Kenntnisprüfung for Caribbean Graduates: The German Knowledge Test 2026

Short answer

Kenntnisprüfung for Caribbean graduates 2026: subjects, oral-practical format, 980 € fee, Berufserlaubnis and the 5 steps from Caribbean MD to Approbation.

The Kenntnisprüfung for Caribbean graduates is the German state knowledge test that turns a Caribbean MD into a full German medical licence, the Approbation. It is a one-day oral-practical exam centred on internal medicine and surgery, with emergency medicine, clinical pharmacology, imaging, radiation protection and medical law also in scope. From 1 November 2026, the direct knowledge test becomes the standard route for doctors whose degree comes from outside the EU.

For a graduate of a Caribbean medical school, that is good news. You already learned medicine in English with US textbooks, you sat USMLE-style question banks for years, and you presented patients on clinical rotations in US hospitals. The knowledge test asks for exactly that clinical reasoning, only in German and in front of a German examination board.

The path has five clear steps: a general German B2 certificate, the medical language exam (Fachsprachprüfung), the knowledge test, the Approbation and finally residency training, called Weiterbildung in Germany. Each step is achievable, and the German language steps can start while you are still studying in the Caribbean.

Last reviewed: October 2026.

What is the Kenntnisprüfung for Caribbean medical graduates?

The Kenntnisprüfung for Caribbean medical graduates is the German knowledge test that confirms a doctor trained outside the EU has the medical knowledge of a German graduate. Passing it in 2026 leads directly to the Approbation, the full licence to practise medicine anywhere in Germany without supervision and without a time limit.

The test does not check your diploma line by line. It checks you. A board of examiners watches you take a history, examine a patient and explain your plan, then asks questions around the case. That is why many Caribbean graduates find the format fairer than they expected.

The legal basis sits in the German Medical Licensure Act and the licensing regulation for doctors. The exam is run by the federal states, so the organisation differs slightly between Bavaria, Berlin or Lower Saxony. The content is the same everywhere.

Who has to take the knowledge test in Germany as a foreign doctor in 2026?

Every doctor with a medical degree from a country outside the EU, the EEA and Switzerland takes the knowledge test in Germany to receive the Approbation. A Caribbean MD from Antigua, Grenada, Barbados, St. Maarten or any other island falls into this group, regardless of nationality. German and EU citizens with a Caribbean MD take the same test.

A new federal law makes this even clearer. The Bundestag passed it on 26 March 2026, the Bundesrat approved it, and it applies from 1 November 2026. From that date the direct knowledge test is the standard route for doctors with a degree from outside Europe. The federal states may also check German skills first, before they look at the medical qualification.

For you, this means one predictable exam instead of months of paperwork. Good timing.

The full sequence: from Caribbean MD to Approbation and residency in Germany

The full sequence to work as a doctor in Germany with a Caribbean degree has five steps in 2026: a German B2 certificate, the Fachsprachprüfung at C1 medical level, the Kenntnisprüfung, the Approbation and residency training (Weiterbildung). The first two steps can be completed during your Caribbean MD, the rest follows graduation.

1 German B2 certificateGeneral German, possible during your MD 2 Fachsprachprüfung (C1 medical)60 minutes, three parts of 20 minutes 3 Kenntnisprüfung (knowledge test)Oral-practical, patient case, one day 4 ApprobationFull, unlimited German medical licence 5 Weiterbildung (residency)Paid specialist training, usually 5 to 6 years
The German route for Caribbean MD graduates in 2026, step 3 is the knowledge test.

Step one is general German at B2 level, certified by a recognised language institute. Step two is the Fachsprachprüfung, organised by the state medical association. It has three parts of 20 minutes each: a simulated doctor-patient conversation, a written medical report and a doctor-to-doctor conversation. No medical knowledge is graded here. Only communication.

Step three is the knowledge test. Step four, the Approbation, is issued by the same state authority once you pass. Step five is your residency. In Germany you apply directly to hospitals, there is no central match, and contracts start throughout the year.

The detailed recognition steps for the whole of Europe are covered in our guide on the Caribbean medical degree in Europe and in the article on Caribbean medical degree recognition in Europe.

What does the Kenntnisprüfung cover? Subjects and content

The Kenntnisprüfung covers internal medicine and surgery as core subjects in 2026. Examiners can also ask about emergency medicine, clinical pharmacology and pharmacotherapy, imaging, radiation protection and the legal aspects of practising medicine in Germany. Questions usually grow out of the patient case, so clinical reasoning matters more than memorised lists.

