Short answer

Are Caribbean medical schools good? Yes: three schools secured 1,588 US residencies in 2026. Same USMLE, same licence, plus a route to Europe.

Are Caribbean medical schools good? Yes: in the 2026 Match, students and graduates of just three Caribbean schools, St. George's University, Ross University and the American University of the Caribbean, secured 1,588 US residency positions between them. They sat the same USMLE, matched through the same NRMP system and will hold the same US state licence as every other doctor in America.

The question usually comes with a second one attached: are Caribbean medical schools legitimate, and is a Caribbean medical school worth it compared with a place in the US or in Europe? The answers below rest on 2026 Match data, on how the licensing system works and on what our team at Medschool Experts sees when we plan these routes with students from Germany, the US, Canada and beyond.

One point surprises most families. A Caribbean MD does not only lead into American hospitals. With German at B2 level, the medical language exam and the knowledge test, the same degree leads to the German Approbation and a career in Europe.

Last reviewed: October 2026.

Are Caribbean medical schools good in 2026?

Caribbean medical schools are good in 2026 by the measure that matters most: what their graduates achieve. St. George's University reported 818 US residency matches by 23 March 2026, Ross University 475 and the American University of the Caribbean 295. All of them sat the same USMLE exams and will hold the same US state licence.

A medical school is as good as the doctors it sends into hospitals. By that yardstick the established Caribbean schools sit firmly inside the American system, not beside it. St. George's University (SGU) in Grenada has been the largest source of new doctors entering US residency for twelve consecutive years as of 2026, the year it celebrates its 50th anniversary. More than 33,000 SGU graduates now work as physicians, veterinarians, scientists and public health leaders.

Three schools. 1,588 new residents. One Match season.

The demand on the other side is just as concrete. The 2026 NRMP Main Residency Match was the largest on record: 44,344 training positions, and 38,354 applicants matched to a first-year (PGY-1) position. US hospitals need doctors, and a large share of those doctors arrive with an MD earned on a Caribbean island.

Are Caribbean medical schools legitimate?

Yes. The established Caribbean medical schools are legitimate, officially accredited institutions listed in the World Directory of Medical Schools (WDOMS). Since 2024, ECFMG certification requires a school accredited by an agency recognised by the World Federation for Medical Education, and the leading Caribbean medical schools meet that standard in 2026.

Legitimacy is visible in everyday paperwork. Graduates of these schools register with ECFMG, sit the USMLE, apply through ERAS and rank programs in the NRMP Match like everyone else. Their clinical rotations take place in US teaching hospitals that sign affiliation agreements with the school. Several of the large Caribbean schools are also approved for US federal student loans, a status the US Department of Education grants only after its own review.

There is a simple check anyone can run in five minutes. Search the school's name in WDOMS, then read its most recent residency list, which reputable schools publish every spring with hospital names and specialties. Lists with hundreds of named US hospitals speak for themselves. For a full overview of the schools that meet this standard, see our guide to accredited Caribbean medical schools and the complete list of Caribbean medical schools.

Do Caribbean medical graduates take the same USMLE and get the same licence?

Yes. Caribbean medical students sit the identical USMLE Step 1 and Step 2 CK as students in Boston or Chicago, at the same test centres, with the same scoring. After ECFMG certification they apply through ERAS, match through the NRMP and, after residency, receive the same unrestricted licence from the same US state medical boards.

The licence on the wall carries no footnote about where the MD was earned. A board-certified internist from Grenada and one from Ohio hold the same title, bill the same insurers and supervise the same residents. That equality is built into the system: the USMLE is the single licensing exam for every MD in the United States, and Step 1 has been scored pass/fail since January 2022, so the comparison happens on clinical performance and Step 2 CK.

The numbers on the ground confirm it. In 2024 the United States had 1,082,187 actively licensed physicians. 23 percent of them graduated from a medical school outside the US and Canada, and 5 percent were US citizens who studied medicine abroad, roughly 54,000 doctors. The full exam sequence is laid out in our USMLE and ECFMG certification pathway for 2026.

Where do Caribbean medical students complete their clinical rotations?

