Caribbean Medical School After High School: The 5-Year MD Track 2026
14.10.2026

Short answer
Caribbean medical school after high school: a premed year plus the 4-year MD takes you from school to Doctor of Medicine at about 23, 3 years faster.
Can you go to a Caribbean medical school after high school?
Yes. Caribbean medical schools accept school leavers into a premedical program that leads directly into the four-year MD. In 2026, students from the USA, Canada and Europe use this five-year route to earn a Doctor of Medicine without first completing a bachelor degree, which the US medical school system normally requires.
The idea is simple. American medical schools are graduate schools: you finish four years of college, then you apply. Caribbean medical schools follow the US curriculum and prepare students for the same licensing exams, but they also open a door one step earlier. The premedical phase replaces the college science years with a focused block of biology, chemistry and physics.
That changes the maths of a medical career. Four years of college become roughly one. For an 18-year-old, that is three years of life back.
For an overview of the region, its schools and the general route to practice, start with the pillar guide Study Medicine in the Caribbean 2026 and the complete list of Caribbean medical schools.
How does a premed program in the Caribbean work?
A premed program in the Caribbean is a foundation phase of roughly one to one and a half years that teaches the natural sciences a US medical school expects from college graduates. After the premedical phase, students move straight into the basic science semesters of the MD at the same university, without a separate bachelor degree.
The content is concentrated. General and organic chemistry, cell and molecular biology, physics for medicine, biochemistry foundations, mathematics and statistics, plus communication and ethics. The aim is not breadth but readiness: by the end, a student should be able to follow lectures in anatomy, physiology and biochemistry from the first day of the MD.
Many schools structure the premed phase in semesters that match their MD intakes in January, May and September. Some offer it on the island campus, others partly online. The format differs from school to school, and so does the length.
One example from the large schools: St. George's University in Grenada describes a seven-year MD track for students with secondary school credentials, with three preclinical years of sciences, social sciences and humanities, two years of basic medical sciences in Grenada and two clinical years at affiliated hospitals in the USA or the UK. SGU also lists five-year and six-year tracks. Shorter premedical phases, as at our partner UHSA, lead to the MD in about five years.
What does a 5 year MD program in the Caribbean look like?
A five-year MD program in the Caribbean combines one premedical year with the standard four-year Doctor of Medicine: about two years of basic sciences on the island campus and two years of clinical rotations at teaching hospitals, often in the USA. The degree at the end is the same MD that graduate-entry students earn.
| Phase | Typical length | Where | Content |
|---|---|---|---|
| Premedical year / foundation | about 1 year (some schools 3 semesters) | Island campus or partly online | Biology, general and organic chemistry, physics, biochemistry basics, maths |
| Basic sciences (MD years 1 and 2) | about 2 years | Island campus | Anatomy, physiology, biochemistry, pathology, pharmacology, microbiology, clinical skills |
| USMLE Step 1 | end of basic sciences | Testing centre | Pass/fail licensing exam for the US route |
| Clinical rotations (MD years 3 and 4) | about 2 years | Affiliated teaching hospitals, e.g. in the USA | Internal medicine, surgery, paediatrics, obstetrics and gynaecology, psychiatry, family medicine, electives |
| Graduation | after about 5 years in total | Doctor of Medicine (MD) |
The rhythm is fast. Caribbean schools teach in trimesters or with short breaks, which is one reason the basic sciences fit into about two calendar years. Students who arrive straight from school notice the tempo in the first weeks. It settles.
For the clinical years, the location matters most for the US route: rotations at American teaching hospitals bring US letters of recommendation and familiarity with the hospital system. The deep dive on this phase is in Caribbean medical school clinical rotations.
UHSA Antigua: a five-year route from school to MD
The University of Health Sciences Antigua (UHSA) in Piccadilly, Antigua, offers a premedical phase that leads into its four-year MD, so school leavers reach the Doctor of Medicine in about five years. UHSA is the official partner of Medschool Experts, and students who apply through us receive a substantial tuition discount.