AreaWhat the examiners look forWhere Caribbean graduates already have practice
Internal medicine (core subject)Cardiology, pulmonology, gastroenterology, nephrology, endocrinology, infectious disease, differential diagnosis and treatment plansUSMLE Step 2 CK, internal medicine core rotation
Surgery (core subject)Acute abdomen, trauma, fractures, perioperative care, indications for surgery, wound managementSurgery core rotation, surgical shelf exam
Emergency medicineABCDE approach, resuscitation, shock, chest pain, stroke pathwaysACLS and BLS training, emergency electives
Clinical pharmacology and pharmacotherapyDrug choice, dosing, interactions, contraindications, German drug namesPharmacology in USMLE Step 1 and Step 2 CK
ImagingChest X-ray, CT and ultrasound basics, choosing the right studyRadiology teaching on rotations
Radiation protectionIndications, dose awareness, protecting patients and staffNew for most graduates, short and learnable
Legal aspects of practising medicineConsent, confidentiality, documentation, death certificate, German professional lawNew, usually covered in prep courses

The table shows something useful. Five of the seven areas map directly onto content that every Caribbean MD student already studies for USMLE Step 1 and Step 2 CK. Radiation protection and German medical law are the genuinely new parts. Both are compact. A few focused weeks cover them.

Some states let the board choose a further subject next to the core subjects, for example neurology or paediatrics. Your invitation letter names the examiners and the subjects in advance.

Kenntnisprüfung format: how the exam day works

The Kenntnisprüfung is an oral-practical exam held on one day in German. In 2026 it typically starts with about 60 minutes for a patient case, followed by an oral examination of 60 to 90 minutes per candidate, with up to four candidates examined together. The whole exam is repeated if it is not passed, and two retakes are allowed.

One exam day, two parts Part 1: Patient case about 60 minutes Part 2: Oral examination 60 to 90 minutes per candidate History and physical exam, supervised by the board, then a written report withdiagnosis, plan and Epikrise Case presentation, then questionson internal medicine and surgery, emergencies, drugs, imaging, radiation protection and law Up to four candidates sit together in the oral part.
Typical structure of the German knowledge test for doctors in 2026.

In the first part you take the history and examine your patient under supervision. Depending on the state, this is a hospital patient or a trained simulated patient. You then write a short report: history, findings, working diagnosis, differential diagnoses, prognosis, treatment plan and a summary in discharge-letter style, called the Epikrise.

In the second part you present the case to the board. The questions start there and move outward. A patient with chest pain may lead to ECG interpretation, the acute coronary syndrome pathway, anticoagulation dosing and the consent conversation before a catheter procedure. Calm structure wins points.

How do Caribbean graduates apply to the Approbationsbehörde?

Caribbean graduates apply for the Approbation, and with it for the knowledge test, at the competent state authority (Approbationsbehörde) of the federal state where they plan to work. In Lower Saxony this is the central licensing office NiZzA, in Berlin the State Office for Health and Social Affairs. A job offer is not required to apply.

The authority checks your degree documents and identity, and each state publishes its own list of what it wants to see. Our team works through that list with you file by file. Once the file is complete, the authority invites you to the Fachsprachprüfung and then to the knowledge test.

One practical point: choose the state carefully. Waiting times for exam dates differ between states, and you generally stay with one authority for the whole procedure. Medschool Experts compares current dates across the states before you submit.

Berufserlaubnis: working as a doctor in Germany with a Caribbean degree while you prepare

A Berufserlaubnis is a temporary German permit to practise medicine that the state authority can issue to Caribbean graduates while they prepare for the knowledge test. In 2026 it can be granted or extended for a total of up to two years and may be limited to specific activities and a specific hospital.

Many graduates use this phase well. You work as an assistant doctor, earn a regular salary, take part in ward rounds in German and write real discharge letters. That is the best possible preparation for the patient case in the Kenntnisprüfung. The Fachsprachprüfung usually comes first, because the permit requires proven medical German.

Time spent on a Berufserlaubnis does not block the Approbation later. It simply bridges the gap until your exam date.

How much does the knowledge test cost in Germany in 2026?

The knowledge test costs 980 euros in Lower Saxony in 2026, paid in advance. The Fachsprachprüfung costs 595 euros in Lower Saxony and 470 euros in Rhineland-Palatinate. Fees differ by federal state. Translations, the B2 exam and the final Approbation certificate come on top.

