Caribbean Medical Degree Recognized in Europe: 2026 Country-by-Country Guide
14.10.2026

Short answer
Is a Caribbean medical degree recognized in Europe? Yes. Licensing routes in 8 countries for 2026, from the German Approbation to Ireland, Malta and Switzerland.
Is a Caribbean Medical Degree Recognized in Europe in 2026?
A Caribbean medical degree is recognized in Europe in 2026 through national licensing procedures for doctors with a degree from outside the EU. Each European country checks the diploma, asks for its official language and one professional exam, and then grants the full licence. Thousands of doctors from outside the EU complete these procedures every year.
The starting point is the same everywhere. Your medical school is listed in the World Directory of Medical Schools (WDOMS), your diploma and transcripts are certified, and the national authority reviews your file. Norway, for example, verifies medical diplomas from outside Europe through ECFMG, the same organisation that certifies Caribbean graduates for the United States. Many Caribbean students already hold that certification by graduation day.
What changes from country to country is the language, the exam format and the time frame. Germany is the clearest and largest market. Ireland and Malta work in English. Scandinavia pays well and asks for a Nordic language. Switzerland rewards clinical experience inside the country.
Caribbean MD in Europe: All Country Routes Compared
A Caribbean MD in Europe follows one pattern in all eight countries below: language proof, one professional exam or assessment, then registration. Germany, Austria, the Netherlands, Sweden, Norway and Switzerland ask for their national language. Ireland and Malta work in English, the language of every Caribbean medical school.
| Country | Language | Professional exam or assessment | Licence or register | 2026 facts |
|---|---|---|---|---|
| Germany | German B2 plus medical language exam at C1 level | Knowledge test (Kenntnisprüfung), oral and practical | Approbation | Lower Saxony: 595 € language exam, 980 € knowledge test |
| Austria | German B2 | Nostrification at a medical university: written test in ten subjects, then the assigned exams | Austrian Medical Chamber | Medical University of Graz: 150 € application fee |
| Netherlands | Dutch B2+ plus an English reading test | Professional test (BI test), assessment by a commission | BIG register | Application free of charge, 6 to 18 months processing |
| Sweden | Swedish, Danish or Norwegian at C1 | Knowledge test where required, supervised practice, course in Swedish health law | Swedish licence (legitimation) | Steps set individually per applicant |
| Norway | Norwegian B2 in every part | Two-day professional exam at the University of Oslo, course in national subjects and medication handling | Norwegian authorisation | Exam held each spring and autumn |
| Ireland | English test | PRES 2 (written) and PRES 3 (clinical), or exemption through a recognised higher qualification | General Division | 2026 plan: PLAB 1 and PLAB 2 accepted in place of PRES |
| Switzerland | German, French or Italian at B2 | Diploma registration, then the federal exam after 36 months of clinical work in Switzerland | MedReg | CHF 800 to 1,200 for diploma registration |
| Malta | English: IELTS Academic 7.5 (6.5 per part) or OET grade B | Medical Council Malta Statutory Examination | Medical Council Malta register | Exam in English |
Read the table by the column that matters most to you. If you want the biggest job market, it is Germany. If you want to stay in English, it is Ireland or Malta. If you already speak a European language at home, that country is usually your shortest path.
Caribbean Medical Degree in Germany: B2, Fachsprachprüfung, Kenntnisprüfung
A Caribbean medical degree leads to the German Approbation in four steps: a German B2 certificate, the medical language exam (Fachsprachprüfung) at C1 level, the knowledge test (Kenntnisprüfung) and the Approbation itself. The first two steps fit into the years of medical school in the Caribbean, so many graduates arrive in Germany with half the route already done.
The medical language exam is a one-hour individual exam in three parts of 20 minutes each: a simulated conversation with a patient, a written medical report and a handover conversation with a doctor. The knowledge test is oral and practical. It starts with a patient presentation with a simulated patient and continues with an exam conversation in groups of up to four candidates. Internal medicine and surgery form the core, with emergency medicine, clinical pharmacology, imaging, radiation protection and medical law as further subjects.
| Germany, step | Format | Fee in Lower Saxony, 2026 |
|---|---|---|
| German B2 certificate | General German from a recognised language institute | Set by the language institute |
| Medical language exam (Fachsprachprüfung) | 60 minutes, three parts, C1 medical German | 595 € |
| Knowledge test (Kenntnisprüfung) | Patient presentation of about 60 minutes plus exam conversation of 60 to 90 minutes | 980 € |
| Approbation | Licence issued by the state authority | Set by each federal state |
Each German federal state runs its own recognition office, so fees and waiting times differ slightly between Hamburg, Bavaria or Lower Saxony. The knowledge test can be repeated twice. Many doctors start working in a German hospital under a temporary licence (Berufserlaubnis) while they prepare for it. The full German route, with preparation tips and timing, is in our guide Caribbean medical degree in Germany and the EU and in the dedicated article on the knowledge test for Caribbean graduates.
