Das Studium dauert sechs Jahre. Die vorklinischen Jahre eins und zwei umfassen Anatomie, Physiologie, Biochemie, Histologie und Embryologie sowie Bulgarischkurse. Ab dem dritten Jahr beginnt der klinische Abschnitt mit Innerer Medizin, Chirurgie, Pädiatrie, Gynäkologie, Neurologie und Psychiatrie. Pleven ist für sein Simulationszentrum bekannt, in dem Untersuchungstechniken und Notfallabläufe vor dem Patientenkontakt trainiert werden. Das sechste Jahr besteht aus Stationsrotationen und entspricht dem deutschen PJ.
