Caribbean Medical Degree in Europe 2026: Work as a Doctor in Germany and the EU

Short answer

Caribbean medical degree in Europe: B2 German, medical language exam (Fachsprachprüfung) and knowledge test (Kenntnisprüfung) lead to Approbation. Transfer to an EU university also possible.

A Caribbean medical degree opens two continents. The MD is built for the United States, and it also leads into European medicine: in Germany the route runs through three clearly defined steps, a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung). All three are achievable, and you can start the first one during your studies on the island.

If you would rather finish your degree in Europe, a transfer into a higher semester at an EU university is possible as well. This guide explains both routes for graduates and students of a Caribbean medical school. Last reviewed: October 2026.

Work as a Doctor in Europe with a Caribbean Medical Degree

Germany and many other European countries need doctors, and they have a clear procedure for physicians who trained outside the EU. A Caribbean MD counts as a degree from outside the EU, so the recognition runs through that procedure. It is structured, it is the same for everyone, and thousands of doctors from outside the EU complete it every year.

The Route to Approbation in Germany at a Glance

StepWhat it isWhen
1. German B2 certificateGeneral German at B2 level from a recognised language instituteDuring your studies
2. Medical language exam (Fachsprachprüfung)Doctor-patient conversation, case documentation and doctor-to-doctor handover in GermanAfter B2
3. Knowledge test (Kenntnisprüfung)Oral and practical exam in internal medicine, surgery and further core subjects at the level of the German state examAfter your degree
4. ApprobationThe unrestricted licence to practise in GermanyAfter the exams

That is the complete list: one German certificate, one language exam and one knowledge test. No new degree, no repeated studies.

Why This Route Is Very Doable

You Have Four Years to Learn German

The two language steps fit into the time you spend in the Caribbean. A student who starts German in the first semester reaches B2 comfortably alongside the basic sciences and arrives at the clinical years ready for the medical language exam.

Your Medical Knowledge Is the Foundation

The Caribbean MD gives you the full basic sciences and, through the USMLE preparation, a very strong exam routine. The knowledge test builds on exactly that. Specialised preparation courses for the Kenntnisprüfung exist, and many candidates prepare in a few focused months.

Clinical Experience Counts

Two clinical years in teaching hospitals mean you already know the ward routine, the patient conversation and the documentation. That experience carries straight into the medical language exam and the first job.

You Can Start Working Earlier

Many doctors take up a position in a clinic under a temporary professional permit (Berufserlaubnis) while they complete the remaining steps, and receive the full Approbation after the knowledge test.

After the Approbation: Specialist Training in Europe

With the Approbation you start residency training in Germany like any other doctor, in internal medicine, family medicine, surgery, paediatrics, psychiatry or any other field, and you can complete the full specialist training in Europe.

Other EU Countries

Every EU country has its own recognition procedure and language requirements. Germany is the best-structured route for graduates from the Caribbean, and for any other target country we explain the exact steps in the consultation.

Transfer to an EU University in a Higher Semester

You do not have to finish the whole degree in the Caribbean. A transfer into a higher semester at an EU university is possible, and the semesters you have already passed are assessed individually for credit. The more you have completed, the higher the entry point we can look for. Through our 25+ partner universities in Europe, we find the highest possible semester for your case and handle the application. Switching to an EU degree also means a European diploma, which opens the EU labour market directly.

Keep Both Doors Open: United States and Europe

A Caribbean MD lets you decide late. If you prepare the USMLE, you can apply for US residency through ERAS and the NRMP Match. If you learn German alongside, you can work in Germany and the EU. Many students plan both and choose when the results are in. More in our guides to Caribbean match rates and studying medicine in the Caribbean.

How We Plan Your Route with You

We plan with you which Caribbean medical school fits, when to start your German, and whether the United States, Germany or a transfer is your goal. Book a free consultation.

Each step of this route has its own guide. Our Caribbean medical degree recognition in Europe country guide compares the process country by country, and our guide to the knowledge test (Kenntnisprüfung) for Caribbean graduates explains the exam on the way to the Approbation in Germany. How to reach the required language level alongside your studies is covered in learn German during Caribbean medical school, and the British route in Caribbean medical degree in the UK: PLAB and GMC.

Summary

  • A Caribbean MD leads to work as a doctor in Germany and the EU as well as in the United States.

  • The language steps can be completed during your studies in the Caribbean.
  • A transfer into a higher semester at an EU university is possible, with semesters assessed individually.

Study medicine abroad: Marcel's own path

Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He is a federally recognised dentist.

What Marcel does at Medschool Experts

Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. • Study dentistry, veterinary medicine or pharmacy in Europe in English
  • Tuition fees and living costs at medical schools in Europe and worldwide