Compared with USMLE fees and US residency application costs, the German route is inexpensive. Most of the budget goes into preparation: a German course, a medical German course and, for many graduates, a structured Kenntnisprüfung prep course. With a Berufserlaubnis, the salary usually covers these costs while you prepare.

Why USMLE routine helps in the Kenntnisprüfung

USMLE routine helps Caribbean graduates in the Kenntnisprüfung because both exams test the same clinical reasoning: building a differential diagnosis, choosing the next best step and justifying treatment. In 2026 the German exam simply asks for this reasoning orally and in German, starting from a real patient instead of a written vignette.

Think of a Step 2 CK question block. Every vignette forces a decision. History, findings, diagnosis, management. The German board asks the same questions in the same order, only face to face. Graduates who trained with US question banks recognise the logic within minutes.

Clinical rotations in US hospitals add a second advantage. Caribbean students present patients to attendings every day during their third and fourth years. Presenting a case to a German examination board is the same skill. Only the language changes, and the language steps are already done by then.

Marcel Kloos, founder of Medschool Experts and a federally recognised dentist, started his own medical studies abroad in Sofia in 2015 at the age of 24. He completed an English-language degree there in 2021 and today lives and works in German-speaking Bern. Switching the exam language is a hurdle. It is a manageable one.

A realistic preparation timeline for the knowledge test

A realistic preparation timeline for the Kenntnisprüfung is 12 to 24 months from the start of German lessons to the exam date for Caribbean graduates in 2026. Students who reach B2 during their MD can often sit the knowledge test within a year of graduation. Starting German early is the single biggest time saver.

A typical plan looks like this. During basic sciences in the Caribbean, two to three German lessons a week lead to B1. During clinical rotations, an online course brings you to B2 and the certificate. After graduation, a medical German course prepares the Fachsprachprüfung. Then come the application, the Berufserlaubnis and three to six months of focused Kenntnisprüfung preparation alongside clinical work.

Our guide on how to learn German during Caribbean medical school breaks the language part down semester by semester.

USMLE or Kenntnisprüfung: do Caribbean graduates have to choose?

Caribbean graduates do not have to choose between USMLE and the Kenntnisprüfung. In 2026 the two routes run in parallel: USMLE and ECFMG certification keep the US Match open, while German B2 and the Fachsprachprüfung prepare the German Approbation. The clinical knowledge behind both exams overlaps almost completely.

StepUS routeGerman route
Basic science examUSMLE Step 1Not required
Clinical knowledge examUSMLE Step 2 CKKenntnisprüfung (oral-practical)
Language requirementEnglish, already metGerman B2 plus Fachsprachprüfung (C1 medical)
Certification or licenceECFMG certification, then state licence via the FSMBApprobation from the state authority
Residency applicationERAS and the NRMP MatchDirect application to hospitals, all year round
Interim work permitResidency contractBerufserlaubnis for up to two years

This is the question few graduates ask early enough. A dual plan costs little extra effort in the first years, because the clinical content is shared. The German language is the only additional piece. Graduates who keep both doors open can decide after Step 2 CK, with real options on the table. More on this in our article on the dual pathway to the USA or Europe.

After the Approbation: residency (Weiterbildung) in Germany

After the Approbation, Caribbean graduates start residency training in Germany, called Weiterbildung, as salaried assistant doctors at hospitals of their choice. In 2026 training in internal medicine or general practice takes five years, general surgery six years. Applications go directly to hospitals, without a central match or fixed start dates.

German hospitals hire throughout the year, and many departments actively recruit doctors who already passed the knowledge test. Your Approbation is also valid for the whole of Germany, so you can move between federal states without a new procedure.

The specialist title you earn at the end is a German qualification. It counts across Europe. A Caribbean MD followed by German specialist training is a strong combination for a long career.

The author's view

I work on visa and licensing cases every week from my desk in Belgrade, and in 2026 I have seen the German procedure change in a way that favours Caribbean graduates. The direct knowledge test replaces a long paper review with one clear exam. For doctors trained on US-style clinical cases, that is the format I would choose myself.

In my own licensing work with students from Dubai and India, the same pattern shows up again and again: the medicine is rarely the hard part, the timing of the language steps is. Those who started German in their first or second year moved faster into the Fachsprachprüfung and a Berufserlaubnis than those who started after graduation.

My advice is simple. Decide early that Germany stays on your list, book your first German lessons this semester and let us plan the federal state and the documents with you. The knowledge test then becomes one more clinical exam in a career full of them.

