Getting started

Can US Americans study medicine abroad and still practice in the United States?

Yes. Thousands of US Americans do exactly this every year. If your school is listed in the World Directory of Medical Schools with an ECFMG Sponsor Note, you take USMLE Step 1 and Step 2 CK, earn ECFMG certification, match into a US residency and then receive your state license, exactly like graduates of US schools.

Why do so many US Americans choose medical school abroad?

US medical schools accept only about four in ten applicants, most require a bachelor's degree first and total costs often exceed €250,000. Abroad you can start right after high school, skip the MCAT at many schools, pay a fraction of US tuition and still return to the US for residency.

What is a US-IMG?

A US-IMG is a US citizen or permanent resident who graduated from a medical school outside the United States and Canada. As a US-IMG you need no visa for residency, which makes you more attractive to many programs than non-US international graduates.

Is studying medicine abroad a shortcut or a second-class route?

Neither. You complete a full medical degree, pass the same USMLE exams as US students and train in the same residency programs. Many US-IMGs work as attending physicians, program directors and faculty at US university hospitals.

How long does it take to become a doctor this way?

In Europe the medical program takes six years after high school. In the Caribbean it is usually a one- to two-year pre-med or basic science track plus four years of MD. After that, residency takes three to seven years depending on the specialty.

Is there an age limit?

No. Universities abroad admit students of every age. Many of our students start at 18, others at 25, 30 or later after a first career. What counts is your school record, your science background and your motivation.

Can I study dentistry, veterinary medicine or pharmacy abroad as well?

Yes. We place students in medicine, dentistry, veterinary medicine and pharmacy. Each profession has its own US licensing route, which we explain further down on this page.

Where to study

Which countries are most popular with US Americans?

The Caribbean, Poland, the Czech Republic, Hungary, Cyprus and Ireland lead the list. Bosnia and Herzegovina, Slovakia, Lithuania, Romania and Bulgaria are growing fast because they combine English-taught programs with moderate tuition.

Why do you recommend the Czech Republic?

Charles University was founded in 1348, its accreditor is recognized by WFME until January 2035 and the Czech Republic is on the NCFMEA comparability list. Tuition starts at about €12,500 per year, and at our official contract partner in Hradec Králové you can receive your admission through us as early as November.

What makes Hradec Králové special?

The Faculty of Medicine in Hradec Králové belongs to Charles University, the oldest university in Central Europe. It teaches the full six-year program in English, trains in a large university hospital and is our official contract partner, so you apply through us with a direct line to the admissions office.

Why is Poland so popular with US Americans?

Poland has a long tradition of English-taught medical programs for North American students, a well-organised six-year curriculum, clinical training in large university hospitals and tuition of roughly €11,000 to €16,000 per year. Several Polish schools run US-focused tracks with USMLE preparation.

What about Hungary?

Hungary offers some of the oldest English-taught medical programs in Europe, with large international student communities in Budapest, Pécs, Szeged and Debrecen. Tuition is typically €16,000 to €20,000 per year.

Should I choose the Caribbean or Europe?

The Caribbean is built around the US system: US-style curriculum, clinical rotations in US hospitals and strong USMLE focus. Europe gives you a full six-year degree straight after high school, lower total cost and a degree that is also valid across the EU. We compare both for your profile in a free consultation.

Which Caribbean schools are best for US Americans?

Look for schools accredited by CAAM-HP or ACCM, clinical rotations in ACGME-affiliated US hospitals, high USMLE pass rates and participation in US federal student loans. Our partner University of Health Sciences Antigua offers one clinical year in the USA. See all options on Caribbean medical schools.

Can I study in Western Europe, for example Germany or Ireland?

Ireland has English-taught programs with high tuition. Germany teaches medicine in German and admits few international students through a very competitive process. For most US Americans, Central and Eastern Europe offer the best balance of quality, cost and admission chances.

Which schools abroad are USMLE-eligible?

Every school listed in the World Directory of Medical Schools with an ECFMG Sponsor Note qualifies its graduates for the USMLE. We only recommend schools that meet this requirement and check it again for every university on your shortlist.

