried into a foreign degree if you want an Indian licence. You start in Year 1 at one university abroad and stay there to the end. If you plan to practise outside India, a transfer with credit is a different question, and we assess it separately.

Choosing the university

In short: The NMC no longer keeps a list of approved foreign colleges. What counts is that the university is listed in the World Directory of Medical Schools and that its programme meets each criterion above. Check every point for the exact programme you enrol in.

We work with universities in Europe where many Indian students already study, and we tell you for each one how it meets the 54 months, the English medium, the internship and the local registration.

Plan your way back to India from day one

One wrong choice, such as a change of university in Year 3, costs you the Indian licence. We plan the whole course at one institution with you before you enrol.

Costs

In short: Tuition at our European partner universities starts at about €4,000 a year, and living costs in many Central and Eastern European cities are €5,000 to €8,000 a year. Compare that with the fees and the bond of a private college in India.

How we help

Medschool Experts checks your NEET status and marks in a free consultation, finds medical schools among our more than 25 partner universities that meet the FMGL criteria, prepares the application and the visa file, and plans your route back to India. More than 500 students have been placed so far. The first step is always a free consultation.

The author's view

When I began dental medicine in Sofia in 2015, I was 24, years older than most of my classmates. I graduated in 2021. Indian students who write to us are often offered a quick transfer into Year 2 somewhere. For a licence in India that offer is worthless, because the NMC wants the whole course at one institution. Starting again in Year 1 hurts, but it is the route that ends with a registration number.

Summary

In India four attempts at the first professional and a time limit of nine years end an MBBS, and no other Indian college may take you over. A new NEET rank is the only way back into an Indian college.

A degree abroad leads back to India if the whole course and the internship are done at one foreign institution, in English, within ten years, followed by FMGE or NExT and the internship in India. We plan that with you from the first day.

Conclusion

Check your NEET validity first, and book your free consultation before you pay any fee anywhere.

Failing MBBS in India: 5 questions and answers

Marcel Kloos is the founder and owner of Medschool Experts, a placement agency for medicine, dentistry, veterinary medicine and pharmacy degrees abroad. Since 2021 the team has secured more than 500 study places at 25+ partner universities in Europe and worldwide.

Study medicine abroad: Marcel's own path

Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. He is a federally recognised dentist.

What Marcel does at Medschool Experts

Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.

Topics Marcel writes about

  • Study medicine abroad independent of your high school diploma grades: universities, costs and admission
  • Study dentistry, veterinary medicine or pharmacy in Europe in English
  • Tuition fees and living costs at medical schools in Europe and worldwide
  • Recognition of foreign medical degrees in the EU, Switzerland, the UK and the US
  • Transferring between medical schools and returning home into a higher semester