Your Caribbean MD Also Leads to Europe

A Caribbean MD opens two continents. The US route runs through USMLE, ECFMG certification, ERAS and the NRMP Match, with the same state licence as US graduates. The European route for Germany runs through German B2, the Fachsprachprüfung and the knowledge test to the Approbation, and all three steps are clearly achievable. Students who prefer to finish their degree in Europe can also transfer into a higher semester at an EU university. Medschool Experts is the world's leading expert for this and frequently achieves entry into year 4 or year 5 at top universities, the outcome depends on the individual file.

Our official partner in the Caribbean is the University of Health Sciences Antigua (UHSA) in Piccadilly, Antigua. Regular tuition is roughly 7,300 euros a year before our partner discount (8,500 USD), one clinical year in the USA is possible, and a four-year graduate-entry track is open to applicants with a prior degree or a completed year of biology, chemistry or physics.

Summary

  • The Kenntnisprüfung is the German knowledge test that leads Caribbean MD graduates to the full Approbation.
  • From 1 November 2026, the direct knowledge test is the standard route for doctors with a degree from outside Europe.
  • Core subjects are internal medicine and surgery, plus emergency medicine, pharmacology, imaging, radiation protection and medical law.
  • The exam day combines about 60 minutes for a patient case with a 60 to 90 minute oral examination.
  • Fees in 2026: 980 euros for the knowledge test and 595 euros for the Fachsprachprüfung in Lower Saxony.
  • A Berufserlaubnis allows supervised work for up to two years while you prepare.
  • USMLE routine transfers directly, and both routes can run in parallel.

Conclusion

The German knowledge test is a clinical exam for clinicians. Caribbean graduates arrive with years of US-style case training, and that makes the Kenntnisprüfung a very reachable goal. The real work is the German language, and that work can start in the first semester on the island.

FAQ: Kenntnisprüfung for Caribbean graduates

Can I work as a doctor in Germany with a Caribbean degree?

Yes. A Caribbean MD leads to the full German licence, the Approbation, once you hold a German B2 certificate, pass the Fachsprachprüfung at C1 medical level and pass the Kenntnisprüfung. While preparing for the knowledge test, the state authority can grant a temporary Berufserlaubnis of up to two years, so you can already work as an assistant doctor in a German hospital in 2026.

What subjects are in the German knowledge test for foreign doctors?

The German knowledge test for foreign doctors focuses on internal medicine and surgery in 2026. Examiners may also ask about emergency medicine, clinical pharmacology and pharmacotherapy, imaging, radiation protection and the legal aspects of practising medicine in Germany. Most questions develop from the patient case that each candidate examines at the start of the exam day.

How much does the Kenntnisprüfung cost in 2026?

The Kenntnisprüfung costs 980 euros in Lower Saxony in 2026, payable in advance once the authority requests it. Other federal states set their own fees. The Fachsprachprüfung costs 595 euros in Lower Saxony and 470 euros in Rhineland-Palatinate. Translations, the B2 language exam and the Approbation certificate are charged separately.

How many times can I take the Kenntnisprüfung?

The Kenntnisprüfung can be repeated twice, which gives you three attempts in total in 2026. If the exam is not passed, the whole exam is repeated, not single parts. The Fachsprachprüfung has no limit on attempts. Most candidates who prepare with a structured course and clinical work under a Berufserlaubnis plan for one attempt.

Does my USMLE score count towards the German Approbation?

A USMLE score does not replace any step of the German Approbation in 2026. The German state authority still requires the Fachsprachprüfung and the Kenntnisprüfung. The preparation, however, transfers almost completely: the clinical reasoning trained for USMLE Step 2 CK is exactly what the German examination board tests, only orally and in German.

When should Caribbean medical students start learning German?

Caribbean medical students who want the German Approbation should start learning German in the first year of basic sciences. Two to three lessons a week bring most students to B1 by the start of clinical rotations and to B2 by graduation. Arriving with B2 saves six to twelve months after the MD.

Which authority handles the Approbation for Caribbean graduates?

The Approbation for Caribbean graduates is handled by the competent state authority (Approbationsbehörde) of the federal state where the doctor plans to work. Each of the 16 German states has its own authority, for example NiZzA in Lower Saxony or the State Office for Health and Social Affairs in Berlin. The same authority organises the knowledge test.

Is the Approbation valid in all of Germany?

Yes. An Approbation issued by any German federal state is valid in all 16 states and has no time limit. Doctors who pass the Kenntnisprüfung in one state can start residency (Weiterbildung) anywhere in Germany without a new procedure. The Approbation also forms the basis for specialist training that is recognised across Europe.