How do I find the right university?

Start with your budget, your preferred start date, your school record and whether you want a US-style or a European program. Browse our university finder or talk to Marcel or another experienced consultant on our team for a personal shortlist.

Admission requirements

Is my US high school diploma enough?

In most cases, yes. Some universities also look at your science grades, AP or SAT results, or hold a short interview. We check your documents before you apply and tell you where your profile fits best.

Do I need a bachelor's degree or the MCAT?

Not for the six-year programs in the Czech Republic, Poland and Hungary: they start right after high school and admit you on your school record. Caribbean schools and four-year tracks usually expect pre-med coursework. If you already hold a degree, see graduate-entry medicine.

Which high school subjects matter most?

Biology and chemistry are the most important, followed by physics and mathematics. AP Biology, AP Chemistry or college-level science courses strengthen your application noticeably.

Is there an entrance exam?

Some universities hold a short written test in biology and chemistry, sometimes with physics, others admit on documents and an interview only. Many exams take place online. We tell you exactly what each university on your shortlist expects.

What GPA do I need?

There is no universal cut-off. Many European universities accept solid B-level science grades, while more selective faculties expect stronger results. Contact us and we check your transcript individually through our partner network.

Do I need to prove my English?

As a native speaker from a US high school you usually need no TOEFL or IELTS. Your diploma is accepted as proof of English.

What documents do I need?

Typically your high school diploma and transcript, a passport copy, a medical certificate and sometimes an apostille. The exact list differs by university. We prepare it with you and take care of the paperwork.

What is an apostille and do I need one?

An apostille is an official certificate that confirms a US document for use abroad. Many European universities require it for your diploma. It is issued by the Secretary of State of the state where your document was issued.

Can I apply if I am still in 12th grade?

Yes. Many universities accept applications with your latest transcript and admit you conditionally until your diploma arrives.

Can I apply with a GED or homeschool diploma?

Often yes. A number of universities accept a GED or an accredited homeschool diploma, sometimes combined with an entrance test. We check your case individually.

I already studied pre-med or biology. Does that help?

Yes. Science coursework strengthens your application, and a completed degree can open four-year tracks or credit transfer. See graduate-entry medicine.

Application and timeline

When can I start?

European programs start in September or October, several Caribbean schools admit in January, May and September. Apply early: popular Czech and Polish faculties fill their English-taught classes months before the semester begins.

When should I apply?

Ideally between November and March for a September start in Europe. Many universities admit on a rolling basis, so early applicants get the widest choice. For the Caribbean, apply four to six months before your intake.

How long does the admission process take?

From the first consultation to your admission letter usually takes a few weeks. Visa and residence permit then take another one to three months, so plan early.

Can I apply to several universities at once?

Yes, and we recommend it. A shortlist of two to four universities raises your chances and gives you a real choice.

What happens after I receive my admission letter?

You pay the tuition deposit, apply for your student visa, arrange housing and health insurance and book your flight. We guide you through every step until you sit in your first lecture.

Is admission guaranteed?

Through our partner network we find a place for every suitable applicant. Which universities fit best depends on your documents, which we review before you apply.

Costs and financing

How much does medical school abroad cost?

Tuition ranges from about €4,000 per year in parts of Eastern Europe to €12,000 to €20,000 per year in the Czech Republic, Poland and Hungary. Leading Caribbean schools charge around €20,000 to €25,000 per semester. Our university finder shows tuition for every school.

How much are living costs?

In the Czech Republic, Poland, Hungary and Bosnia and Herzegovina you need about €600 to €1,000 per month for rent, food and transport. Capital cities are more expensive than smaller university towns.

How does the total cost compare with a US medical school?

A four-year US MD after a four-year bachelor's degree often exceeds €300,000 including living costs. A six-year program in Central Europe typically totals €100,000 to €160,000 including living costs, and you start earning as a resident up to two years earlier.

Can I use US federal student loans?

Participation in US federal student loan programs is decided school by school. Several schools in the Caribbean and Europe take part. We check this for every university on your shortlist and show you further options in our funding guide.

Can I use my 529 plan?

