Same Exams, Same Licence
Caribbean graduates sit exactly the same USMLE as every US medical student. After ECFMG certification they apply through ERAS and the NRMP Match on the same calendar. Once residency and Step 3 are complete, the state licence makes no distinction between a doctor trained in Grenada, Barbados, Antigua or Boston.
Why the Caribbean Is Faster
A US medical school admits once a year, and in 2025 more than half of all applicants did not get a seat. Caribbean medical schools admit three times a year. A student who decides in spring can start in May or September instead of waiting a full cycle. That alone can bring residency one to two years closer.
Clinical Years in US Teaching Hospitals
The clinical years of the Caribbean MD take place in American hospitals, under American attendings. Students rotate through internal medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry and family medicine in cities such as New York, Miami, Chicago and Houston. Rotating through several hospitals also means meeting more programme directors before the residency application.
Costs: Caribbean vs. US Medical School
A private US MD often costs well above 250,000 euros in total. At our partner university, the University of Health Sciences Antigua, regular tuition is roughly 7,300 euros a year before our partner discount. Several Caribbean schools are also approved for US federal student loans.
{{cta}}Residency and Career after a Caribbean MD
Caribbean graduates match every year into US residency programmes, especially in internal medicine, family medicine, paediatrics and psychiatry, the fields with the fastest-growing demand for doctors. SGU in Grenada reports that around 85 to 90 percent of its residency applicants match. From internal medicine, fellowships in cardiology, gastroenterology, nephrology or oncology remain open. More in our overview of Caribbean match rates.
The International View: Caribbean, US and Europe
In the US, the path to an MD runs through a bachelor's degree, the MCAT and a single annual cycle. In Europe, English-taught medicine takes six years directly after high school, at our partner universities from around 3,500 euros a year, and graduates can also sit the USMLE. Canada runs the CaRMS match with a similar four-year MD after an undergraduate degree. The Caribbean is the only route that combines a four-year programme, several intakes a year and US clinical years.
Which Route Fits You?
If your goal is to practise medicine in the United States and you want to start soon, the Caribbean is the most direct route. Caribbean MD: Also Strong in Europe
A Caribbean MD is not limited to the United States. In Germany, graduates from outside the EU work as doctors after a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung), all of which you can prepare during your studies. A transfer into a higher semester at an EU university is possible as well. Details in our guide to the Caribbean medical degree in Europe.
Summary
- Caribbean and US medical schools lead to the same MD and the same US state licence.

Is a Caribbean medical degree recognised in the United States?

No. The state medical licence is identical for Caribbean and US graduates.

Study medicine abroad: Marcel's own path
Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. In Switzerland he completed the federal recognition procedure and holds a Swiss dental licence.
What Marcel does at Medschool Experts
Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.
Topics Marcel writes about