UHSA was founded in 1982 and teaches the basic sciences on its campus at Dow's Hill near Falmouth and English Harbour, about 19 kilometres from St. John's. The clinical phase runs at affiliated hospitals, and one clinical year in the USA is possible. Students with a prior degree or a completed year of biology, chemistry and physics can enter the four-year graduate-entry track directly instead.
Regular tuition is roughly 7,300 euros a year (8,500 USD) before our partner discount. That is a fraction of what many larger Caribbean schools charge per semester. All details about campus, curriculum and recognition are on our UHSA university page and in the island guide Medical studies in Antigua.
Who decides which track fits? Whether you start in the premedical phase or directly in the MD depends on your individual school record. We check it in a free consultation and tell you which intake and which track make sense. Book your free consultation.
How old are you when you graduate? Five-year Caribbean track vs the US 4+4 route
A school leaver who starts a five-year Caribbean MD track at 18 graduates as a Doctor of Medicine at about 23. On the US route, four years of college plus four years of medical school lead to the MD at about 26 at the earliest. A six-year European medical programme ends at about 24.
Those three years are not a detail. Residency follows the MD on every route: three years for internal medicine or family medicine in the USA, five for general surgery, about five to six years of specialist training in Germany. A doctor who starts residency at 23 or 24 is a fully trained specialist before many peers on the 4+4 route have finished their own training.
Age also matters the other way round. Marcel Kloos, founder of Medschool Experts, started his own medical studies abroad in Sofia in 2015 at the age of 24, and graduated in 2021. He was a late starter. A school leaver on the five-year Caribbean track would have been finished at that age.
A side-by-side view of both systems is in Caribbean medical school vs US medical school.
Medical school without a bachelor's degree in the Caribbean: who uses this route?
Medical school without a bachelor's degree in the Caribbean is used by school leavers from the USA and Canada who want to skip four years of college, and by European school leavers with a German Abitur, A-levels or the International Baccalaureate who want an MD taught in English with access to the US and the European job market.
| School leaver from | Typical age at school exit | Earliest MD on the five-year track | What the route adds |
|---|---|---|---|
| USA (high school diploma) | 18 | about 23 | Skips college, same USMLE route as US students |
| Canada (secondary school diploma) | 17 to 18 | about 22 to 23 | Return route via USMLE and ECFMG certification |
| Germany (Abitur) | 18 to 19 | about 23 to 24 | English-taught MD, USA or return to Germany |
| UK (A-levels) | 18 | about 23 | US route plus European options |
| International Baccalaureate (IB) | 18 | about 23 | Recognised school-leaving qualification worldwide |
The European group is growing. German school leavers in particular combine the Caribbean MD with a clear plan for the way home: German language certificates during the studies, then the knowledge test (Kenntnisprüfung) and the Approbation. British pupils use the route for the US match and the European options together.
Canadian students find a dedicated guide in Caribbean medical schools for Canadian students, and European school leavers in Caribbean medical school for European students. Whether your school-leaving certificate leads into the premedical phase or directly into the MD is decided individually; we check this with you in a free consultation.
What does a Caribbean medical school straight out of high school cost?
At our partner UHSA in Antigua, tuition is roughly 7,300 euros a year (8,500 USD) before the Medschool Experts partner discount, so five years of tuition come to around 36,500 euros before the discount. Housing on Antigua, flights, exam fees and the clinical years abroad come on top and depend on the student.
The real saving sits in the structure. A school leaver on the US route pays four years of college before the first day of medical school. On the five-year Caribbean track, the premedical year replaces those four years, and the student starts earning a resident salary about three years earlier.
| Cost item | Five-year Caribbean track (UHSA) | US 4+4 route |
|---|---|---|
| Years of study until MD | about 5 | 8 |
| Tuition before the MD | roughly 7,300 euros a year before partner discount | four years of college tuition plus four years of medical school |
| Earliest start of residency salary | about age 23 | about age 26 |
| Bachelor degree required | No | Yes |

{{cta}}Route one: US residency after a five-year Caribbean MD
A five-year Caribbean MD leads to a US residency through USMLE Step 1 and Step 2 CK, ECFMG certification, an application through ERAS and the NRMP Match. In the 2026 Main Residency Match, 70.0 percent of US-citizen international medical graduates matched, and 9,682 international medical graduates in total started first-year positions.