Caribbean Medical Degree in Austria: Nostrification
A Caribbean medical degree is recognized in Austria through nostrification at a public medical university such as Vienna, Graz or Innsbruck. Applicants take a joint written test in ten clinical subjects, and the result decides which individual exams follow. Afterwards the doctor registers with the Austrian Medical Chamber. German at B2 level is required for the test.
The Austrian system is generous in one detail. The written test cannot be failed in the classic sense. A subject counts as passed at 60 percent of correct answers, and every passed subject is one exam fewer. Candidates who pass six or more of the ten subjects take only the remaining ones. Pharmacology and prescribing plus forensic medicine are always part of the programme. During the procedure you are enrolled as an extraordinary student, with full access to lectures and exams. The Medical University of Graz charges an application fee of 150 euros (2026).
Can I Practice in the Netherlands with a Caribbean Medical Degree?
Yes, a doctor with a Caribbean medical degree can practise in the Netherlands after registration in the BIG register. The route runs through a certificate of professional competence: diploma assessment, the professional test (BI test), Dutch at B2+ level, an English reading test and a final assessment by a commission. Processing takes 6 to 18 months in 2026.
The Dutch link to the Caribbean is old and close. Sint Maarten, Curaçao and Aruba belong to the Kingdom of the Netherlands, and Dutch is spoken on the streets of Philipsburg. For students at a school on Dutch Sint Maarten, such as American University of the Caribbean, learning Dutch can start right outside the lecture hall. One more detail: the BIG application itself is free of charge in September 2026. You only pay for language courses, tests and document translations.
Caribbean MD in Sweden and Norway: Licensing in Scandinavia
A Caribbean MD leads to a medical licence in Sweden and Norway once the doctor learns a Scandinavian language and completes a set of national steps. Sweden asks for C1 in Swedish, Danish or Norwegian, a knowledge test where required, supervised practice and a course in Swedish health law. Norway asks for Norwegian B2 and a two-day professional exam.
Sweden
Sweden decides each file individually. The national health authority (Socialstyrelsen) compares your degree with Swedish medical training and then sets your programme: a knowledge test, a period of supervised practice in Swedish healthcare and a course in Swedish laws and regulations. Language proof at C1 level can come from a test, from Swedish 3 at upper-secondary level or from a certificate issued by your employer. A Norwegian or Danish C1 certificate counts as well, which keeps your options open across the whole region.
Norway
Norway publishes a fixed list. Norwegian at B2 level in every part of the test, a two-day practical, oral and written professional exam at the University of Oslo (held each spring and autumn), a course in national subjects and a course in medication handling. All additional requirements are completed within three years of the diploma assessment. Norway verifies non-European medical diplomas through ECFMG, a step many Caribbean graduates have already completed for the United States.
Can I Practice in Ireland with a Caribbean Medical Degree?
Yes, a Caribbean medical degree leads to registration in the General Division of the Medical Council of Ireland. Doctors who qualified outside the EU pass the pre-registration examination PRES, with a written part (PRES 2) and a clinical part (PRES 3), or qualify through a recognised higher qualification. An English language test is part of every application.
Ireland is moving closer to the UK route in 2026. The Medical Council has proposed that PLAB 1 counts in place of PRES 2 and PLAB 2 in place of PRES 3, and it ran a public consultation on the change until June 2026. For Caribbean graduates this means one exam set could serve two countries. Since 2020, doctors from outside the EEA with General Division registration can also apply for Irish specialist training without an Irish-style internship. The UK route itself is covered in our guide to the Caribbean medical degree in the UK with PLAB and GMC.
Caribbean Medical Degree in Switzerland: MedReg and Federal Exam
A Caribbean medical degree is registered in Switzerland in the medical professions register MedReg, which allows work under supervision as the canton approves. After 36 months of full-time clinical work in Switzerland, the doctor gains direct admission to the Swiss federal exam. After 60 months, only the theoretical part remains. Language proof at B2 is required.
Switzerland pays for experience, quite literally. The longer you work in a Swiss hospital, the shorter your exam. Registration of a diploma from outside the EU and EFTA costs CHF 800 to 1,200 in 2026 (roughly 850 to 1,300 euros), and a language entry in German, French or Italian costs CHF 50 to 100. For independent practice, the cantons also expect a recognised specialist title, which you earn through Swiss further training. Our founder Marcel Kloos is a federally recognised dentist who lives in Bern, so the Swiss procedure is one our team knows from the inside.
