ECFMG Certification: Requirements, Pathways and Timeline for IMGs
No ECFMG certificate, no US residency. It sounds strict, but the process is straightforward once you know the order. Here is what it takes, when to start and where students lose time.
Every doctor who trained outside the US and Canada goes through the Educational Commission for Foreign Medical Graduates. That includes US citizens. ECFMG checks that your medical school counts, that you passed the right exams and that your English is where it needs to be. Once you hold the certificate, you can start residency.
The good part: nothing here is a surprise. If you pick the right school and keep the order in mind, certification is paperwork, not a hurdle.
ECFMG certification requirements: the four things ECFMG checks
1. A medical school that qualifies
Your school has to be listed in the World Directory of Medical Schools with an ECFMG note for US applicants, and it has to be accredited by an agency recognized by the WFME. This is decided before you even enroll, which is why school choice matters so much. Every school we place US students at meets both conditions.
2. USMLE Step 1 and Step 2 CK
Both need to be passed. Step 1 is pass/fail. Step 2 CK comes with a score, and that score is what residency programs care about most. More in USMLE for international medical graduates.
3. English: the OET Medicine
Even native speakers take it. OET Medicine is an English test built around clinical situations, and ECFMG uses it for the communication skills requirement inside the Pathways. For most US students it is a formality. Plan it for your final years so it does not collide with Step 2 CK.
4. Your diploma, verified
After graduation ECFMG contacts your university directly to confirm your degree and transcript. Some universities answer within weeks, others take longer. Request it as soon as your diploma is issued.
ECFMG Pathways 2027: clinical and communication skills
Since Step 2 CS ended, IMGs prove their clinical and communication skills through the ECFMG Pathways. There are six of them. Most students from European schools qualify through a pathway in which their medical school confirms their clinical skills, and Pathway 6 works through six real patient encounters evaluated by licensed physicians. Every pathway also requires a satisfactory score on OET Medicine.
The date to remember: anyone who needs a pathway for the 2027 Match must apply by January 31, 2027. Miss it and you sit out that Match. A certificate earned through a pathway can also expire if you do not start residency in time, so it pays to apply in the year you actually plan to match.
ECFMG certification timeline: the order that saves you a year
- Year 1 to 2: request your USMLE/ECFMG identifier early and get your ECFMG account set up
- End of year 3: Step 1
- Years 5 to 6: Step 2 CK and OET, ideally before you submit ERAS
- Right after graduation: diploma verification
- Before July 1 of your first residency year: certificate issued
Where students lose time: taking Step 2 CK too late, so programs cannot see the score in September. Or waiting months for a university to confirm the diploma. Both are avoidable with a bit of planning.
FAQ: ECFMG certification
Do US citizens need ECFMG certification?
Yes, if they studied medicine outside the US and Canada. Citizenship does not change that. It does mean you need no visa for residency.
Can I start ECFMG steps while still in medical school?
Yes. You register with ECFMG and take Step 1, Step 2 CK and the OET as a student. Only the diploma check waits until you graduate.
How do I know if my school is eligible?
Look it up in the World Directory of Medical Schools and check the ECFMG note. Or ask us. We only work with schools that qualify.
How long does ECFMG certification take?
Most of the time goes into the exams, which you take as a student. After graduation, the diploma verification usually takes a few weeks to a few months, depending on how quickly your university answers. Plan it so the certificate is issued before July 1 of your first residency year.
What are the ECFMG Pathways?
The Pathways are how IMGs meet the clinical and communication skills requirement since Step 2 CS ended. There are six, and all of them require OET Medicine. For the 2027 Match, the application deadline is January 31, 2027.
Does ECFMG certification expire?
A certificate based on an accepted Pathway can expire if you do not enter residency in time. Certificates earned through the old Step 2 CS do not expire.
Which medical schools abroad lead to ECFMG certification?
Schools listed in the World Directory of Medical Schools with an ECFMG note and accredited by a WFME-recognized agency. That includes the Caribbean schools and the European universities we place US students at. See our list of accredited medical schools abroad for US students.