The 2026 Match was the largest on record, with 44,344 positions. Graduates of the five-year track enter that system on exactly the same terms as graduate-entry students from the same school: same exams, same certification, same licence in the state of their residency.
The school leaver has one advantage that rarely gets mentioned. Starting at 18 leaves time. A student who wants research experience, an extra elective or a strong Step 2 CK preparation can take it without arriving at residency in their thirties.

Route two: Europe and Germany after a Caribbean MD
A Caribbean MD also leads to Europe. For Germany, graduates need a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung), followed by the Approbation, the full German medical licence. The language steps can be completed during the five years of study.
| Step | US route | Europe route (Germany) |
|---|---|---|
| Degree | Caribbean MD | Caribbean MD |
| Language | English (taught in English) | German B2, then medical language exam (Fachsprachprüfung) |
| Licensing exams | USMLE Step 1 and Step 2 CK | Knowledge test (Kenntnisprüfung) |
| Certification / licence | ECFMG certification, state licence | Approbation |
| Access to training | ERAS application and NRMP Match | Direct application to hospitals for specialist training |
| Can be prepared during studies | USMLE preparation | German courses and B2 certificate |
The Europe route has a second form. Students who decide during their studies that they want to finish in Europe can transfer into a higher semester at an EU university. Medschool Experts is the world's leading expert for this kind of transfer and frequently achieves entry into year 4 or year 5 at top universities; the outcome depends on the individual file, which we review in a free consultation.
The full explanation is in Caribbean medical degree in Europe, the knowledge test for Caribbean graduates in Germany and learning German during Caribbean medical school.
Do you have to choose between the USA and Europe at 18?
No. A school leaver who starts a five-year Caribbean MD track does not have to decide between a US residency and a career in Europe at 18. The first three years are identical for both routes, and the decision usually becomes concrete during the clinical years, when USMLE results and language progress are known.
Families raise this question in our consultations again and again. At 18, nobody knows where they want to live at 25. The five-year track keeps both doors open: USMLE Step 1 is useful on both routes as proof of knowledge, and German lessons from the first year cost little time in the premedical phase.
Our practical advice: plan both routes from day one, then let the results decide. A student who reaches B2 German by the end of the basic sciences and passes Step 1 has a genuine choice in year four. That is a strong position for a 21-year-old.
The dual strategy is described step by step in Caribbean MD: USA or Europe, the dual pathway.
How do you plan the premed year and the German language plan in parallel?
A school leaver on a Caribbean premed program can start German in the first year, aim for B2 by the end of the basic sciences, and prepare the medical language exam during the clinical years. Medschool Experts builds this plan together with the school choice, so the Europe route is ready when the MD is finished.
The premedical year is the best moment to start. The workload is lighter than in the anatomy semesters, and a few hours of German per week add up to a solid A2 or B1 level within a year. Online courses fit around trimester schedules.
We set the plan up with each student: school and intake (January, May or September), the application with the UHSA partner discount, the German preparation plan, and later the European recognition steps or a transfer to one of our 25+ partner universities in Europe.
The author's view
I started my own medical studies abroad in Sofia in 2015, at 24. A late starter. I graduated in 2021, at an age when a school leaver on the five-year Caribbean track would already be in the middle of residency. Looking back, I would always choose the earlier start.
That is why I like the five-year Caribbean track. It gives an 18-year-old an English-taught MD, a direct line to the US Match and, with German learned on the side, the full route back to Europe. In our placement work with 500+ students, the families who plan both routes from the first semester are the ones who later have the most choice.
My advice: do not wait four years for a bachelor degree you do not need. Talk to us, and we will find the track that fits your school record.