Caribbean medical students usually complete the basic sciences on the island and then spend their clinical years, core rotations and electives, at affiliated teaching hospitals in the United States. Some schools add sites in Canada or the United Kingdom. At UHSA in Antigua, students can complete one clinical year in the USA in 2026.

This structure is the quiet strength of the Caribbean model. A student who rotates through internal medicine in New York or surgery in Florida collects US clinical experience, US letters of recommendation and US references before the residency application even opens. Program directors read evaluations written by physicians they know. Many residency interviews start with a conversation about a rotation at a hospital down the road.

The rotation network differs from school to school, and that network is one of the first things we compare with each student. The details for each school are in our guide to Caribbean medical school clinical rotations.

Which specialties do Caribbean medical graduates match into?

Caribbean medical graduates match into the full range of specialties. SGU graduates entered 23 specialties in 2026, from internal medicine, pediatrics and psychiatry to surgery and dermatology. About two thirds of SGU graduates choose primary care, a field that offered 20,712 US residency positions in the 2026 Match, a record high.

Primary care is where the US system grows fastest, and Caribbean graduates are a large part of that growth. Internal medicine alone offered 11,632 first-year positions in 2026, 280 more than in 2025. Family medicine added 134 positions to reach 5,491. Psychiatry, emergency medicine and general surgery appear on Caribbean match lists as well.

How do US residency programs view a Caribbean MD in 2026?

US residency programs assess a Caribbean MD with the same criteria as any application: USMLE Step 2 CK, clinical evaluations, US letters of recommendation and the interview. In the 2026 Match, US-citizen international graduates matched at 70 percent, up 2.2 points from 2025, and Ross and AUC reported a combined 97 percent first-time residency attainment rate.

The 97 percent figure counts every graduate or expected graduate who took part in the 2026 Match or secured a position outside it. It shows what happens when a school prepares students well for the USMLE and places them in strong US rotations. The trend for US-citizen graduates of international schools points upward in 2026.

Inside many US teaching hospitals, the attending who signs the evaluation studied abroad too. That shared experience is part of the culture in internal medicine and family medicine. School-by-school results are in our analysis of Caribbean medical school residency match rates and the AUC residency match results 2026.

What do students ask most before choosing a Caribbean medical school?

Students who research Caribbean medical schools ask five questions again and again in 2026: whether they can return to practise in their home state, whether hospitals take the degree seriously, how island life affects studying, what the full program costs and whether Europe stays open. Each question has a clear, positive answer.

Can I practise back home? Yes. The US licence is issued by the state medical board after residency, and Caribbean graduates practise in states across the whole country. Many match into a program in their home state precisely because they rotated there.

Do hospitals respect the degree? They hire it every year. SGU alone placed residents in 42 states plus Washington, DC in 2026.

Is island life a distraction? The basic science semesters are intense and structured. Island campuses follow a small-campus rhythm: lectures, library, exam blocks. The beach is a ten-minute reward, not a lifestyle.

What does it cost? Tuition varies widely between schools. At our partner UHSA it is roughly 7,300 euros a year (8,500 USD) before our partner discount. A full comparison is in our Caribbean medical school cost guide and our article on financing and student loans.

Does Europe stay open? Yes, and that answer deserves its own section below.

Is Caribbean medical school worth it for students who also want Europe?

A Caribbean medical school is worth it for students who also want Europe because one MD serves two continents. The degree leads to the US Match and, for Germany, to licensure through a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung), followed by the Approbation. The German steps fit into the study years.

Few degrees open the American and the European medical systems at the same time. A Caribbean MD does. A student can sit USMLE Step 1 in year two, rotate in US hospitals, and learn German in the evenings and breaks. At graduation two doors stand open, and the student chooses the country instead of the country choosing the student.

Can you switch from a Caribbean medical school to a European university?

Yes. Students at a Caribbean medical school can transfer into a higher semester at a European university. Medschool Experts is the world's leading expert for this kind of transfer and frequently achieves entry into year 4 or year 5 at top universities. The outcome depends on the individual file and is assessed in a free consultation.

This is the follow-up question almost nobody answers before enrolment, and it changes the whole decision. Life moves. A partner takes a job in Vienna, a parent in Munich needs care, a student discovers a love for the European hospital culture during an elective. A Caribbean MD student is never locked into one continent. The credits earned on the island travel with the student.

