Study medicine in the Caribbean: 4-year MD in English, USMLE built in, US clinical rotations and 3 intakes a year. The complete Caribbean medical school guide 2026.
Why Study Medicine in the Caribbean?
Caribbean medical schools follow the North American model of medical education. The MD takes four years: two years of basic sciences on campus and two years of clinical rotations at affiliated teaching hospitals in the United States. Teaching is in English throughout, and the curriculum is written around the USMLE.
Three advantages follow from that structure. Timing: most Caribbean medical schools run three intakes a year, in January, May and September, so a completed file never costs you a year. Curriculum: subjects arrive in the order the USMLE expects them. Clinical phase: you finish your degree already familiar with American charting, rounds and clinical culture.
The Caribbean Medical School MD
Years 1 and 2: Basic Sciences
Anatomy, physiology, biochemistry, pathology, pharmacology, microbiology and behavioural science, taught in semester blocks with clinical skills integrated from early on. Campuses are small and teaching staff are accessible in a way that large lecture-hall faculties are not. Students coming straight from high school start with a premedical or foundation track at several schools, including our partner UHSA.
USMLE Step 1
Step 1 sits at the hinge of the programme, after the basic sciences and before the clinical years. Caribbean medical schools schedule a dedicated preparation block and run internal readiness assessments before an exam date is booked.
Years 3 and 4: Clinical Clerkships in the US
Core rotations in internal medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry and family medicine, followed by electives at affiliated US hospitals. This is also where the letters of recommendation for your residency application come from.
USMLE at Caribbean Medical Schools
| Stage | When | What it does |
|---|---|---|
| USMLE Step 1 | End of year 2 | Pass or fail; opens the clinical years |
| USMLE Step 2 CK | During the clinical years | Scored; the main number on your residency application |
| USMLE Step 3 | First year of residency | Final step towards full licensure |
Step 2 CK carries the weight now that Step 1 is pass or fail. The students with the strongest results work through a question bank steadily from year three onward and agree a target score with their adviser before booking the exam.
US Clinical Rotations
Years three and four take place at teaching hospitals in the United States that hold affiliation agreements with the school. You are on the wards of an American hospital, supervised by American attendings, for the better part of two years. Residency programmes want exactly this US clinical experience, and on the Caribbean route it comes as standard. Rotation cities include New York, Miami, Chicago and Houston. Students who state their preference early have the most say in where they end up.
Accreditation of Caribbean Medical Schools
Three things open the door from a Caribbean medical school to a US residency. The school is accredited by an agency recognised by the World Federation for Medical Education, such as CAAM-HP or the Grenada Medical and Dental Council. It is listed in the World Directory of Medical Schools with the ECFMG Sponsor Note. And every graduate obtains ECFMG certification after passing the USMLE. We place students only at schools that meet all three and re-verify each campus for every intake.
Caribbean Medical School Requirements
Caribbean medical schools admit by individual file review, not by a single cut-off number. The file typically consists of a school-leaving certificate or degree, science coursework, proof of English, a CV, a personal statement and a short online interview. Each school sets its own thresholds, so we check your documents individually. Details in our guide to Caribbean medical school requirements.
{{cta}}Caribbean Medical School Cost
Tuition differs widely between Caribbean medical schools. At our partner university, the University of Health Sciences Antigua, regular tuition is roughly 7,300 euros a year before our partner discount, which makes it one of the most affordable ways to study medicine in the Caribbean. Several schools are also approved for US federal student loans. All figures in our guide to Caribbean medical school cost.
Best Caribbean Medical Schools
The best-known Caribbean medical schools include St. George's University in Grenada, Ross University in Barbados, the American University of the Caribbean in St. Maarten, Saba University and the medical schools in Antigua. Compare them in our ranking of the best Caribbean medical schools 2026 or see every school in the complete list of Caribbean medical schools.
Caribbean, US and Europe Compared
In the United States, an MD takes four years after a four-year bachelor's degree, with one admission cycle a year; in 2025 there were 54,699 applicants for 23,440 seats. In Europe, English-taught medicine takes six years directly after high school, at our partner universities from around 3,500 euros a year. Canada runs a four-year MD after an undergraduate degree and its own CaRMS match. To study medicine in the Caribbean combines the four-year format, several intakes a year and US clinical years, which no other route abroad offers.
Your Timeline

How We Plan Your Route
Marcel Kloos started his own medical education abroad in Sofia in 2015, at 24, without anyone to plan the route with him. Today our team does exactly that for every applicant. We review your certificates, your timeline and your target specialty, shortlist the Caribbean medical schools that fit, handle the complete application and secure the partner discount at UHSA. During your studies you keep the same adviser, especially around booking Step 1 and choosing rotation sites.
Work in Europe with Your Caribbean MD
The Caribbean MD does not tie you to one continent. In Germany, graduates from outside the EU work as doctors after three defined steps: a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung). All three are achievable and can be prepared during your studies. A transfer into a higher semester at an EU university is possible as well. Details in our guide to the Caribbean medical degree in Europe.
Summary
- To study medicine in the Caribbean means a four-year MD in English: two years of basic sciences on the island, two years of clinical rotations in US teaching hospitals.

- Our partner, the University of Health Sciences Antigua, offers a tuition discount, one clinical year in the USA and a four-year graduate-entry track.

Study medicine abroad: Marcel's own path
Born in Stuttgart, Marcel started his degree late: at 24 he enrolled at the Medical University of Sofia, Faculty of Dental Medicine, and graduated in 2021 from the English-language programme. He went through the same steps his clients face today — nostrification of school certificates, entrance interviews, finding a flat in Sofia and later the recognition of a foreign degree. In Switzerland he completed the federal recognition procedure and holds a Swiss dental licence.
What Marcel does at Medschool Experts
Today Marcel lives in Bern and runs Medschool Experts full-time. He visits every partner university in person, negotiates placement agreements directly with faculties and deans, and leads the free consultations in which applicants learn at which university they can start next semester. His YouTube channel on studying medicine abroad has helped thousands of applicants compare universities, costs and recognition.