Yes, if the foreign university is eligible for US federal student aid and has a Federal School Code. Your 529 funds can then cover tuition, fees and qualifying housing costs.

Can I use the GI Bill?

Veterans and eligible dependents can use GI Bill benefits at foreign schools that are approved for VA education benefits. We check the approval status for your shortlist.

Are there scholarships?

Several universities offer merit scholarships or tuition reductions from the second year for top students. Private lenders and foundations provide additional funding. See our funding guide.

Is tuition fixed for the whole program?

Most European universities fix tuition for your cohort or raise it only moderately. We show you the exact terms before you sign.

How do I pay tuition?

Usually by international bank transfer in euros once or twice per year. Some schools accept payment in installments.

USMLE and ECFMG

What is the USMLE?

The United States Medical Licensing Examination is a three-step exam that every doctor needs for a US license. Step 1 covers the basic sciences, Step 2 CK covers clinical knowledge and Step 3 is usually taken during residency.

What is ECFMG certification?

ECFMG certification confirms that an international medical graduate is ready to enter US residency. You need it before you start residency. It requires USMLE Step 1, Step 2 CK, an English communication test in medicine and verification of your medical degree.

What does the ECFMG Sponsor Note mean?

The Sponsor Note in the World Directory of Medical Schools confirms that students and graduates of a school can apply for ECFMG certification and the USMLE. Without it, you cannot take the US route.

What is the WFME accreditation requirement?

Since 2024 you can only apply for ECFMG certification if your medical school is accredited by an agency recognized by the World Federation for Medical Education. We recommend exclusively schools that meet this rule.

When do I take USMLE Step 1 as a student abroad?

Usually after the basic science years: after year two in the Caribbean, after year three in six-year European programs. You can take it at Prometric test centers in many countries, including the US and across Europe.

Is Step 1 still scored?

No. Step 1 has been pass/fail since 2022. Step 2 CK is scored and therefore carries great weight in residency applications.

Do European programs prepare me for the USMLE?

The medical content is the same, but the exam format is American. Many of our students add a dedicated USMLE question bank from year two, and several partner universities offer USMLE preparation courses.

Can I take the USMLE while still in medical school?

Yes. Students can take Step 1 and Step 2 CK before graduation once their school confirms enrollment. Most students finish both before applying for residency.

Residency and the Match

How do I get a residency in the US?

You apply through ERAS in September, interview in the fall and winter, submit your rank list in February and receive your result on Match Day in March. Residency starts on July 1.

What are my chances in the Match as a US-IMG?

In recent Match years, around two thirds of US-IMGs matched into a first-year position. Strong Step 2 CK scores, US clinical experience and good letters of recommendation raise your chances significantly.

Which specialties are most accessible for US-IMGs?

Internal medicine, family medicine, pediatrics, psychiatry, neurology and pathology match the most international graduates. Surgery, radiology and anesthesiology are possible with outstanding scores and research.

Can I do clinical rotations in the United States?

Yes. Caribbean schools complete the clinical years in US hospitals, and our partner University of Health Sciences Antigua offers one clinical year in the USA. At European schools, strong students can arrange electives in the US, which count as valuable US clinical experience for the Match.

What is US clinical experience and why does it matter?

US clinical experience means hands-on work in a US hospital, for example electives or sub-internships. Program directors value it because it shows you know the US health system. Aim for at least eight to twelve weeks before you apply.

Do I need letters of recommendation from US doctors?

Most programs expect three or four letters, and letters from US physicians carry the most weight. US electives are the best way to earn them.

Can I apply to every residency program with a degree from abroad?

Your degree from abroad counts as an MD-equivalent. Since the single accreditation system started in 2020, all US residency programs are accredited by ACGME and open to MD, DO and international graduates alike.

Do I need a visa for residency?

No. As a US citizen or permanent resident you need no visa, which is a major advantage over non-US international graduates.

What if I do not match right away?

Many graduates match in their second attempt after gaining research experience or more US clinical work. The Supplemental Offer and Acceptance Program in Match Week also fills remaining positions.

License and career

How do I get my state medical license?