We map that move with the student as a planned step, not an emergency. Our 25+ partner universities in Europe give us a wide choice of places, and the paperwork, from course descriptions to transcripts, is the part we handle every week. The step-by-step version is in switching from the Caribbean to Europe without losing time.

Which Caribbean medical school does Medschool Experts recommend in 2026?

Medschool Experts' official Caribbean partner in 2026 is the University of Health Sciences Antigua (UHSA) in Piccadilly, Antigua. Regular tuition is roughly 7,300 euros a year (8,500 USD) before our partner discount, one clinical year in the USA is possible, and a four-year graduate-entry track is open to applicants with a prior degree.

The graduate-entry track also suits applicants who have completed a year of biology, chemistry or physics at university. It shortens the route to the MD for career changers and science graduates, a group our team at Medschool Experts advises every month. Intakes run in January, May and September, so nobody waits a full year for a start date.

Applying through Medschool Experts brings a substantial tuition discount at UHSA that applicants do not receive when they apply on their own. With that discount, UHSA is the route we recommend to students who want a US-style MD at a price that keeps family finances calm. The school profile is on our UHSA Antigua university page and in the detailed UHSA guide.

The author's view

I started medicine abroad myself. In 2015, at the age of 24, I moved to Sofia to study at the Medical University's Faculty of Dental Medicine, older than most of my classmates and without a single contact in the country. I graduated in 2021 and am today a federally recognised dentist and the founder of Medschool Experts, based in Bern. That path taught me one thing early: where a degree comes from matters far less than what its holder does with it.

When families ask me whether Caribbean medical schools are good, I point to the hospital lists. Our team has placed more than 500 students at medical schools abroad, and in our own placement experience the students who do best are those who plan both routes from the first semester: USMLE preparation on one side, German lessons on the other. A Caribbean MD gives them that double option, and I know of very few degrees that do.

That is why we chose UHSA in Antigua as our Caribbean partner. The tuition is fair, the US clinical year is real and the graduate-entry track suits the late starters I identify with. If you are weighing this decision, talk to us before you sign anything. We will tell you plainly which route fits your file.

Your Caribbean MD also leads to Europe

A Caribbean MD is a passport to two medical systems. In the United States it leads through the USMLE, ECFMG certification and the NRMP Match to residency and a state licence. In Germany it leads through a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung) to the Approbation, the full licence to practise medicine. Both language steps fit into the study years, so a graduate can start in either system without a gap.

And if life points to Europe before graduation, a transfer into a higher semester at a European university is possible. Medschool Experts plans both routes with the student: the school choice, the application, the partner discount at UHSA, a German preparation plan, the European recognition steps or the transfer to one of our 25+ partner universities in Europe. Details for Germany are in Caribbean medical degree in Germany and the EU and the knowledge test for Caribbean graduates.

Summary

  • Caribbean medical schools are good: SGU, Ross and AUC secured 1,588 US residency positions in the 2026 Match.
  • The established schools are legitimate, officially accredited and listed in the World Directory of Medical Schools.
  • Graduates take the same USMLE, receive ECFMG certification and hold the same US state licence.
  • US-citizen international graduates matched at 70 percent in 2026, up 2.2 points; Ross and AUC reported 97 percent first-time residency attainment.
  • Clinical years take place in affiliated US teaching hospitals; UHSA offers one clinical year in the USA.
  • The same MD leads to the German Approbation via B2, Fachsprachprüfung and Kenntnisprüfung.
  • A transfer to a European university in a higher semester is possible; Medschool Experts frequently achieves entry into year 4 or year 5.
  • Our partner UHSA in Antigua costs roughly 7,300 euros a year before our partner discount.

Conclusion

Caribbean medical schools are good, legitimate and worth it for students who want a US-style MD with US clinical years and an open door to Europe. The evidence sits in hospital rosters across 42 states, in licences that look exactly like everyone else's and in a growing number of doctors who use the same degree in Germany.

The right school depends on your goals, your budget and the continent you picture in ten years. That plan is what we build with you. Book a free consultation with Medschool Experts and we will map your route, from the first semester in Antigua to the licence in the country you choose.