Caribbean Medical Degree in Malta: The English Route into the EU
A Caribbean medical degree leads to registration in Malta through the Medical Council Malta Statutory Examination, a professional and linguistic exam held in English. Before sitting it, the doctor shows IELTS Academic 7.5 overall with at least 6.5 in each part, or OET Medicine with grade B in every part. Malta is a full EU member state.
Malta is the shortest language path in the whole EU for a Caribbean graduate. You already study, write and examine in English for four years. The island is also a familiar format: small, warm, English-speaking, with a single national hospital system. Several of our students know the feeling from Antigua or Grenada already.
Which Language Do You Need for Each European Country?
The language decides the timeline more than any exam. Germany, Austria and Switzerland ask for German (Switzerland also French or Italian), the Netherlands for Dutch, Sweden and Norway for a Scandinavian language. Ireland and Malta work in English. Most Caribbean students reach B2 in two to three years of steady study alongside medical school.
Start early. A student who opens a German course in the first semester of basic sciences usually holds the B2 certificate before the clinical years begin. Our plan for exactly that sits in the guide learn German during Caribbean medical school.
{{cta}}Does a Licence in One European Country Open the Others?
Yes, partly. Under Article 3(3) of the EU Directive 2005/36/EC, a degree from outside the EU that one member state has recognised, followed by three years of professional experience in that state, is treated as an EU qualification in the other member states. The next country then assesses your file in the simpler EU procedure.
Few people plan for this, and it changes the strategy. A doctor who earns the German Approbation and works three years in a German hospital later moves to Austria, the Netherlands or Ireland on far easier terms. Switzerland follows a similar logic. A diploma recognised by an EU or EFTA state, combined with three years of recent clinical experience there and EU or Swiss citizenship of the doctor or the spouse, opens indirect recognition. Norway exempts doctors who have worked three years with full rights in another EEA country from its additional requirements. One licence, earned well, becomes a European passport for your career.
US Route vs Europe Route for Caribbean Medical Graduates
The US route for Caribbean medical graduates runs through USMLE Step 1 and Step 2 CK, ECFMG certification, ERAS and the NRMP Match. The Europe route runs through a national language, one professional exam and direct applications to hospitals. Both routes are open with the same Caribbean MD, and many students prepare both in parallel.
| Topic | US route | Europe route (Germany as example) |
|---|---|---|
| Exams | USMLE Step 1 and Step 2 CK, Step 3 during residency | Medical language exam and knowledge test |
| Language | English | German B2, medical German at C1 |
| Certification | ECFMG certification | Approbation from the state authority |
| Finding a position | ERAS application and NRMP Match, once a year | Direct application to hospitals, all year round |
| Specialist training | Residency, 3 to 7 years | Specialist training (Weiterbildung), typically 5 to 6 years |
| Licence | State medical licence | Approbation, valid in all of Germany |
The two routes share more than it seems. The clinical reasoning drilled for Step 2 CK is the same reasoning the German knowledge test asks for in internal medicine and surgery. A student who prepares both loses little time and gains a real choice at graduation. Our guide USA or Europe: the dual pathway for Caribbean MDs shows how to schedule both, and USMLE and ECFMG certification 2026 covers the American side step by step.
Transfer to an EU University Instead of Licensing a Caribbean MD
A transfer into a higher semester at a European university is the alternative to licensing a Caribbean MD after graduation. The student moves during the studies and graduates with an EU degree, which is recognised automatically in every EU country. Medschool Experts arranges these transfers through our 25+ partner universities in Europe.
Medschool Experts is the world's leading expert for this kind of transfer and frequently achieves entry into year 4 or year 5 at top universities. The exact entry point depends on your individual file: the courses you passed, the hours, the clinical rotations. We check the file in a free consultation and tell you where you stand. More on timing in switching from the Caribbean to Europe without losing time.
Planning the Route from the First Semester: UHSA Antigua and Medschool Experts
The European route is easiest when it is planned before the first semester in the Caribbean. School choice, German or Dutch lessons, USMLE preparation and the later licensing file all build on each other. Medschool Experts plans these steps with every student, from the application to the European licence or the transfer.
Our official partner in the Caribbean is the University of Health Sciences Antigua (UHSA) in Piccadilly, Antigua. Regular tuition is roughly 7,300 euros a year (8,500 USD) before our partner discount, and students who apply through Medschool Experts receive a substantial reduction. One clinical year can be completed in the United States. Applicants with a prior degree or a completed year of biology, chemistry and physics can join the four-year graduate-entry track. That combination of moderate cost, US clinical exposure and an English-language MD makes UHSA a strong base for both continents.