Your Caribbean MD also leads to Europe
A Caribbean MD earned on the five-year track is not a one-way ticket to the USA. With a German B2 certificate, the medical language exam (Fachsprachprüfung) and the knowledge test (Kenntnisprüfung), graduates receive the Approbation and work as doctors in Germany. All of this is clearly achievable, and the language steps fit into the years of study. A transfer into a higher semester at an EU university is a second option. Read more in Caribbean medical degree in Europe.
Summary
- Caribbean medical schools accept school leavers into a premedical program followed by the four-year MD.
- The five-year track leads from high school to the MD at about 23, roughly three years earlier than the US 4+4 route.
- School leavers from the USA, Canada and Europe (Abitur, A-levels, IB) use the route.
- Our partner UHSA in Antigua charges roughly 7,300 euros a year before the Medschool Experts partner discount.
- Route one: USMLE, ECFMG certification, ERAS, NRMP Match. In 2026, 70.0 percent of US-citizen IMGs matched.
- Route two: German B2, Fachsprachprüfung, Kenntnisprüfung, Approbation, or a transfer to an EU university.
- The decision between the USA and Europe can wait until the clinical years.
Conclusion
The Caribbean five-year track is one of the shortest English-taught ways from a school-leaving certificate to a Doctor of Medicine with access to the US Match. It saves years, keeps costs predictable at a partner school like UHSA, and leaves the door to Europe open from the first semester to the last.

FAQ: Caribbean medical school after high school
Can I go to a Caribbean medical school straight out of high school?
Yes. Many Caribbean medical schools run a premedical program for school leavers that leads directly into their four-year MD. Students from the USA, Canada and Europe start this way without a bachelor degree. Whether you begin in the premedical phase or directly in the MD depends on your individual school record, which Medschool Experts reviews with you in a free consultation.
How long is a Caribbean MD program after high school?
A Caribbean MD program after high school usually takes about five years: one premedical year, about two years of basic sciences on the island campus and about two years of clinical rotations at teaching hospitals. Some schools offer longer tracks. St. George's University in Grenada, for example, lists five-year, six-year and seven-year MD tracks.
Does UHSA Antigua accept high school graduates?
The University of Health Sciences Antigua (UHSA) offers a premedical phase that leads into its four-year MD, so school leavers reach the Doctor of Medicine in about five years. UHSA is the official partner of Medschool Experts. Tuition is roughly 7,300 euros a year before the partner discount that students receive when they apply through Medschool Experts.
Is a Caribbean MD earned after a premed year the same degree?
Yes. A student who enters a Caribbean medical school through the premedical year graduates with the same Doctor of Medicine as a graduate-entry student of the same school. The degree leads to the same USMLE exams, the same ECFMG certification and the same NRMP Match, and in Germany to the knowledge test (Kenntnisprüfung) and the Approbation.
Can German Abitur or A-level holders study medicine in the Caribbean?
Yes. School leavers with a German Abitur, British A-levels or the International Baccalaureate use the Caribbean five-year track to earn an English-taught MD. Many combine it with German language preparation during the studies, so both the US route and the return to Germany via Kenntnisprüfung and Approbation stay open. The right track is decided individually in a free consultation.
How old am I when I finish a Caribbean five-year MD?
A student who starts a five-year Caribbean MD track at 18 graduates at about 23. On the US route, four years of college plus four years of medical school lead to the MD at about 26 at the earliest. A six-year European medical programme started at 18 ends at about 24.
Which intakes does a Caribbean premed program have?
Many Caribbean medical schools start new students in January, May and September, and their premedical phases follow the same calendar. This means a school leaver does not have to wait a full year after graduation. Which intake fits your situation is something Medschool Experts clarifies with you in a free consultation.
Can I switch to a European university later?
Yes. Students on a Caribbean MD track can transfer into a higher semester at an EU university. Medschool Experts is the world's leading expert for this kind of transfer and frequently achieves entry into year 4 or year 5 at top universities. The outcome depends on the individual file, which we review in a free consultation.
