BS/MD Programs: Acceptance Rates, Length and the 6-Year Alternative Abroad
10.10.2026
Short answer
BS/MD programs explained: how they work, 7 or 8 years, acceptance rates of 1 to 5%, MCAT rules, and the 6-year MD abroad that also starts right after high school.
What Is a BS/MD Program?
A BS/MD program (also called a BA/MD, combined or direct medical program) admits high school seniors into a college degree and medical school at the same time. Students complete their bachelor's degree, meet the program's conditions and then move into the partner medical school without applying again.
Most BS/MD programs run eight years (4+4). Some compress the path to seven years by shortening the undergraduate phase. A handful of accelerated tracks go even faster, but they are the exception.
The conditions that come with the seat
The medical school seat is conditional, not automatic. Typical requirements during college are a minimum GPA, often 3.5 or higher, a minimum MCAT score, clinical and research experience and good standing. Many students are surprised that the MCAT is still part of the deal: most BS/MD programs now require it, only with a fixed threshold instead of a competitive score. Just a small group of programs waives it entirely.
Most programs are also binding. Students commit to the affiliated medical school and usually cannot apply elsewhere without giving up their reserved seat.
BS/MD Program Acceptance Rates
BS/MD programs are among the most selective admissions in American education. The figures below show how narrow the door is.
| Program | Acceptance rate |
|---|---|
| University of Rochester (REMS) | Under 1% |
| Brown University (PLME) | About 2% (84 admits from 3,827 applicants, class of 2026) |
| Northwestern (HPME) | About 2 to 3% |
| Most elite BS/MD programs | 2 to 5% |
| Less selective programs | Up to about 10 to 15% |
For comparison: in the regular US medical school cycle, 54,699 people applied in 2025 and 23,440 started, roughly 43 percent. A BS/MD seat is many times harder to win than a regular medical school seat four years later.
Pros and Cons of BS/MD Programs
What speaks for a BS/MD program
- Certainty early on. The medical school seat is reserved from day one, as long as the conditions are met.
- Less application stress. No full AMCAS cycle, no secondaries, no medical school interviews in senior year of college.
- Sometimes faster. Seven-year programs save one year of tuition and one year of life.
What speaks against it
- The odds. With acceptance rates of 1 to 5 percent, most excellent applicants won't get in.
- Still eight years in most cases. The majority of programs don't shorten the path at all.
- Full US cost. Students still pay four years of college plus four years of US medical school tuition.
- Locked in at 17. A binding commitment to one medical school limits choice later on.
The Alternative: a 6-Year MD Abroad Straight After High School
Most families looking at BS/MD programs want one thing: to start on medicine right after high school instead of gambling four years later. That exact model is standard in Europe. Medical school there is a single six-year degree that students enter directly after high school, taught fully in English at many universities, with no bachelor's degree in front of it and no MCAT. Read the full overview in our guide to 6-year medical programs for high school students.
| BS/MD program (US) | 6-year MD in Europe | |
|---|---|---|
| Start | After high school | After high school |
| Length to MD | 7 to 8 years | 6 years |
| MCAT | Usually required (minimum score) | Not required |
| Acceptance | Often 1 to 5% | Entrance exam or document review, plannable |
| Tuition | US college plus US medical school | At our partner universities from about 3,500 € a year |
| Route back to the US | US MD, standard Match | USMLE, ECFMG certification, Match |
How the way back to the US works
A European MD leads into US residency through the same exams every international medical graduate takes. The school has to be accredited by an agency recognized by the WFME and listed in the World Directory of Medical Schools with an ECFMG note. Students pass USMLE Step 1 and Step 2 CK, receive ECFMG certification, apply through ERAS and take part in the NRMP Match. Step by step: how IMGs match into US residency.
Where students start
American families most often look at the Czech Republic, Poland and Hungary, with long-standing English-taught programs, and at state faculties in the Balkans such as Foča in Bosnia and Herzegovina or Niš in Serbia, where tuition starts at about 3,500 € a year. Compare the countries on our hub for US students and in medical schools that don't require the MCAT.
And for those who already have a bachelor's degree
If the BS/MD door closed and your child is now finishing college, the Caribbean is often the closer fit: a four-year MD built on the US curriculum, three intakes a year and clinical years in US hospitals. See our comparison of the best Caribbean medical schools.
{{callout}}BS/MD or Medical School Abroad: Which Fits Your Child?
A BS/MD program makes sense for the rare applicant who gets an offer from a program they would genuinely want to attend, ideally one that shortens the path. For everyone else, waiting until senior year of college and hoping for a regular medical school seat is the default, and that is exactly where a six-year MD abroad changes the math: same start right after high school, fewer years, a fraction of the cost and an open road back to US residency.
How We Help
We check your child's documents, recommend the university that fits their goals and budget from our 25+ partner universities, handle the full application and support the family through visa and arrival. The first consultation is free and takes about 30 minutes.
The Author's View
When parents bring up BS/MD programs in consultations, what they are really asking for is certainty. They want their child to know at 17 that medicine is happening. I understand that wish completely. I started dental school in Sofia at 24, after a detour, and I know how much it weighs when the path is unclear.
The problem is that BS/MD programs sell certainty to only a handful of applicants. The rest spend their senior year on applications with odds below 5 percent. In Europe, the same certainty is the normal way in: you finish high school, you apply, you start medicine that autumn. That is the conversation I wish more American families had before they put everything on one very narrow door.
Summary
- There are around 50 BS/MD programs in the US, most with 5 to 20 seats a year.
- Elite BS/MD programs admit 1 to 5 percent of applicants; Rochester's REMS is under 1 percent.
- Most programs take eight years, still require the MCAT with a minimum score and bind students to one medical school.
- A six-year MD in Europe also starts right after high school, needs no MCAT and leads back to US residency via USMLE and ECFMG.
- Tuition at our partner universities starts at about 3,500 € a year.
Conclusion
BS/MD programs are a great outcome for a very small group of students. For the many strong applicants who don't get in, there is a direct route into medicine that starts the same autumn after high school. Book a free consultation and we will show you which partner university fits your child.
Frequently Asked Questions about BS/MD Programs
How many BS/MD programs are there in the US?
There are around 50 BS/MD programs in the United States. The list changes slightly from year to year as programs open, close or change their structure.
What is the acceptance rate for BS/MD programs?
Most elite BS/MD programs admit 2 to 5 percent of applicants. The University of Rochester's REMS program is under 1 percent, while less selective programs reach about 10 to 15 percent.
Do BS/MD programs require the MCAT?
Most do. Students usually need to reach a minimum MCAT score during college to keep their medical school seat. Only a small number of programs waive the MCAT completely.
How long do BS/MD programs take?
Most take eight years (four years of college plus four years of medical school). Some take seven years, and a few accelerated tracks are shorter.
Are BS/MD programs worth it?
For students who get an offer from a program they like, often yes: the seat is reserved early. The downside is the full US price for eight years and a binding commitment made at 17.
Is there an alternative to BS/MD programs?
Yes. In Europe, medicine is a six-year MD that students start straight after high school, without a bachelor's degree and without the MCAT. Graduates of accredited schools can take the USMLE and match into US residency.
Can my child become a doctor in the US after a 6-year MD abroad?
Yes. With an accredited school, USMLE Step 1 and Step 2 CK and ECFMG certification, graduates apply through ERAS and the NRMP Match like every international medical graduate.
Which partner university fits my child?
That depends on grades, budget and goals. Book a free consultation and we check it individually.