Marcel Kloos, our founder, started his own medical studies abroad in Sofia in 2015 at the age of 24, a late starter himself, and today his team has placed 500+ students. Recognition procedures are therefore not theory for us. They are the files we handle every week.
The author's view
I work on visa and licensing files every week, and the pattern I see is always the same. The exam is rarely what slows a doctor down. The paperwork and the language are. When I worked with students from Dubai and India, the ones who reached their European licence quickly had started their language course years before graduation and kept their documents certified and translated as they went.
From Belgrade, where I am based today, I look at European recognition as a map of open doors rather than walls. Germany has the clearest procedure. Ireland and Malta are a natural fit for anyone who thinks in English. Switzerland rewards patience in a Swiss hospital. Each of these routes works with a Caribbean MD.
My advice is simple. Pick your European target country in year one, not in the final year. Then the Caribbean MD becomes what it should be: a degree that opens two continents.
Your Caribbean MD Also Leads to Europe
A Caribbean MD is built for the United States, and it also leads into European medicine. Germany asks for a German B2 certificate, the medical language exam and the knowledge test before the Approbation. All of these steps are achievable, and the language steps can be completed during your studies on the island. A transfer into a higher semester at an EU university is possible too. Our guide to the Caribbean medical degree in Germany and the EU goes deeper into the German route.
Summary
- A Caribbean medical degree is recognized in Europe through national licensing procedures in every country.

Conclusion
The question is not whether a Caribbean MD works in Europe. It does, in Germany, Austria, the Netherlands, Scandinavia, Ireland, Switzerland and Malta. The real question is which country fits your language, your family and your career plan, and when to start preparing. Students who begin their language in year one arrive at graduation with half the route done.
We plan that route with you: the right Caribbean school, the UHSA partner discount, the language plan, the US exams, the European licence or the transfer to Europe. Book a free consultation and get a clear plan for your Caribbean MD in Europe.
FAQ: Caribbean Medical Degree Recognition in Europe
Is a Caribbean medical degree recognized in Europe?
Yes. Every European country has a licensing procedure for doctors who graduated outside the EU, and a Caribbean MD from a school listed in the World Directory of Medical Schools enters that procedure. The doctor proves the national language, passes one professional exam or assessment and then registers. In Germany this leads to the Approbation, the full licence to practise.
Can I practice in Germany with a Caribbean medical degree?
Yes. A doctor with a Caribbean MD needs a German B2 certificate, the medical language exam (Fachsprachprüfung) at C1 level and the knowledge test (Kenntnisprüfung). After passing, the state authority grants the Approbation. In Lower Saxony the language exam costs 595 euros and the knowledge test 980 euros in 2026. Many doctors work under a temporary licence while they prepare.
Which European country is fastest for a Caribbean MD?
Ireland and Malta are the fastest in language terms, because both work in English, the teaching language of Caribbean medical schools. Germany is the largest job market and very structured. A student who learns German during medical school often reaches the Approbation as quickly as an English-language route, because the language steps are already done at graduation.
Does ECFMG certification help with a European licence?
Yes. Each European country sets its own diploma check, and Norway verifies medical diplomas from outside Europe through ECFMG. Many Caribbean graduates complete ECFMG certification anyway for the United States, so their documents are already verified when the European file starts. Holding ECFMG certification keeps the US option open while the European licence is prepared in parallel.
Can a Caribbean medical graduate work in Switzerland?
Yes. The diploma is registered in MedReg, the Swiss register of medical professions, and the canton approves work under supervision. After 36 months of full-time clinical work in Switzerland the doctor is admitted directly to the federal exam. Registration costs CHF 800 to 1,200 in 2026. Language proof at B2 level in German, French or Italian is required.
Does a German Approbation help me move to another EU country later?
Yes. Under Article 3(3) of the EU Directive 2005/36/EC, a degree from outside the EU that one member state has recognised, plus three years of professional experience in that state, counts as an EU qualification in other member states. A doctor with three years in Germany therefore moves to Austria, the Netherlands or Ireland in a much simpler procedure.
Can I transfer from a Caribbean medical school to a European university?
Yes. A transfer into a higher semester at a European university leads to an EU degree that is recognised automatically across the EU. Medschool Experts frequently achieves entry into year 4 or year 5 at top universities through our 25+ partner universities. The entry point depends on your individual file, which we check in a free consultation.