After passing USMLE Step 3 and completing the required years of residency, you apply to your state medical board. The license is the same as for US graduates.

Do international graduates need more residency years for a license?

In many states, yes: international graduates often need two or three years of US residency before a full license, while US graduates need one. After residency you practice without restriction.

Are there new state laws for internationally trained doctors?

Since 2023 many states have passed laws that create alternative license pathways for internationally trained physicians. These mostly target doctors who already practiced abroad. For students, the classic route through residency remains the standard.

Can I become board certified?

Yes. After residency you take the board exams of your specialty exactly like US graduates.

Can I work in Europe with my degree as well?

Yes. A degree from an EU country is recognized across the EU. To practice there you usually need the local language and, as a non-EU citizen, a work permit. Many of our graduates keep both options open.

Can I work in Canada, the UK or Australia later?

Each country has its own exam route for international graduates, for example the MCCQE in Canada or PLAB in the UK. A degree from a school listed in the World Directory of Medical Schools is the common starting point.

Can I practice in California?

Yes. California accepts graduates of schools listed in the World Directory of Medical Schools and requires the same steps as other states: USMLE, ECFMG certification and residency.

Life abroad

Do I need a visa to study in Europe?

For studies longer than 90 days you need a long-term student visa or residence permit of your host country. You apply for it after receiving your admission letter. We tell you exactly what to prepare.

Do I need to speak Czech, Polish or Hungarian?

Not for your studies, which are fully in English. Universities teach you the local language in the first years so you can talk to patients during clinical training.

Where do students live?

Most students rent shared apartments or studios close to the faculty, some live in university dormitories. We help you find accommodation before you arrive.

How safe are the university cities?

The Czech Republic, Poland, Hungary and many other host countries rank among the safest countries in the world.

Do I need health insurance?

Yes. International students need health insurance that is recognized in the host country. Many students combine a local student policy with coverage for trips home.

Can I open a bank account and get a phone contract?

Yes. With your residence permit you can open a local bank account and sign a phone contract. Many students also use a US account with low foreign transaction fees.

How often can I travel home?

Semester breaks in winter and summer give you several weeks at home each year. Direct flights connect Prague, Warsaw and Budapest with major US airports or with one short stop.

Can I work part-time during my studies?

Student visas allow limited part-time work in most countries, but medical school keeps you busy. Most students focus on their studies and electives.

Can my parents visit me?

Yes. US citizens can visit the Schengen area for up to 90 days without a visa.

Will I be the only American?

No. English-taught programs bring together students from the US, Canada, Scandinavia, Germany, the UK and many other countries. Several universities have large North American communities.

Dentistry, veterinary and pharmacy

Can I practice dentistry in the US with a dental degree from abroad?

Yes. Internationally trained dentists usually pass the INBDE and complete a two- to three-year advanced standing program at a US dental school, which awards a US DDS or DMD. A few states offer additional routes.

Can I practice as a veterinarian in the US after studying abroad?

Yes. Graduates of schools accredited by the AVMA follow the same path as US graduates. Graduates of other schools use the ECFVG or PAVE certification route before the national licensing exam.

Can I practice as a pharmacist in the US after studying abroad?

Yes. You apply for FPGEC certification, pass the FPGEE and a US internship, then take the national and state pharmacy exams.

Is it easier to get into dental school abroad than in the US?

Usually yes. European dental faculties admit students right after high school without the DAT and at much lower tuition than US dental schools.

Transfers and special cases

Can I transfer from a Caribbean school to Europe or the other way round?

Yes, in many cases. The receiving university evaluates your completed courses and places you in the matching year. See university transfer.

Can I transfer to a US medical school later?

Transfers into US medical schools are rare and very competitive. The reliable path back to the US is USMLE, ECFMG certification and residency.

I am a nurse, paramedic or physician assistant. Can I study medicine abroad?

Yes. Your clinical background is an advantage, and several universities value it in admission. We look at your qualifications individually.

I hold dual citizenship with an EU country. Does that help?

Yes. With EU citizenship you may qualify for EU tuition terms at some universities, need no student visa and can work in the EU without a permit after graduation.
